Paliperidone extended-release in schizoaffective disorder: a randomized, controlled study comparing a flexible dose with placebo in patients treated with and without antidepressants and/or mood stabilizers.

This 6-week, double-blind, placebo-controlled study evaluated paliperidone extended-release (ER) as both monotherapy and adjunctive therapy to mood stabilizers and/or antidepressants (MS/ADs) for schizoaffective disorder. Included subjects had a schizoaffective disorder diagnosis; a Positive and Negative Syndrome Scale (PANSS) total score of 60 or higher; a score of 4 or higher on 2 or more of the PANSS items for hostility, excitement, tension, uncooperativeness, or poor impulse control; and prominent mood symptoms (≥16 on the Young Mania Rating Scale and/or the 21-item Hamilton Rating Scale f

Association of DISC1 with autism and Asperger syndrome.

The DISC1 gene at 1q42 has generated considerable interest in various psychiatric diseases, since a balanced translocation interrupting the gene was found to cosegregate with schizophrenia and related mental illnesses in a large Scottish pedigree. To date, linkage and association findings to this locus have been replicated in several study samples ascertained for psychotic disorders. However, the biological function of DISC1 in neuronal development would suggest a potential role for this gene also in other, early onset neuropsychiatric disorders. Here we have addressed the allelic diversity of

A Scoping Review of Interventions Designed to Support Parents With Mental Illness That Would Be Appropriate for Parents With Psychosis.

The experience of psychosis can present additional difficulties for parents, over and above the normal challenges of parenting. Although there is evidence about parenting interventions specifically targeted at parents with affective disorders, anxiety, and borderline personality disorder, there is currently limited evidence for parents with psychotic disorders. It is not yet known what, if any, interventions exist for this population, or what kinds of evaluations have been conducted. To address this, we conducted a scoping review to determine (1) what parenting interventions have been develope

Drug detection in a suburban psychiatric emergency room.

This study evaluated the yield and utility of urine toxicological screening (UTS) in psychiatric emergency room patients. We also assessed the effect of race, gender, and cocaine and/or alcohol detection on clinical symptoms and diagnoses. The study's null hypothesis is that recent use of cocaine and/or alcohol in psychiatric emergency patients does not change rates of psychiatric symptoms at evaluation. We did a retrospective chart review of 325 consecutive psychiatric emergency room patients and of 112 consecutive patients with benzoylecgonine (BE; major metabolite of cocaine) on UTS. At ass

Mixed mania associated with cessation of breastfeeding.

BACKGROUND: This case chronicles the unique presentation of psychotic mixed mania in a female 5 months after parturition and 1 week following breastfeeding discontinuation, highlighting a rarely recognized mania risk factor that is temporally delayed from parturition: breastfeeding discontinuation. CASE PRESENTATION: A 25-year-old G1P1 female with a past psychiatric history of a depressive episode in adolescence presented to the Emergency Department with her 5-month-old daughter, fiancée, and family 1 week after breastfeeding cessation. She endorsed sleep-deprived energy enhancement, unfulfil

First presentation psychosis among the elderly in Singapore.

INTRODUCTION: In tandem with our ageing population, it is observed there is a growing trend of elderly patients presenting for the first time with psychotic symptoms. Clinical experience suggests differences in the phenomenology of late-onset psychosis in our Asian context compared to studies done in the West. This study aimed to analyse the characteristics and psychopathology of first presentation psychosis in our local elderly and to determine the treatment outcome over a 12-month period. METHODS: A total of 64 subjects with first presentation psychosis were consecutively recruited. Those w

Precipitation and determination of the onset and course of schizophrenia by substance abuse--a retrospective and prospective study of 232 population-based first illness episodes.

Onset and lifetime prevalence of substance abuse were assessed retrospectively using the IRAOS interview in a population-based, controlled sample of 232 first episodes of schizophrenia (ABC sample). Subjects with schizophrenia were twice as likely as controls to have a lifetime history of substance abuse at the age of first admission (alcohol abuse: 23.7 versus 12.3%; drug abuse: 14.2 versus 7.0%). 88% of the patients with drug abuse took cannabis. The sequence of substance abuse and schizophrenia was studied on the timing of abuse onset and illness onset, the latter as based on various defini

Complex PTSD symptoms predict positive symptoms of psychosis in the flow of daily life.

BACKGROUND: Post-traumatic stress disorder (PTSD) has been shown to predict psychotic symptomology. However, few studies have examined the relative contribution of PTSD compared to broader post-traumatic sequelae in maintaining psychosis. Complex PTSD (cPTSD), operationalized using ICD-11 criteria, includes core PTSD (intrusions, avoidance, hyperarousal) as well as additional "disturbances of self-organisation" (DSO; emotional dysregulation, interpersonal difficulties, negative self-concept) symptoms, more likely to be associated with complex trauma histories. It was hypothesized that DSOs wou

Diagnostic shift in first episode psychosis: Results from the 2-year follow-up of the "Parma Early Psychosis" program.

