Shockwave lithotripsy for kidney stones as a first-line therapy in children younger than 2 years.

BACKGROUND: The incidence of urolithiasis in children has risen worldwide over the last decades with geographical varieties. As pediatric patients begin forming stones earlier in life, they have high risk of recurrence. Extracorporeal shockwave lithotripsy (SWL) is a non-invasive treatment modality that is preferred in the management of pediatric stones. Reports about the safety and efficacy of SWL in patients younger than 24 months are scarce in the literature. OBJECTIVE: The aim of the study was to evaluate the effectiveness and safety of SWL in patients younger than 24 months. The authors …

Hereditary xanthinuria in a goat.

A 2-year-old mixed breed goat was presented for a 1-day history of anorexia and 1 week of weight loss. Serum biochemistry disclosed severe a … urs is the first report describing the clinical presentation and pathology associated with xanthine urolithiasis in a goat. The data support hereditary xanthinuria, but functional studies are needed to conclusively determine the causal variant(s).

The role of alpha-adrenoreceptors in the treatment of urological diseases.

One of the most important clinical pharmacological invention in the last decades the role of adrenoreceptors in urological disease. The disorders of emptying the bladder are associated to the urology. Plant extracts efficacy is low, indicated only in mild symptoms, surgery has to be performed in advanced cases. Recognise, discovering the role of adrenoreceptors in the prostate, bladder neck has changed the treatment of benign prostatic hyperplasia. Relieving the muscle tension leads to a better urinary flow, decreased residual urine and less complaints. Combination with 5alpha-reductase inhibi

An overview of the privileged synthetic heterocycles as urease enzyme inhibitors: Structure-activity relationship.

Urease is a metalloenzyme including two Ni2+ ions, found in some plants, bacteria, fungi, microorganisms, invertebrate animals, and animal tissues. Urease acts as a significant virulence factor, mainly in catheter blockage and infective urolithiasis as well as in the pathogenesis of gastric infection. Therefore, studies on urease lead to novel synthetic inhibitors. In this review, the synthesis and antiurease activities of a collection of privileged synthetic heterocycles such as (thio)barbiturate, (thio)urea, dihydropyrimidine, and triazol derivatives were described and discussed according to…

Anatrophic nephrolithotomy and pyelolithotomy for dual staghorn calculi in bifid collecting system.

A bifid ureter is an atypical anatomical variation that occurs with an incidence of 1-10%. This anomaly is in a continuum of duplex collecting systems and most commonly involves a common distal ureter. This is usually asymptomatic and is predominantly an incidental diagnosis, nevertheless, is a potential risk factor for urolithiasis formation. Current surgical management of larger staghorn calculi favours percutaneous nephrolithotomy (PCNL) over traditional open surgery, however for multiple calculi and complex anatomy PCNL would require multiple punctures, with increased risk of bleeding, ple…

[MINI-PERCUTANEOUS NEPHROLITHOTRIPSY – OUR FIRST EXPERIENCE].

Urolithiasis is a significant problem in the developed countries due to the increased number of patients with stones. Just a few decades ago open surgery was the only surgical treatment which is today, in most cases, replaced with minimally-invasive methods. One of these new methods is mini-percutaneous nephrolihotripsy. We present four patients in whom mini-percutaneous nephrolithotripsy was performed. In all patients the stone was located in the renal pelvis. In three patients the stone was in the native kidney and in one in the transplanted kidney. In all patients laser lithotripsy was succ…

Primary adenocarcinoma of the renal pelvis: histologic features of a stepwise process from intestinal hyperplasia to dysplasia in a patient with chronic renal abscess.

Pure adenocarcinomas of the urothelium are very rare and their location in the pelvis is uncommon. Although their pathogenesis is not well defined, adenocarcinomas are likely to originate from neoplastic transformation of the glandular cells of the urothelial intestinal metaplasia usually arising in response to chronic irritating stimuli, such as long-duration inflammation, urolithiasis, and hydronephrosis. We report a case of an 81-year-old woman who underwent right nephrectomy for relapsing renal abscess due to a staghorn calculus. Histological examination disclosed an infiltrating adenocarc…

Partial hypoxanthine-Guanine phosphoribosyltransferase deficiency as the unsuspected cause of renal disease spanning three generations: a cautionary tale.

Hypoxanthine-guanine phosphoribosyltransferase (HPRT) deficiency is an X-linked defect of purine metabolism. Clinical manifestations are usually related to the degree of enzyme deficiency: complete HPRT deficiency (Lesch-Nyhan syndrome) presenting with severe neurologic or renal symptoms, or partial HPRT deficiency (Kelley-Seegmiller syndrome) manifesting as a gout-urolithiasis syndrome. A 3-generation kindred is described in which the recognition of partial HPRT deficiency in 2 adolescent male siblings presenting with uric acid lithiasis led to the diagnosis in 2 maternal uncles already in re…

Prophylaxis of cystine calculi by low dose of alpha-mercapto-propionyl-glycine administered every other day.

