Total adenosine deaminase cases as an inflammatory biomarker of pleural effusion syndrome.

BACKGROUND: Although inflammatory diseases commonly affect the pleura and pleural space, their mechanisms of action remain unclear. The presence of several mediators emphasizes the concept of pleural inflammation. Adenosine deaminase (ADA) is an inflammatory mediator detected at increased levels in the pleural fluid. AIM: To determine the role of total pleural ADA (P-ADA) levels in the diagnosis of pleural inflammatory diseases. METHODS: 157 patients with inflammatory pleural effusion (exudates, n = 124, 79%) and non-inflammatory pleural effusion (transudates, n = 33, 21%) were included in t

Role of corticosteroids in the treatment of tuberculosis: an evidence-based update.

Corticosteroids are often used as an adjunct in the treatment of various forms of tuberculosis (TB) and for the prevention of complications, such as constrictive pericarditis, hydrocephalus, focal neurological deficits, pleural adhesions, and intestinal strictures. Notwithstanding, they have been proven in clinical trials to improve the following outcomes only--death or disability in human immunodeficiency virus (HIV)-seronegative patients with tubercular meningitis and tubercular pericarditis. Despite a lack of specific evidence for efficacy in HIV co-infected patients with tubercular meningi

Bacterial isolates from blood and lung aspirate cultures in Gambian children with lobar pneumonia.

As part of a study of the aetiology of pneumonia in malnourished and well nourished children, 100 children aged between 3 and 58 months with pneumonia were investigated using blood culture and culture of lung or pleural aspirates. In 44 children one species of bacterium was isolated from blood (6), lung culture (30), or both (8), while in eight children two species were isolated. In four of these eight children, one organism was isolated from blood culture, while a different organism was isolated from lung or pleural aspirate, indicating that blood culture does not always reflect the true aeti

Diagnostic Accuracy of Pleural Fluid Biomarkers and Nucleic Acid Amplification Tests in Tuberculous Pleural Effusion: An Umbrella Review.

INTRODUCTION/OBJECTIVE: Tuberculous Pleural Effusion (TPE) is a common but diagnostically challenging form of extrapulmonary tuberculosis. Many overlapping systematic reviews have evaluated pleural-fluid biomarkers and Nucleic Acid Amplification Tests (NAATs). This umbrella review aimed to synthesise and compare the diagnostic accuracy of pleural-fluid tests for TPE within a single appraised framework. METHODS: We conducted an umbrella review (JBI methodology; PRISMA 2020 reporting) of 20 systematic reviews with meta-analysis, appraised them using AMSTAR 2, quantified primary-study overlap wi

Probable Pleural-Based Pulmonary Tuberculoma Mimicking Lung Malignancy: A Case Report.

Pleural-based lung masses can mimic lung cancer and differentiating malignancy from other causes remains a diagnostic challenge. We report a case of a 50-year-old man who presented with an acute onset pleuritic chest pain and a history of weight loss. Chest radiography and computed tomography scan showed a solitary pleural-based lung lesion that was suspicious for malignancy. Two sessions of computed tomography guided biopsies yielded only necrotic tissue. An endobronchial ultrasound-guided transbronchial needle aspiration showed inflammatory cells but was non-diagnostic. Video-assisted thorac

Cytokine gene polymorphisms in Colombian patients with different clinical presentations of tuberculosis.

Tuberculosis (TB) has different clinical presentations. Pulmonary TB affects only the lungs and exhibits variable anti-mycobacterial immune responses. Pleural TB is a localized disease with a strong immune response. Miliary TB is a disseminated form with poor immune response. Cytokines play a pivotal role in anti-mycobacterial response and may determine the type of TB. Thus, gene polymorphisms associated with cytokine production may be associated with clinical presentations of TB. In this study, 54 tuberculin-negative healthy controls, 81 tuberculin-positive healthy controls, 140 patients with

Helper T cell type 1 and 2 cytokines regulate C-C chemokine expression in mouse pleural mesothelial cells.

