Diagnosis of intracranial aneurysms by computed tomography angiography using deep learning-based detection and segmentation.

BACKGROUND: Detecting and segmenting intracranial aneurysms (IAs) from angiographic images is a laborious task. OBJECTIVE: To evaluates a novel deep-learning algorithm, named vessel attention (VA)-Unet, for the efficient detection and segmentation of IAs. METHODS: This retrospective study was conducted using head CT angiography (CTA) examinations depicting IAs from two hospitals in China between 2010 and 2021. Training included cases with subarachnoid hemorrhage (SAH) and arterial stenosis, common accompanying vascular abnormalities. Testing was performed in cohorts with reference-standard d

Usefulness of Silent MRA for Evaluation of Aneurysm after Stent-Assisted Coil Embolization.

OBJECTIVE: To determine the usefulness of Silent MR angiography (MRA) for evaluating intracranial aneurysms treated with stent-assisted coil embolization. MATERIALS AND METHODS: Ninety-nine patients (101 aneurysms) treated with stent-assisted coil embolization (Neuroform atlas, 71 cases; Enterprise, 17; LVIS Jr, 9; and Solitaire AB, 4 cases) underwent time-of-flight (TOF) MRA and Silent MRA in the same session using a 3T MRI system within 24 hours of embolization. Two radiologists independently interpreted both MRA images retrospectively and rated the image quality using a 5-point Likert scal

Diploic arteriovenous fistulas with marked cortical venous reflux.

Diploic arteriovenous fistulas are rare arteriovenous shunts involving the skull, which often drain antegradely into the internal or external jugular veins. Diploic arteriovenous fistulas with marked cortical venous reflux are extremely rare. Here, we present the case of a patient with diploic arteriovenous fistulas with marked cortical venous reflux and a literature review. A 73-year-old woman presented with headache. Magnetic resonance angiography revealed abnormal signal intensity in the diploic layer of the left frontal bone. Digital subtraction angiography demonstrated a diploic arteriove

Spontaneous resolution of intraorbital arteriovenous fistulas.

Spontaneous intraorbital arteriovenous fistulas are extremely rare. A 50-year-old man presented with progressive right blurred vision for 1 year and right proptosis for 2 months. He had no trauma history or family history of vascular disorders. Orbital MRI showed increased vascularity in the right orbital apex and bilateral dilated superior ophthalmic veins. Digital subtraction angiography of the carotid circulation demonstrated several small fistulas between the right ophthalmic artery and the right superior and inferior ophthalmic vein. He did not receive any intervention treatment for the i

Delayed hemorrhage from a variant renal-origin inferior phrenic artery after liver trauma repair: A case report.

We report the case of a 19-year-old woman who experienced delayed hemorrhage 4 h after emergency liver repair for traumatic rupture. Digital subtraction angiography (DSA) confirmed that the source of bleeding was an anatomically variant right inferior phrenic artery (originating from the right renal artery), and the patient subsequently underwent successful hemostasis via transcatheter arterial embolization (TAE). This case reveals the masking effect of hypotension during compensated shock on vascular injuries and highlights the limitations of routine surgical exploration in identifying rare v

Acute Ischemic Stroke Due to a Mobile Thrombus Adherent to a Carotid Web.

A 48-year-old man presented with impaired consciousness and aphasia. Magnetic resonance imaging (MRI) of the head revealed acute cerebral infarction in the left temporal and parietal lobes. Carotid ultrasonography revealed a mobile structure at the origin of the left internal carotid artery. Cervical MRI using fat-saturation T1 imaging showed an isointense structure at the origin, with a high-signal structure extending toward the head. Digital subtraction angiography revealed contrast stagnation, suggesting the presence of a carotid web (CaW). The mobile structure was released, indicating thro

Bilateral superior cerebellar artery pure arterial malformation masquerading as aneurysms.

Pure artery malformation (PAM) is defined as a coil-like dilated and or overlapping appearance of arterial vessels in the absence of a venous component [1]. We report the case of a young adult who presented with a headache and was found to have a pure artery malformation of the bilateral superior cerebellar arteries (SCA), initially masquerading as a basilar tip aneurysm. Given the uncertainty in the diagnosis on noninvasive neuroimaging, a digital subtraction angiography (DSA) was pursued, confirming the diagnosis of PAM. The long-term significance of PAMs remains unknown, but diligent initia

Multimodal Imaging Reveals Labbé Vein Thrombosis Mimicking Subdural Hematoma: A Diagnostic Pitfall in Emergency Neuroimaging.

Labbé vein thrombosis is a rare cause of cerebral venous thrombosis that may mimic more common intracranial hemorrhagic pathologies, such as acute subdural hematoma or cortical hemorrhage, on initial CT. This case involves a 20-year-old male who presented with an acute right temporal headache and a hyperdense subcortical lesion on CT. The thrombus measured approximately 65 HU. MRI revealed a T1 hyperintense thrombus in the vein of Labbé, and digital subtraction angiography confirmed venous occlusion. The patient was treated with anticoagulation and recovered without neurological deficit. This

A Rare Case of Compression Neuritis due to Intraorbital Arteriovenous Fistula (IOAVF) Mimicking Retrobulbar Optic Neuritis.

