Patients with obstructive sleep apnea syndrome (OSAS) present upper airway obstruction during sleep which can be documented by electromyography. The cause of weakness in oropharyngeal muscles is still unknown. Lesions of pons and medulla oblongata have to be expected. Brainstem auditory evoked potentials (BAEP) may indicate pathological changes in these regions. Several studies described normal BAEP in OSAS-patients. Moderate forms of OSAS as well as central sleep apnea syndromes were investigated, however. In our study 20 patients (17 men, 3 women, mean age 53.9 +/- 2.1 ys) with severe OSAS (…

Hypersomnia in obstructive sleep apnea syndrome (OSAS) is due to sleep fragmentation caused by arousals related to repeated episodes of apnea. Arousals may also be triggered by other non-apneic respiratory events (NAREs). The contribution of NARE to sleep disruption and nocturnal desaturation has been assessed by evaluating the polysomnograms of 58 consecutive OSAS patients. We studied 28 cases (25 men) presenting more than 5 NAREs/hour of sleep in addition to apneic events. Mean age was 48.8 +/- 14 years, mean body mass index was 38.1 +/- 10.2 kg/m2 and mean oxygen saturation at baseline, bef…

A toxic shock syndrome occurred after a femoral nail removal requiring revision surgery. After administration of suxamethonium (1 mg.kg-1), an apnoea prolonged over 45 minutes was observed. The trachea was extubated 105 minutes after suxamethonium administration. For the nail removal, two days before, the anaesthetic had been given by the same anaesthesiologist, with a similar protocol. Apnoea extended over 20 minutes. The day of the revision surgery, plasma cholinesterase activity was 410 UI.L-1 and reached 910 UI.L-1, 9 months later. Dibucaine number was 20 and fluorure number 17. The apnoea

Lesions of the brainstem have been associated with obstructive sleep apnea in previous reports. We now report a case in which retromastoid craniectomy with microvascular decompression of the medulla and ninth and tenth cranial nerves resulted in the complete resolution of severe obstructive sleep apnea. Possible mechanisms for this observation are discussed.

Important pathophysiological mechanisms in obstructive sleep apnea at night are increasing pleural pressure swings, hypoxia and hypercapnia, as well as central nervous arousals with consecutive fragmentation of regular sleep structure. They influence the cardiovascular system, at first only at night and at a later stage also during the day. This might result in cardiac structural changes: dilation and hypertrophy of the right ventricle, hypertrophy of the left ventricle, (especially of the muscular ventricular septum), dilation of right and left atrium. It is suggested that these cardiac struc…

Patients with sleep apnea syndrome (SAS) suffer considerable morbidity and increased mortality. The most common symptoms of SAS include excessive daytime sleepiness, nocturnal breath cessation, snoring and gasping sounds. We reviewed the characteristics of 20 Saudi patients with sleep apnea (15 males, five females) who were studied at our tertiary care center in 1992 and 1993. The ages ranged from 15 to 61 years. All were obese. Two each were acromegalic and hypothyroid. Hypertension was noted in seven (35%) patients and one had symptomatic bradycardia. Two (10%) patients presented with recurr…

A critical role of peripheral chemoreceptors in terminating apnea and in initiating normal breathing has been emphasized. Since these are the only organs which signal hypoxia, failure of its adequate development can contribute to the disease of respiratory failure in neonatal life. The failure may reside in any of the steps from initiation of O2 chemoreception involving respiratory and non-respiratory pigments, ion balance including H+ and Ca2+, neurotransmitter mechanisms and transduction.

Sleep apnea syndrome is a relatively common disorder characterized by periodic cessation of breathing during sleep because of upper airway obstruction (obstructive sleep apnea) or by reduction of ventilatory drive (central sleep apnea). We report the case of a middle-aged, obese man who underwent tracheostomy for the treatment of obstructive sleep apnea documented by polysomnography. Although tracheostomy resulted in marked clinical improvement, relief of upper airway obstruction unmasked severe central sleep apnea. We report the successful treatment of this patient's central sleep apnea syndr…

