Screening for childhood strabismus by primary care physicians.

OBJECTIVE: To review the clinical classification of strabismus, to describe the timing and method of strabismus screening examinations, and to discuss the principles of treatment. QUALITY OF EVIDENCE: Current literature (1983 to 1995) was searched via MEDLINE using the MeSH headings strabismus, ocular motility disorders, and amblyopia. Articles were selected based on their date of publication, clinical relevance, and availability. Preference was given to more recent articles, articles with large numbers of subjects, and well-designed cohort studies. Official recommendations from academic grou…

Development of pattern ERG and pattern VEP spatial resolution in kittens with unilateral esotropia.

PURPOSE: To follow the development of strabismic amblyopia longitudinally by comparing mean amplitudes and the visual spatial resolving ability of retinal and cortical pattern responses at various stages of postnatal development in unilateral iatrogenic convergent strabismic kittens. METHODS: Surgery to produce iatrogenic convergent strabismus was performed on 20 kittens at 3 weeks of age; three kittens were used for controls. The monocular transient pattern electroretinogram (PERG) and pattern visual evoked potential (PVEP) were recorded simultaneously on the 23 kittens throughout developmen…

Surgery for congenital esotropia.

The diagnosis of esotropia in an infant is not generally a difficult one for a skilled ophthalmologist to confirm. Uncovering all the subtle … single procedure and that additional surgery, both early and delayed, will be necessary. Similarly, amblyopia and accommodative esotropias will need to be treated in a significant segment of these patients. The stability of the final result depends upon the success of the treatment in all parameters. The results obtained are far from ideal. Some patients ac …

Modification of visual function by early visual experience.

Physiological experiments, involving recording from the visual cortex in young kittens and monkeys, have given new insight into human develo … hat eye in the cortex. These results can be correlated with the production of "stimulus deprivation amblyopia" in infants who have had one eye patched. Induction of a strabismus causes a loss of binocularity in the visual cortex, and in humans it leads to a loss of stereoscopic vision and binocular fusion. Exposing kittens to lines of one orientation modifi …

[Clinical research progress in infantile nystagmus syndrome].

Infantile nystagmus syndrome (INS) is an ocular motor disorder that presents at birth or early infancy. It is clinically characterized by involuntary and conjugated oscillation of the eyes, which often causes several complications such as amblyopia, lateral view, strabismus and torticollis. The etiology of INS is not fully understood, and this disease cannot be cured completely. This paper reviews the progress of research on the concept, etiology and pathogenesis, clinical manifestations, common examination methods, diagnosis and treatment of INS.

[Optic nerve atrophy--the main cause of visual impairment in children and young adults].

Blindness and visual disability are a very significant problem all over the world. Inflammation, metabolic disorders, tumours, hereditary optic neuropathies may all lead to visual impairment. The main cause of visual impairment and disability in children and young adults is optic nerve atrophy. THE AIM of the paper is the presentation of clinical features and treatment of nerve atrophy in children. Patients with optic nerve disorders should receive effective refraction and amblyopia treatment.

Cataract extraction and intraocular lens implantation in an infant with a monocular congenital cataract.

A cataract extraction and primary capsulotomy with intraocular lens (IOL) implantation was performed on a 17-day-old infant with a monocular mature congenital cataract. The IOL power, calculated at the time of surgery, was undercorrected to compensate for the growth of the eyeball. Two additional surgical procedures were required to resolve visual axis obstruction caused by lens epithelial cell proliferation. Follow-up was 18 months, during which amblyopia prevention therapy was instituted.

[The Teller Acuity Card Test: possibilities and limits of clinical use].

The Teller Acuity Card test was used to examine 49 normal children, 77 with strabismus, 9 with anisometropia and 19 with various organic ocular diseases. The vision of some of these children was also tested with the Landolt C and fixation preference tests. A comparison of the three tests showed that strabismic amblyopia is not reliably detected with the Teller Acuity Cards. On the other hand, this test appears to be a good one for detecting loss of acuity due to ocular diseases.

Accommodative esotropia: review of current practices and controversies.

The treatment of accommodative esotropia requires a thorough evaluation at first presentation to elucidate all factors involved. The degree of hypermetropia, character of the esodeviation (i.e., intermittent or constant), presence or absence of amblyopia, and the AC/A ratio are all important in the initial management of these patients. In addition, the binocular status can be of prognostic value. Close follow-up is needed to ensure maintenance of binocularity and appropriate early intervention to modify the course before bifixation is lost.

A new technique of congenital cataract surgery with primary posterior chamber intraocular lens implantation.

We present a series of nine cases involving primary intraocular lens implantation in congenital cataracts. Because of the problems encountered with the technique used in the earlier cases, we developed a new surgical technique that may be described as primary epilenticular posterior chamber lens implantation followed by pars plana endocapsular lensectomy. This new technique ensures a clear pupillary area, minimizes postoperative complications, and ultimately prevents amblyopia, resulting in useful vision.

Anterior polar congenital cataract and corneal astigmatism.

Seven eyes of six patients with anterior polar congenital cataract were examined. All patients presented with corneal astigmatism greater than 1.50 diopters. The combination of these two anomalies has often been responsible for amblyopia and occasionally for squint. The association of corneal astigmatism with this type of congenital cataract is possibly due to a delayed separation of the lens from the surface ectoderm during fetal development and thus may be considered a new syndrome.

Sjögren-Larsson syndrome: case reports of two brothers.

Sjögren-Larsson syndrome (SLS) is an autosomal recessively inherited disorder characterized by the triad of congenital ichthyosis, spastic diplegia or tetraplegia and mental retardation. SLS is also often associated with a variety of other anomalies, including signs that are of interest to optometrists and ophthalmologists. Two case histories are presented. Both patients exhibited maculopathies and uncorrected astigmatism. The reduced visual acuities in one case appeared to be caused at least partly by astigmatism and the associated astigmatic amblyopia.

Visual acuity and the developing visual system.

The objective measurement of visual acuity in infants is reviewed. Recent techniques have shown that visual acuity levels as early as four to six months after birth. This and other evidence indicates that the most sensitive period in the development of the visual system may be during the first six months of life. With this as a rationale, it is suggested that eye care practitioners consider eye examinations and correction of refractive errors during infancy as a means of prevention of future strabismus and amblyopia.

Study on the relationship between scoliosis and vision problems: A narrative review.

Adolescent scoliosis is one of the most common surgical disorders of the pediatric spine. With timely detection and early treatment, most scoliotic children can avoid major and expensive surgery. Vision problems are also frequently found at an early age and can take a toll on individuals quality of life. However, scoliosis, a severe health hazard to adolescents, is often accompanied by vision problems clinically, including myopia, astigmatism, strabismus, amblyopia, horizontal paralysis, and blindness. And people with genetic defects have a higher probability of suffering both spinal problems …

Ophthalmological manifestations in 6 patients diagnosed with Williams-Beuren syndrome and literature review.

Medical history of 6 patients diagnosed with Williams-Beuren Syndrome (SWB) in pediatric age was revised. All the patients presented characteristic elf facies and cardiovascular abnormalities. All presented good visual acuity, except one case of unilateral amblyopia. The most frequent refractive error was hyperopia (n = 6; 100%) and astigmatism (n = 5; 83.3%). Ocular motility alterations were found in 2 patients (1 case of exophoria with hyperfunction of right inferior oblique and another of congenital endotropia with bilateral hyperfunction of inferior oblique). On the cognitive function, 66.…
