Prevention of Spinal Anesthesia-Induced Hypotension During Cesarean Delivery by 5-Hydroxytryptamine-3 Receptor Antagonists: A Systematic Review and Meta-analysis and Meta-regression.

BACKGROUND: Hypotension remains a frequent complication of spinal anesthesia, increasing the risk of nausea and vomiting, altered mental status, and aspiration. The aim of this systematic review and meta-analysis was to determine whether 5-hydroxytryptamine3 (5-HT3) receptor antagonists, administered before the initiation of spinal anesthesia, mitigate hypotension. METHODS: After a systematic literature search in various databases, randomized placebo-controlled double-blind trials studying the preventive effect of 5-HT3 receptor antagonists were included. A random-effects model was applied, r

Incidence and Risk Factors of "Postdural Puncture Headache" in Women Undergoing Cesarean Delivery under Spinal Anesthesia with 26G Quincke Spinal Needle, Experience of Medical College in Rural Settings in India 2019: A Prospective Cohort Study Design.

INTRODUCTION: Almost every cesarean delivery is done under spinal anesthesia because of ease of doing, rapid onset, avoids maternal and fetal risk of general anesthesia, promotes early recovery. Major complication especially in young women undergoing LSCS under spinal anesthesia is post dural puncture headache (PDPH) which is caused by cerebrospinal fluid leakage. There is wide variation in reported incidence of PDPH (0.3% to 40 %) after spinal anesthesia being affected by various procedure and non procedure related risk factors like age, gender, needle size and type, numbers of spinal attempt

Autoregressive With Exogenous Input (ARX) Decision Support for Blood Pressure Maintenance During Cesarean Delivery Under Spinal Anesthesia: A Prospective Pilot Study With Matched Nonconcurrent Controls.

BACKGROUND: Spinal anesthesia for cesarean delivery commonly causes maternal hypotension, which may compromise uteroplacental perfusion and maternal comfort. Guidelines recommend maintaining maternal blood pressure near baseline with prophylactic vasopressor strategies, yet titration remains reactive. We evaluated an autoregressive with exogenous input (ARX) decision-support algorithm that provides real-time forecasts of maternal mean arterial pressure (MAP) to support vasopressor management during cesarean delivery under spinal anesthesia. METHODS: In this single-center, open-label, prospect

S-ketamine plus dexmedetomidine vs. S-ketamine plus propofol for sedation-analgesia during positioning for spinal anesthesia in older adults undergoing lower extremity fracture surgery.

BACKGROUND: Positioning for neuraxial anesthesia in geriatric lower extremity fracture surgery is painful and can destabilize hemodynamics. Sedation-analgesia must balance effective analgesia with respiratory safety and physiologic stability. METHODS: This single-center retrospective cohort study included patients aged ≥65 years undergoing lower extremity fracture surgery under spinal anesthesia who received S-ketamine plus dexmedetomidine (Group A, n = 48) or S-ketamine plus propofol (Group B, n = 46). Primary outcomes included a numerical rating scale (NRS) positioning pain score of 0-10 an

A Randomized Controlled Trial Comparing Isobaric Versus Hypobaric Plus Isobaric Bupivacaine in Thoracic Segmental Spinal Anesthesia for the Reduction of Shoulder Pain During Laparoscopic Cholecystectomy.

Introduction Laparoscopic cholecystectomy (LC) is traditionally performed under general anesthesia (GA). However, thoracic segmental spinal anesthesia (TSSA), where low doses of local anesthetics (LA), often with adjuvants, are used at thoracic spinal levels, is also being explored by some researchers. Shoulder pain is a common issue during LC, adversely impacting the patient's perioperative experience. A combination of hypobaric and isobaric LA at the thoracic level has been described to mitigate this complication. The primary objective of the study was to compare the efficacy of a combinatio

Epidural administration of 2% Mepivacaine after spinal anesthesia does not prevent intraoperative nausea and vomiting during cesarean section: A prospective, double-blinded, randomized controlled trial.

BACKGROUND: Intraoperative nausea and vomiting (IONV) is a common symptom during cesarean section (CS) delivery causing significant discomfort to patients. Combined spinal and epidural anesthesia (CSEA) can provide both intraoperative anesthesia and postoperative analgesia. During CSEA, it is reasonable to administer local anesthetics to the epidural space before patient complaints to compensate for the diminished effect of spinal anesthesia. Therefore, we hypothesized that intraoperative epidural administration of 2% mepivacaine would reduce the incidence of IONV. METHODS: Patients who were

Ephedrine-Induced Increases in Blood Pressure and Heart Rate Due to Suspected Cardiac Sympathetic Denervation Supersensitivity in a Patient with Parkinson's Disease Under Spinal Anesthesia.

BACKGROUND Denervation supersensitivity to sympathomimetic drugs has been noted in patients with Parkinson's disease (PD) whose cardiac sympathetic nerves are denervated. This phenomenon is not as well recognized as other complications of PD patients, but anesthesiologists should be aware of it because sympathomimetic drugs can sometimes be dangerous to these patients. CASE REPORT A 60-year-old woman was scheduled for total hip joint replacement under combined spinal-epidural anesthesia and sedation. She had been diagnosed as PD (stage 4 on the Hoehn and Yahr scale) with a history of orthostat