The recruitment of leukocytes to an area of injury or inflammation site is one of the most fundamental host defenses. Pulmonary tuberculosis is characterized by granulomatous inflammation with an extensive infiltration of mononuclear cells. In tuberculous pleurisy pleural mesothelial cells are exposed to mycobacteria in the pleural space. In this study we demonstrate that mouse pleural mesothelial cells (PMCs), when stimulated with BCG or IFN-gamma, produced MIP-1alpha and MCP-1 in vitro. IFN-gamma enhanced the BCG-mediated MIP-1alpha and MCP-1 expression in a concentration-dependent manner. T

Reactivation disease: the commonest form of tuberculous pleural effusion in Edinburgh, 1980-1991.

From 1980-1991 82 (7.2%) of 1134 tuberculosis notifications in Edinburgh were for pleural effusion. Study of the available records of 62 cases satisfying defined diagnostic criteria identified 14 cases (6 M, 8 F) with a mean age of 27.6 years (range 11-51 years) of primary tuberculous effusion and 25 cases (21 M, 4 F) with a mean age of 51 years (range 19-79 years) with pleural effusion due to reactivation disease. Twenty-three patients (19 M, 4 F) with a mean age of 48.9 years (range 25-85 years) defied classification. Symptoms, associated and diagnostic test findings were similar in all thre

Yellow nail syndrome: missed diagnosis of a rare syndrome.

A 61 year old male, with a bilateral persistent and recurrent pleural effusion, had undergone frequent tapping over a period of eight months, prior to the referral. The patient was treated earlier to the referral empirically for pulmonary tuberculosis with no response to the treatment. Malignancy was suspected and ruled out. A detailed examination showed that he also had atrophic nails with yellow discoloration and lymphedema of feet. Yellow nail syndrome was diagnosed on the basis of the clinical findings of the triad viz. yellow atrophic nails, lymphedema and bilateral pleural effusions. Pat

Application of NGS in Diagnosis of Tuberculous Pleurisy with Multiple Negative Tests: A Case Report.

BACKGROUND: Tuberculous pleurisy is inflammation caused by direct infection of Mycobacterium tuberculosis (MTB) and/or delayed allergic reaction of the pleura to MTB thallus components. The diagnosis of tuberculous pleurisy is mainly confirmed by bacterial culture, smear staining or histopathology, but has some clinical limitations. Next-generation sequencing (NGS), as a new diagnostic technology, has good application prospects in the diagnosis of tuberculous pleurisy. CASE PRESENTATION: A patient admitted with right pleural effusion and pneumonia was actively treated with anti-infection, ant

ACE/ACE2 ratio and MMP-9 activity as potential biomarkers in tuberculous pleural effusions.

OBJECTIVE: Pleural effusion is common problem, but the rapid and reliable diagnosis for specific pathogenic effusions are lacking. This study aimed to identify the diagnosis based on clinical variables to differentiate pleural tuberculous exudates from other pleural effusions. We also investigated the role of renin-angiotensin system (RAS) and matrix metalloproteinase (MMPs) in the pathogenesis of pleural exudates. EXPERIMENTAL DESIGN: The major components in RAS and extracellular matrix metabolism, including angiotensin converting enzyme (ACE), ACE2, MMP-2 and MMP-9 activities, were measured

A modified Abrams needle biopsy technique.

STUDY OBJECTIVE: To compare the diagnostic sensitivity of a modified Abrams needle pleural biopsy technique (A1) with the standard Abrams (A2) and Cope needle biopsy methods. The modified Abrams pleural biopsy technique consisted of suctioning each tissue sample into a syringe without removing the needle completely from the chest until the completion of the entire procedure. Both the standard Abrams and Cope needle techniques required needle removal from the chest after each pleural biopsy. DESIGN: Retrospective chart analysis. SETTING: Community teaching hospital affiliated with Stanford Un

Clinical Characteristics of Pediatric Pleuropulmonary Paragonimiasis with Pleural Effusion: A Single-Center Retrospective Analysis.