Intraorbital arteriovenous fistulas (IOAVFs) are rare vascular abnormalities. We describe a case of an IOAVF featuring a direct shunt between the accessory meningeal artery and the superior ophthalmic artery. A 55-year-old woman presented with a 1-month history of visual impairment in her right eye, and magnetic resonance imaging (MRI) revealed optic neuritis-like findings. Steroid pulse therapy temporarily resolved visual impairment. However, 1 month later, she experienced decreased visual acuity, ocular conjunctival hyperemia, edema, and a pulsatile murmur. Contrast-enhanced MRI and digital

[A case report of tracheobronchial Dieulafoy's disease].

Tracheobronchial Dieulafoy's disease (TBDD) is a rare bronchial artery vascular malformation, characterized clinically by sudden, recurrent, and life-threatening massive hemoptysis. This article reports the case of a 9-year-old female patient who presented with massive hemoptysis lasting two weeks. Following ineffective treatment at a local hospital, she was transferred to our institution. Bronchoscopy revealed a smooth, submucosal protrusion at the orifice of the right lower lobe basal segment. Digital subtraction angiography (DSA) revealed tortuous bronchial arteries near the lesion, indicat

An Intraosseous Pterygopalatine Fossa Dural Arteriovenous Fistula.

Intraosseous dural arteriovenous fistulas (DAVFs) are distinct in that the fistula is located within the bone rather than the dura through which the dural vessels pass. It has been stated that only fistulas within marrow should be considered as intraosseous DAVFs rather than DAVFs with traditional angioarchitecture that erode into bone or are located within a bony foramen. The ambiguity in the definition may have contributed to the oversight and scarcity of relevant cases reported in the literature. Three- or four-dimensional digital subtraction angiography is useful for determining the locati

Mild trauma causing cervical fracture through transverse foramen leading to a vertebral arteriovenous fistula.

Vertebral arteriovenous fistula (vAVF) is an uncommon vascular disease defined as abnormal connections between the vertebral artery or its branches extracranially with nearby venous structures. This case report outlines the case of a man in his late 70s presenting with C1-C3 fractures after a mild trauma falling down a small staircase. CT angiogram (CTA) gave suspicion of vertebral artery dissection and pseudoaneurysm; however, digital subtraction angiography revealed a fracture-induced vAVF successfully treated endovascularly with coils. In conclusion, cervical fractures involving the transve

A rare cause of ischemic stroke in childhood: spontaneous long segment intracranial dissection.

Craniocervical arterial dissection is an important cause of arterial ischemic stroke in children. Recognition of dissections is of particular importance both in determining the risk of recurrence and in bringing about different treatment alternatives. We report a 10-year-old girl who presented with acute ischemic stroke due to spontaneous long segment dissection involving the parasellar internal carotid artery up to the distal M1 portion of the middle cerebral artery. Three-dimensional digital subtraction angiography with flat panel detector revealed the presence of major vessels originating f

A case of pancreatic AV malformation in an elderly man.

A 60-year-old man presented with recurrent abdominal pain and weight loss for 6 months. Abdominal imaging showed a large vascular lesion in the head and neck of pancreas suggestive of arteriovenous malformation (AV malformation). Endoscopic ultrasound was done which showed features of AV malformation with no evidence of pancreatic malignancy. Surgery was planned for definitive treatment of malformation. Digital subtraction angiography with angioembolization was done prior to surgery to reduce vascularity of the lesion. He recovered after a pylorus preserving pancreaticoduodenectomy. Histopatho

Transvenous Embolization of the Basilar Plexus Before Endoscopic Transsphenoidal Transclival Surgery.

BACKGROUND: The endoscopic transclival approach is a method for reaching the anterior side of the brainstem. In making this approach, bleeding from the basilar plexus is often observed when cutting the dura overlying the clivus. Controlling such bleeding is essential. CASE DESCRIPTION: We performed a transvenous embolization of the basilar plexus before making the transclival approach. After embolization, bleeding from the basilar plexus during the operation decreased considerably. CONCLUSIONS: To our knowledge, this is the first report of transvenous embolization of the basilar plexus. If t

Periprocedural complications in endovascular stroke treatment.

Endovascular stroke treatment is a neurointerventional emergency where the main goal is the early recanalization of the occlusion within the critical time window, as safely as possible. Although the time window and rate of complications for endovascular stroke treatment differ with anterior and posterior circulation strokes, awareness of potential periprocedural complications is important, as they affect patient morbidity and mortality. Periprocedural complications are classified as haemorrhagic complications, procedure-/device-related, puncture site complications, and late-onset events includ

Aggressive primary olfactory neuroblastoma of the sphenoclival region: a case report and literature review.

Olfactory neuroblastoma (ONB) is an uncommon malignant tumor of neural crest origin, which commonly arises in the superior nasal cavity. Ectopic origin of an ONB is exceedingly rare. We describe a rare case of an ectopic sphenoclival ONB with extensive involvement of the central skull base and with development of systemic metastases. To our knowledge, ours is the first case that describes the imaging features of this rare entity on computed tomography (CT), magnetic resonance imaging, 18 F-fluorodeoxyglucose-positron-emission tomography/CT, and digital subtraction angiography. We also describe

Posttraumatic carotid-cavernous fistula associated with evolving contralateral internal carotid artery pseudoaneurysm.

Head trauma can result in vascular injuries at the base of skull. We present a unique case of carotid-cavernous fistula (CCF) on one side with evolving contralateral internal carotid artery (ICA) aneurysm resulting from a single trauma. There were different pathomechanisms involved in the development of these lesions. Both these lesions were successfully managed with endovascular coiling. The case cautions us about a potential threat of another delayed lesion forming on contralateral side once an ipsilateral posttraumatic vascular lesion has been treated. The case also highlights the importanc
