Progressive ankle valgus in childhood requiring surgical correction is usually because of paralytic disease or conditions that produce a short distal fibula such as multiple exostoses or both. Surface epiphysiodesis of the distal medial tibial physis was used to correct valgus deformity in ten ankles in seven patients. Ankle arthroscopy was performed in 127 patients. It was possible to evaluate 107 of these patients. Follow-up covered a mean period of 39 months (12 to 94 months). We studied 31 patients (17 females, 14 males; average age, 34) with more than 2 years of followup who had chronic anterolateral ankle pain following inversion injury. All had failed to respond to at least 2 months of conservative treatment and had negative stress radiographs to rule out instability. Between 1917 and 1984, we performed 217 arthrodesis of the ankle joint. Posttraumatic states were the cause of the osteoarthritis in 153 cases, whereas 118 of these cases were malleolar fractures. The accident X-rays were analyzed, and the fractures were classified in accordance with the classification of Weber (11). In brief: With the increasing interest in figure skating over the past 20 years has come an increase in the number of injuries. The foot, ankle, and lower leg are the most common injury sites; this paper focuses on boot-related injuries to the foot and ankle. Of particular importance is the fact that almost all overuse injuries related to this sport respond to nonoperative treatment, which consists primarily of altering the skating boot. Changes in the skater's training and/or technique may also be necessary.

A total of 64 patients with open fractures of the ankle were reviewed to assess the benefits of rigid primary internal fixation. Of these, 27 patients were treated by rigid primary internal fixation; the remainder were treated by a combination of internal splintage with K-wires and plaster casting. Patients who were treated by rigid primary fixation fared better in terms of joint mobility, maintenance of reduction and reduced sepsis rate. Although skiers' feet are immobilized in extremely rigid boots, ligamentous lesions of the ankle may occur, especially in world class slalom skiers. The lesion arises when a sudden external rotation of the ankle causes the talus to press against the fibula and thus open the distal tibiofibular articulation. In 1950, Wolin and his colleagues described nine patients who exhibited a "meniscoid" lesion of the ankle. In the 10 years 1972 through 1982, the senior author performed 21 operations on 15 runners with persistent foot and ankle pain. The operative procedures involved decompression of peripheral nerves in the foot and ankle, consisting of release of soft tissues in the tarsal tunnel and foot or removal of abnormal bony excrescences that were irritating these nerves. The goal of reduction in displaced ankle fractures is the restoration of normal function in the involved extremity. This can be accomplished only by restoring the normal anatomy of the involved joints. Closed reduction is not always successful or indicated if vascular compromise, interspersed soft tissue, or osseous fragments are present. Open reduction with internal fixation allows for direct visualization of the involved parts and an accurate and rigid form of fixation. A case report of a displaced ankle fracture treated with open reduction and internal A-O fixation is presented.