Obstructive sleep apnea syndrome (OSAS) is a subtle but severe sleep disorder of early childhood. It occurs in 1.6%-3.4% of children between 6 months and 6 years of age. OSAS is often difficult to detect, and may have long-term consequences, including failure to thrive, behavioral disturbances, developmental delay, and cor pulmonale. Pediatric nurses are pivotal in assessing the clinical symptoms of OSAS so that prompt evaluation and treatment can be initiated. Early symptoms of OSAS include awakening with a startle or gasp, restlessness, retractions, inspiratory stridor, mouth breathing, and …

The sleep apnoea syndrome (SAS) involves impaired regular breathing during sleep. It causes poor quality sleep and thus subsequent somnolence in daytime, it leads to periodic oxygen desaturation and has serious cardiovascular consequences. The authors describe methods used for the diagnosis and quantification of the intensity of SAS which they used (all-night extended polysomnography, ambulatory equipment MESAM 4 and afternoon polysomnography) and they list also the main therapeutic procedures used in SAS.

Obstructive sleep apnea syndrome (OSAS) is associated with severe cardiac arrhythmias and conduction abnormalities. Cor pulmonale and right-sided heart failure may ensue. Uvulopalatopharyngoplasty (UPPP) is one of several treatment modalities suggested for OSAS. Tracheotomy and CPAP treatment in adult OSAS patients and adenotonsillectomy in children with OSAS were shown to lead to improvement in some cardiac parameters. Cardiac function was prospectively evaluated in 19 OSAS patients before and after UPPP. No significant changes after surgery were noted on electrocardiographic studies. Improve…

In this study we investigated the relationship between the size of adenoids and/or tonsils and obstructive sleep apnea syndrome (OSAS) in 19 children aged 3 to 7 years. After clinical examination by an ENT specialist, electrocardiogram (ECG), vectorcardiogram (VCG) and echocardiogram (EC) were performed while the patients were awake, and static charge sensitive bed (SCSB) method recording (noninvasive monitoring of ballistocardiogram, respiration and body movements) while asleep. After adenoidectomy and/or tonsillectomy, the volume of the tonsils and the adenoids was determined. RVH was found …

Nine males with sleep apnea DOES syndrome and three males with sleep apnea DIMS syndrome were treated with prosthetic mandibular advancement (PMA). The method uses a prosthesis, which is designed to advance the mandible 3-5 mm to prevent upper airway occlusion during sleep. The apnea index in the obstructive-type apnea and the percentage of time spent in obstructive apnea decreased significantly with PMA. Although the apnea index showed merely a tendency to decrease in central apnea (p < 0.1), the percentage of time spent in central apnea decreased significantly with PMA. A marked improvement …

Sleep obstructive apnea syndrome (SOAS) is a common condition with a strong male predominance. Its incidence is more than 1 percent in the population as a whole. It exists in snorers. Both snoring and SOAS are linked to the presence of abnormalities (congenital or acquired) of the upper respiratory tract. The nocturnal cardiovascular consequences of SOAS are directly linked to apnea. Bradycardia occurs during apnea and tachycardia when ventilation restarts. Paroxysmal nocturnal hypertension is a constant feature. Even in individuals who are normotensive during the day, each restarting of venti…

This study describes a prediction method for central apnea episodes of newborns during sleep. The cross-correlation between heart rate and respiration obtained from newborns is calculated. Employing the cross-correlation, the displacement between heart rate and respiration before central sleep apnea episodes was much larger than the displacement in the stationary state. As the result, it is possible to predict central sleep apnea episodes several minutes before their occurrence.

Several types of neurologic diseases can contribute to disturbed respiration during sleep. Abnormal function of respiratory and upper airway muscles can occur with neuromuscular diseases and lead to upper airway obstruction and alveolar hypoventilation during sleep. Central nervous system disorders that cause dysarthria and dysphagia can also lead to obstructive and central sleep apnea, even in the absence of the usual anatomic deformities associated with OSA. Appropriate treatment requires an understanding of the effect of the underlying disease process on upper airway and respiratory functio…

Apnea in sleeping patients with syringobulbomyelia can be accounted for by damages in afferent and efferent impulsation from the respiratory centre due to cysts in medulla oblongata and spinal channel distension. Controlled lung ventilation has been applied to such patients in sleep for more than 8 years. In Arnold-Chiari syndrome apnea in sleep is associated with paroxysms of occlusive hydrocephalus. Periodic controlled lung ventilation during sleep is obligatory in these patients. Both nosological forms are characterized by lower sensitivity of the respiratory centre to hypoxia and hypercapn…
