Gradual and safe technique for sinus floor elevation using trephines and osteotomes with stops: a cadaveric anatomic study.

OBJECTIVE: The aim of this study was to develop a new technique for maxillary sinus floor elevation through the crestal approach, using trephines and osteotomes with stops, and to assess the risk of sinus membrane lesion. STUDY DESIGN: The study was performed on 30 heads removed from fresh nonpreserved cadavers with subsinus bone height >or=5 mm. The anatomic specimens were sectioned axially on a plane passing 1 cm below the infraorbital foramen, to be able to see and film the sinus floor covered by the membrane. A total of 112 implants were placed using this technique (48 without grafting ma

A new technique for reconstruction of the atrophied narrow alveolar crest in the maxilla using morselized impacted bone allograft and later placement of dental implants.

BACKGROUND: In cases when the alveolar crest is too narrow to host an implant, lateral augmentation is required. The use of autogenous bone blocks harvested from the iliac crest is often demanded. One disadvantage is the associated patient morbidity. PURPOSE: The purpose of this study was to clinically and histologically evaluate the use of morselized impacted bone allograft, a novel technique for reconstruction of the narrow alveolar crest. MATERIALS AND METHODS: Two patients with completely edentulous maxillae and one partially edentulous, with a mean age of 77 years (range 76-79 years) we

The use of bovine porous bone mineral in combination with collagen membrane or autologous fibrinogen/fibronectin system for ridge preservation following tooth extraction.

Preservation of the alveolar ridge following tooth extraction is desirable since it facilitates placement of endosseous implants and may improve the adverse esthetics often associated with fixed partial dentures. The purpose of this study was to compare the clinical effectiveness of bovine porous bone mineral (BPBM) used as a graft material combined with either guided tissue regeneration (GTR) or with the autologous fibrinogen/fibronectin system (AFFS) in preserving alveolar ridges following tooth extraction. Twenty-six patients who required extraction of two or more anterior or bicuspid teeth

Guided bone regeneration using titanium mesh and xenogeneic graft alone: Histologic findings and 7-year follow-up.

BACKGROUND: Guided bone regeneration is a reliable technique for treating alveolar ridge deficiencies, especially in cases of severe maxillary atrophy. This case report describes the clinical management of a partially edentulous female patient presenting horizontal bone resorption in the anterior maxilla. METHODS: Horizontal ridge augmentation was performed using a titanium mesh in combination with a xenogeneic particulate bovine bone graft and a resorbable collagen membrane, without the use of autogenous bone. After 10 months of healing, the titanium mesh was removed, and a bone sample was c

Formation of the Philtral Column Using a Dermal Graft in Secondary Unilateral Cleft Lip.

INTRODUCTION: The philtral column has an important aesthetic significance and is especially important in patients with cleft lip. If the integrity of the philtrum cannot be preserved in patients of unilateral clefts, these patients will have profound abnormalities in their facial musculature. The purpose of this study was to analyze the effects of dermal grafting on the philtral column in patients with a unilateral alveolar cleft. METHODS: The authors retrospectively studied dermal grafts performed in Konkuk University Medical Center between January 2009 and January 2015. A total of 26 patien

Porous bovine bone mineral in healing of human extraction sockets. Part 1: histomorphometric evaluations at 9 months.

BACKGROUND: Extraction socket wound healing is characterized by resorption of the alveolar bone at the extraction site. This produces a decrease in ridge volume, deformations of ridge contours, and, thus, difficulties in delayed placement of root-form implants in an ideal position. Cancellous porous bovine bone mineral (PBBM) applied to fresh extraction sockets has recently been proposed to minimize the reduction in ridge volume. The aim of this study was to investigate the influence of PBBM grafted particles on the histopathologic pattern of the intrasocket regenerated bone and to evaluate hi

Tissue Engineering Therapy for Atrophic Maxilla Using Minimally Manipulated CD90 and CD105 Bone Marrow Stem Cells: A Case Report.

Bone deficiencies in the jaws of edentulous patients limit the possibility of successful placement and osseointegration of endosseous implants. Bone resorption is defined as a vertical and horizontal loss of quality and quantity in the residual bone ridges; lack of bone structure requires bone grafting procedures to enable implant placement and to improve the function, predictability, and longevity of implants. The objective of this case study was to demonstrate the feasibility of a novel cell therapy treatment for localized jaw bone defects with tissue repair cells composed of a mixture of bo

Socket seal surgery: Clinical uses in implant dentistry and guided bone regeneration procedures for single tooth replacement in the esthetic zone.

Restoring failing anterior teeth with a dental implant is considered a complex treatment even with thorough biologic knowledge of the situation. The goal is to produce a result in which the labial soft tissues and the papillae remain stable over time. Treatment of the fresh extraction socket in the alveolar ridge presents a challenge in everyday clinical practice. Regardless of the subsequent treatment, maintenance of the ridge contour will frequently facilitate all further therapeutic steps. Socket seal surgery and socket preservation in combination with immediate, early, or delayed implant p

Maxillary sinus augmentation using computer-aided design/computer-aided manufacturing (CAD/CAM) technology.

BACKGROUND: Maxillary sinus augmentation is a common method for increasing bone height for insertion of dental implants. In most cases, the graft is manually cut into a roughly appropriate shape by visual estimation during the operation; accordingly, the shape of the graft depends considerably on the experience of the surgeon. We have developed a computer-aided design/computer-aided manufacturing (CAD/CAM) technique to generate custom-made block grafts for sinus augmentation, and a customized cutting guide to precisely position the lateral wall and facilitate membrane elevation, using cone-bea

Simultaneous implant placement and vertical ridge augmentation with a titanium-reinforced membrane: a case report.

The present report demonstrates a clinical approach to achieve vertical ridge augmentation around endosseous implants. Two implants were placed, leaving the threads exposed, in the atrophic mandibular right posterior quadrant of a male patient. Both implants were covered with a titanium-reinforced expanded polytetrafluoroethylene (e-PTFE) membrane. Second-stage surgery was performed 12 months after implant placement. Upon membrane removal, growth of mineralized tissue was observed around both implants, covering areas previously not covered by bone. Implants were then progressively loaded and r

Implant-Guided Vertical Bone Augmentation Around Extra-Short Implants for the Management of Severe Bone Atrophy.

The purpose of this study is to describe the conservative treatment of severe vertical bone atrophy by combining the insertion of extra-short implants and implant-guided bone augmentation. For that, a low-speed drilling protocol was selected to facilitate the collection of bone particles and to maintain graft osteogenic properties. Extra-short implants were incompletely inserted because of the severe atrophy, and the denuded implant surface was covered by autologous bone particles held together by the adhesive properties of plasma rich in growth factors. The surgical site was then covered with