Paragonimiasis, a cause of pleuropulmonary disease, is a neglected etiology of pediatric pleural effusion. We retrospectively analyzed children with paragonimiasis-induced pleural effusion admitted to our center, aiming to summarize their diagnosis, treatment, and prognosis. A total of 103 children were included, with a mean age of 8.2 years, and 87.4% resided in rural areas. 81/91 (89.0%) were initially misdiagnosed with other infectious diseases including community-acquired pneumonia (43.9%; 40/91) and tuberculosis (16.5%; 15/91). The predominant blood test findings included hypereosinophili

Meigs syndrome with pleural effusion as initial manifestation: A case report.

BACKGROUND: Meigs syndrome is a rare neoplastic disease characterized by the triad of benign solid ovarian tumor, ascites, and pleural effusion. In postmenopausal women with pleural effusions, ascites, elevated CA-125 level, and pelvic masses, the probability of disseminated disease is high. Nevertheless, the final diagnosis is based on its histopathologic features following surgical removal of a mass lesion. Here we describe a case of Meigs syndrome with pleural effusion as the initial manifestation. CASE SUMMARY: A 52-year-old woman presented with a 2-mo history of dry cough and oppression

Elevated Adenosine Deaminase in Pleural Effusion A Case of Non-Hodgkin Lymphoma Misdiagnosis.

UNLABELLED: Despite a recent decline, tuberculosis (TB) infection is still a frequent diagnosis in Portugal. Adenosine deaminase (ADA) measurement has become an important tool in the timely diagnosis of this infection. However, ADA elevation in bodily fluids is not pathognomonic of TB infection. We present the case of a 70-year-old woman, undergoing treatment for pleural TB, diagnosed based on elevated ADA levels in a pleural effusion. Due to worsening symptoms she was readmitted, and the previous diagnosis was reconsidered. Thoracocentesis was repeated and cytometry analysis of the fluid was

Epidemiologic evaluation of pleurisy diagnosed by surgical pleural biopsy using data from a nationwide administrative database.

BACKGROUND: Pleural biopsies for investigating the causes of pleurisy are performed through modalities including needle biopsies, local anesthetic thoracoscopic procedures, and surgery (video-assisted thoracoscopic surgery and open thoracotomy). To date, there have been no large-scale nationwide epidemiological studies regarding pleurisy diagnosed via surgical pleural biopsy. This study examined the epidemiology of pleurisy diagnosed via surgical pleural biopsy in a Japanese nationwide administrative database. METHODS: We evaluated Japanese Diagnosis Procedure Combination data of 24 173 patie

[Posterior thoracotomy in ventral decubitus].

The rigidity of the chest wall, thank to its bone framework, determines the variety of operative access in thoracic surgery, both thoracoscopic and open. The posterior thoracotomy on the bed of the resected rib in ventral decubitus is traditionally but gratuitously rarely used access. The method permits comfortable access to trachea, bifurcation, main bronchi and thoracic esophagus. It can also be used in cases of foregoing thoracothomy. Authors own the experience of 111 cases with the use of posterior thoracotomy in ventral decubitus. The access proved to be preferable for the operations on t

Diagnostic Value of CD25, CD69, and CD134 on Tuberculosis-Specific Antigen-Stimulated CD4+ T Cells for Tuberculous Pleurisy.

Rapid and accurate methods for the diagnosis of tuberculous pleurisy (TP) are urgently needed. Activation markers of tuberculosis (TB)-reactive T cells are considered promising for the diagnosis of active TB (ATB). Different activation indexes may play different roles in the progression of TB, but there are few reports on T cell activation indicators, except for HLA-DR. Hence, we evaluated the expression of early (CD25 and CD69) and late (CD134) activation markers on TB antigen-stimulated CD4+ T cells in populations with different TB infection status and investigated their diagnostic value for

Mycobacterium tuberculosis inducing disseminated intravascular coagulation.

Disseminated intravascular coagulation (DIC) can develop infrequently in patients with tuberculosis and has a very high mortality rate. We conducted a retrospective study to evaluate the incidence of tuberculosis-induced DIC and to investigate the clinical manifestation, outcome, and prognostic factors of such patients. From January 2002 to December 2003, all culture-proven tuberculosis patients who developed DIC before starting anti-tuberculosis treatments were selected for this study. Patients who had other clinical conditions or were infected by other pathogens that may have been responsibl