In a case of typical cystinuria type I with left kidney hypoplasia and bilateral calculosis, calculi were dissolved by means of alpha-mercapto-propionyl-glycine and prophylaxis of the calculosis was performed by the same drug continuously administered in two divided doses (10-4 mg.kg-1.day-1) for 9 years, without urolithiasis. Then, the alpha-mercaptopropionyl-glycine every other day in one dose, 4 mg.kg-1.day-1, for two years was administered without calculosis. The arbitrary discontinuation of the drug by the patient was followed after 6 months from a right ureteral calculosis with transitor…

Laparoscopic pyelolithotomy.

Laparoscopic surgery has been applied to virtually every aspect of urinary tract disease. Presented is a case of laparoscopic-extended pyelo … tripsy and percutaneous and endoscopic stone techniques have drastically modified the management of urolithiasis. However, select cases in which these techniques may not be applicable (such as this toddler with bulky cystine lithiasis) may require open surgery. The laparoscopic approach represents an excellent, yet less-invasive option.

[Foreign body in the urinary bladder].

Foreign bodies in the urinary bladder are not uncommon, however, only a few cases have been reported in recent literature. This is not a fatal disease, however, it may lead to serious complications such as chronic cystitis, urolithiasis, or rectal abscess formation. These foreign bodies were inserted for autoerotic or unknown reasons by patients. In this paper we report 4 cases of foreign bodies in the urinary bladder. They include a cucumber, glass tube, chewing gum, and filliform catheter. We found that most of the foreign bodies in the urinary bladder can be removed endoscopically, but if t…

Magnesium and inflammatory bowel disease.

Mg deficiency is a frequent complication of inflammatory bowel disease (IBD) demonstrated in 13-88% of patients. Decreased oral intake, malabsorption and increased intestinal losses are the major causes of Mg deficiency. The complications of Mg deficiency include: cramps, bone pain, delirium, acute crises of tetany, fatigue, depression, cardiac abnormalities, urolithiasis, impaired healing and colonic motility disorders. Serum Mg is an insensitive index of Mg status in IBD. Twenty-four-hour urinary excretion of Mg is a sensitive index and should be monitored periodically. Parenteral Mg require…

Recurrent nephrolithiasis associated with keratinizing desquamative squamous metaplasia.

Keratinizing desquamative squamous metaplasia (KDSM) in the renal pelvis is a rare condition with unclear malignant potential. Recent reports suggest it is likely benign and favor endoscopic treatment approaches. Medical record review was completed on two cases at our center to obtain history, physical examination, radiographic findings, and management. A literature review was completed to identify all published cases of KDSM. Both patients at our center suffered recurrent urolithiasis, hypothesized to be secondary to KDSM. Both were managed with a percutaneous approach to ensure complete ston…

[Asymptomatic classical hereditary xanthinuria type 1].

We report on a girl who was diagnosed with classical hereditary xanthinuria due to an incidental finding of extremely low Levels of uric acid in the blood. The girl is compLetely asymptomatic. Hereditary xanthinuria is a rare autosomal recessive disease that usually causes early urolithiasis but may cause rheumatoid arthritis-like disease and even be associated with defects in the formation of bone, hair and teeth. In Israel it has mostly been described in patients of Bedouin origin. Throughout the world, only about 150 cases have been described; about two thirds of these patients were asympto…

The renal and urologic complications of inflammatory bowel disease.

: Extraintestinal manifestations of inflammatory bowel disease (IBD) are common. Clinically significant renal/urologic complications occur in 10-15% of patients with IBD. In this article, we review the incidence, pathogenesis, diagnosis, and management of the various renal/urologic complications. In general, renal/urologic complications in IBD may be directly related to the underlying intestinal disease process (acalculous hydronephrosis, fistula formation, abscess) related to metabolic consequences of the disease (urolithiasis) or to the medication used in the treatment of IBD. Less clearly u…

[Nephropathies in a region contaminated by heavy metal salts and the possibilities for therapeutic and prophylactic measures].

The study of the population in the region contaminated with heavy metal salts has revealed high incidence of nephropathies even in preschool children manifesting initially in the majority of cases with hematuria. All the patients had the signs of urinary dysembryogenesis and marked membranopathological process. Long-term exposure to even small doses of heavy metals is supposed to cause nephropathy. Urinary disease arose more frequently in those genetically predisposed to renal and urinary tract affections. Because urolithiasis is a frequent result of dismetabolic nephropathy in endemic regions…
