[Translabyrinthine--transtentorial approach for removal of large acoustic neuromas].

In 30 consecutive patients with large acoustic neuromas, which were more than 4.0 cm in diameter, tumor excision was performed by a one-stage combined translabyrinthine-transtentorial approach. The tumors were totally removed in 29 patients (96.7%), without death. The facial nerve was preserved anatomically in 16 patients (53.3%), and functionally in 11 (36.7%). Main advantages of this approach include: (1) direct approach with a more extensive exposure; (2) identification of both the origin of the facial nerve at the brain stem or in the internal auditory canal: (3) minimal postoperative reac

Normal hearing in acoustic neuroma patients: a critical evaluation.

A series of 128 acoustic neuromas (ANs) managed in our center in the last 6 years were evaluated. Sixteen cases (12.5%) were found to have normal pure tone audiograms. A comparatively younger age was an important characteristic of these patients. Tinnitus, dysequilibrium, and subjective hearing loss or distortion were their most common complaints. Pure tone audiometry showed a mean pure tone average of 14.5 dB. Speech discrimination score and performance intensity phonetically balanced index were very insensitive and showed very high false-negative results. Auditory brainstem responses proved

Acoustic parameters and speech analysis after supracricoid hemilaryngopharyngectomy.

Selected characteristics of speech and voice were compared in 10 patients who had undergone supracricoid hemilaryngopharyngectomy (SCHLP) and 20 normal adult laryngeal (NAL) speakers. Durational features of tape-recorded speech samples were measured using a stopwatch. Frequency features were analyzed with the Computerized Speech Lab and the multidimensional voice program. SCHLP speech proved comparable to NAL speech in average fundamental frequency, speech rate, and group phrasing. SCHLP speech and voice were statistically less efficient than NAL speech in fundamental frequency range, jitter,

Predictable and pseudo random saccades in patients with acoustic neuroma.

We studied predictive and pseudo random saccades in patients with cerebellopontine angle tumor. Target for the saccades was a laser beam which was reflected on the wall in front of the subject, and the stimulus was controlled by a computer. The fixed target was spaced at a distance of +/- 30 to each side of the subject. The random saccades jumped 20 degrees/40 degrees/60 degrees in an unpredictable manner. The saccade analysis program was based on pattern recognition. In the random saccade test none of the mean values of saccadic reaction time (SRT), saccadic accuracy (SA) and saccadic peak ve

Cerebrospinal fluid leak after acoustic neuroma surgery.

BACKGROUND: CSF rhinorrhea is a common complication of acoustic neuroma surgery. METHODS: Twenty cases of persistent cerebrospinal fluid rhinorrhea, 2 of transient leaks, and 2 of late meningitis occurring in a series of 156 posterior fossa acoustic neuroma removals were studied. Prophylaxis of leaks was usually attempted by filling opened air cells in the drilled internal auditory canal (IAC) with bone wax. Communications were repaired by rewaxing the IAC, or by fat packing of either the mastoid or the eustachian tube, middle ear, and mastoid. RESULTS: Most leaks were diagnosed within 1 wee

Effect of stress on cochlear glucocorticoid protein: acoustic stress.

Levels of glucocorticoid (GR) receptor protein were determined by a quantitative enzyme-linked immunosorbent assay (ELISA) technique in inner ear tissue of rats exposed daily to 85 dB SPL white noise for 4 hours on 3 consecutive days. GR levels in spiral ligament and organ of Corti tissues were detected using a monoclonal antibody to the GR receptor, BuGR2. A non-significant 13% decrease in GR levels of spiral ligament tissues was observed in the noise exposed animals relative to untreated animals. A statistically significant decrease of 27% in GR protein levels was seen in the organ of Corti

Acoustic prediction of severity in commonly occurring voice problems.

The relative effectiveness of three acoustic measures (jitter, shimmer, and harmonic/noise ratio) in predicting the dysphonic severity of a diverse clinical population singly and together was investigated. Phonatory samples were recorded from 20 normal subjects and 60 patients representing 3 laryngeal groups (nodules, paralysis, and functional). The phonatory samples were evaluated by 22 listeners using a 7-point equal-appearing interval scale. Shimmer produced a bivariate correlation of 0.54 with dysphonic severity; harmonic/noise ratio correlated -0.32 with dysphonic severity; and jitter pro

The effect of acoustic overexposure on the tonotopic organization of the nucleus magnocellularis.

We assessed the effect a sound-induced cochlear lesion had on the tonotopic organization of the nucleus magnocellularis (NM) immediately after acoustic overexposure and following a twelve day recovery period. The acoustic overexposure was a 0.9 kHz tone at 120 dB sound pressure level (SPL) for 48 h. Initially after the acoustic overexposure, the tonotopic organization of the NM was statistically different from that of age-matched controls. Specifically, it appeared that the center frequencies of units in the frequency region of the NM associated with the acoustic overexposure had higher center

Stability of hearing preservation following acoustic neuroma surgery.

Identification of small acoustic neuromas has become commonplace. Frequently, affected individuals are identified prior to the development of significant hearing loss. Whereas many studies have focused on hearing preservation surgery, few have reported on stability of hearing results after resection of acoustic neuroma. Between 1985 and 1991, 36 patients underwent resection of an acoustic neuroma via a retrosigmoid, internal auditory canal approach with attempted hearing preservation. Hearing was preserved in 24 patients; 17 were available for testing for the present study. Follow-up ranged fr

Vulnerability of hearing function during acoustic neuroma surgery.

Intraoperative monitoring of auditory evoked potentials reveals possible correlations between surgical maneuvers and the function of auditory structures and pathways in cerebellopontine angle surgery. Direct cochlear nerve action potentials (CNAPS) furnish realtime information on the function of the cochlea and cochlear nerve and were utilized in 14 patients undergoing removal of small (12-18 mm) unilateral acoustic tumors. The latency of the first negative component (N1) of the CNAPs proved extremely sensitive in detecting intraoperative auditory damage and was a good predictive index of post

The choice of approach in surgery for acoustic neuromas (vestibular schwannomas).

The advantages and disadvantages of the different surgical approaches for surgery of vestibular schwannomas are described, as well as the problems in evaluating the outcome of the treatment, regarding especially the evaluation of facial nerve function and hearing capacity. The eclectic Copenhagen treatment algorithm is outlined: 1) All tumors measuring 25 mm or more on MRI are operated via the translabyrinthine approach. 2) All patients with PTA poorer than 30 dB, and SDS poorer than 70% are operated via the translabyrinthine approach. 3) Tumors less than 10mm extrameatally, and PTA better tha

Facial nerve outcome after acoustic neuroma surgery.

Factors influencing facial nerve preservation and function in acoustic neuroma surgery were studied in 108 cases. Anatomic preservation of the nerve was inversely related to tumor size and improved as the series progressed. When the nerve was saved, normal postoperative function was inversely related to tumor size and was more common if the cochlear nerve was also saved. Most intact nerves eventually recovered some function, but late function was seldom completely normal unless there was some early recovery. The results demonstrate the importance of tumor size, operator experience, and ease of

Acoustic rhinometry in the evaluation of children with nasal or oral respiration.

The purpose of this study was to determine if there are any differences in the nasal geometry of children with nasal respiration or predominantly oral respiration (mouth breathing). The technique utilized was the acoustic reflection technique or acoustic rhinometry. Acoustic rhinometry is a non-invasive method, which can be easily applied to young children. Twenty children participated and both nostrils were evaluated. Statistically significant differences were found in the total nasal volumes between the two groups, with the mouth breathers presenting with reduced nasal volumes. No difference