PURPOSE: Although the stability of current diagnostic criteria for people with First Episode Psychosis (FEP) is essential for treatment, it still remains poorly investigated. As its examination necessarily requires a prospective evaluation of diagnostic trajectories, the aims of the current longitudinal investigation were: (a) to assess diagnostic changes in an Italian FEP population treated within an "Early Intervention in Psychosis" service during a 2-year follow-up period, and (b) to identify potential sociodemographic and clinical moderators of diagnostic instability at entry. METHODS: Al

Exploring attachment, trauma, and cannabis use in psychotic disorders: a qualitative study of patient and family perspectives.

BACKGROUND: Psychotic disorders substantially impact individuals psychologically, occupationally, and socially. Although modifiable risk factors such as insecure attachment styles, history of psychological trauma, and substance use disorders (of which cannabis is the most frequent) are known to increase risk, it remains unclear how they may interact with each other to create pathways of vulnerability. More importantly, it is also unknown to what extent patients and their families recognize and address these risks to prevent illness, or support recovery. To address this gap, this study aimed to

A Comprehensive Analysis of Cerebellar Volumes in the 22q11.2 Deletion Syndrome.

BACKGROUND: The presence of a 22q11.2 microdeletion (22q11.2 deletion syndrome [22q11DS]) ranks among the greatest known genetic risk factors for the development of psychotic disorders. There is emerging evidence that the cerebellum is important in the pathophysiology of psychosis. However, there is currently limited information on cerebellar neuroanatomy in 22q11DS specifically. METHODS: High-resolution 3T magnetic resonance imaging was acquired in 79 individuals with 22q11DS and 70 typically developing control subjects (N = 149). Lobar and lobule-level cerebellar volumes were estimated usin

Brain metabolism in substance-induced psychosis and schizophrenia: a preliminary PET study.

INTRODUCTION: The relation between schizophrenia and cannabis abuse has been widely discussed from etiopathogenetic, psychopathological and neurometabolic points of view. Relatively little has been written about the differences between schizophrenia with co-occurrent cannabis abuse and substance-induced psychotic disorder (SIPD). Given these premises, our study aims to investigate the psychopathological and neurometabolic features of these clinical entities. METHODS: We enrolled patients experiencing an acute psychotic episode, affected either by schizophrenia with or without cannabis abuse (

Psychiatric adverse effects of pediatric corticosteroid use.

Corticosteroids, highly effective drugs for myriad disease states, have considerable neuropsychiatric adverse effects that can manifest in cognitive disorders, behavioral changes, and frank psychiatric disease. Recent reviews have summarized these effects in adults, but a comprehensive review on corticosteroid effects in children has not been published since 2005. Here, we systematically review articles published since then that, we find, naturally divide into 3 main areas: (1) chronic effects of acute prenatal and neonatal exposure associated with prematurity and congenital conditions; (2) im

Profiles of "manic" symptoms in bipolar I, bipolar II and major depressive disorders.

BACKGROUND: Classical authors such as Kraepelin, as well as the emerging literature during the past decade, indicate that manic-like signs and symptoms are present to a variable degree in all mood disorders. Current nosography does not differentiate between them and only the number of symptoms or severity is used for classification. This is particularly true for mania and hypomania. This paper will analyze the patterns of manic symptoms in bipolar I (BP-I), bipolar II (BP-II) and major depressive disorders (MDD), to test the hypothesis that mania and hypomania have different profiles, and asce

Relationships between attachment insecurity, beliefs about the self and others, paranoia, and social functioning across the psychosis continuum.

OBJECTIVES: Self-beliefs and other-regarding beliefs are related to attachment experiences and may contribute to paranoid beliefs and social functioning difficulties in psychosis. However, their relationships have not been examined jointly from an ecological perspective, while considering different degrees of psychosis risk. DESIGN: Relationships between avoidant and anxious attachment, self-beliefs and other-regarding beliefs, paranoia, and social functioning in daily-life were examined across the psychosis continuum of familial risk. METHODS: The sample comprised 29 patients with non-affec

Contributions of genetic liability and the COVID-19 pandemic to rising psychopathology among youth in the United States.

Adolescent mental health concerns surged during the COVID-19 pandemic, but whether the pandemic amplified pre-existing genetic vulnerabilities remains unclear. This study examined how shared genetic liability for eight psychiatric disorders interacted with adolescent mental health during the pandemic. Using genomic structural equation modeling, we identified four latent genetic factors: Compulsive, Psychotic, Neurodevelopmental, and Internalizing. We computed corresponding polygenic risk scores (PRS) in 10,374 participants from the Adolescent Brain Cognitive Development (ABCD) Study© across fi
