Surgical approach for appendicitis in neutropenia: a case report and review of the literature.

BACKGROUND: Acute appendicitis is a common abdominal pathology, particularly in younger patients presenting with abdominal pain. The clinica … of monocyte increase, one of the first signs of bone marrow regeneration, and delayed laparoscopic appendectomy was performed. The perioperative and postoperative course was uneventful. CONCLUSION: We discuss the different treatment strategies in patients with neutropenia presenting with acute appendicitis (i.e., conservative management, delayed appendectomy …

[Appendiceal carcinoid tumors. Evaluation of long-term outcomes in a tertiary level].

BACKGROUND: Appendiceal tumors are found in about 1% of appendectomies performed and 0.5% of intestinal neoplasias. Appendiceal carcinoids are the predominant histology in this group and are usually casual after appendectomy for other reasons. The prognosis is excellent and survival is 95% at 5 years after surgery. METHODS: Retrospective analysis of all patients with appendiceal carcinoid surgery in our hospital for 20 years (1990-2010) and survival at 5 years. We also discuss the need for additional treatment and testing for follow-up visits. RESULTS: 42 patients underwent surgery for appen…

Enterobius vermicularis as a Mimic of Acute Appendicitis: A Case Report and Updated Systematic Review.

A 12-year-old premenarchal female patient presented with right iliac fossa pain, perianal pruritus, urticaria, angioedema, arthralgia, peripheral eosinophilia, and an initially undetectable C-reactive protein. Serial ultrasonography demonstrated increasing intra-abdominal free fluid. Diagnostic laparoscopy demonstrated appendiceal serosal injection, turbid pelvic fluid, and mesenteric lymphadenopathy, prompting appendectomy. Histopathology identified Enterobius vermicularis (EV) within the appendiceal lumen, with reactive lymphoid hyperplasia but no acute suppurative inflammation. Symptoms res…

Burkitt-type lymphoma incidentally found as the cause of acute appendicitis: a case report and review of literature.

BACKGROUND: Appendectomy remains one of the most common emergency operations. Recent research supports the treatment of uncomplicated appendicitis with antibiotics alone. While nonoperative management of appendicitis may be safe in some patients, it may result in missed neoplasms. We present a case of acute appendicitis where the final pathology resulted in a diagnosis of a Burkitt-type lymphoma. CASE PRESENTATION: An 18-year-old male presented to the emergency department with 24 h of right lower quadrant pain with associated urinary retention, anorexia, and malaise. Past medical history was …

Appendiceal abscess revisited.

Appendiceal abscess developed in 2% of 2,621 patients with acute appendicitis seen between 1962 and 1976. While representing a commendable decline in frequency from earlier studies, in view of the demonstrated prolonged delay in seeking medical care, further decreases in incidence could be affected by increased patient education. Sixty-one of 68 patients underwent surgical drainage of the abscess, with a 28% complication rate. Interval appendectomy was performed in 42 cases, with a 19% complication rate. Two patients (3%) died. These rates do no differ appreciably from those reported during th…

Acute appendicitis in children is associated with an abundance of bacteria from the phylum Fusobacteria.

BACKGROUND: Although luminal obstruction has traditionally been viewed as the underlying cause of appendicitis, recent evidence has suggested that the disease may result directly from invasion by specific pathogens, e.g. Fusobacterium nucleatum. The purpose of this study was to survey microbial communities within pediatric appendectomy specimens using a culture-independent approach. METHODS: We performed 16S ribosomal gene sequence analysis to profile the microbiota present within luminal fluid obtained from 22 pediatric appendectomy specimens. These included 10 simple appendicitis cases, 5 p…

Treatment options of inflammatory appendiceal masses in adults.

At present, the treatment of choice for uncomplicated acute appendicitis in adults continues to be surgical. The inflammation in acute appendicitis may sometimes be enclosed by the patient's own defense mechanisms, by the formation of an inflammatory phlegmon or a circumscribed abscess. The management of these patients is controversial. Immediate appendectomy may be technically demanding. The exploration often ends up in an ileocecal resection or a right-sided hemicolectomy. Recently, the conditions for conservative management of these patients have changed due to the development of computed t…

It is a diagnosis by exclusio … ably a result of differences in the geographical distribution of these diseases. Recently, interval appendectomy has been suggested as an important cause of granulomatous appendicitis. Case report--mechanical bowel obstruction with appendicitis without strangulation and leukocytosis.

Intestinal obstruction is a common surgical emergency caused by varied conditions. Acute appendicitis is considered as one of the unusual cause of intestinal obstruction especially functional and very few cases of mechanical obstruction. Here we report a Case of 62-years-old male who presented here with a clinical picture of small bowel obstruction. On laparotomy, there was a dilated gut with long appendix having inflamed tip buried into adjoining mesentery and then appendectomy was done and obstruction was relieved. Also, bowel was found to be viable hence simple appendectomy was found to be …

A Retrospective Analysis of Conservative Management Versus Early Surgical Intervention in Appendicular Lump.

Introduction Acute appendicitis happens to be increasingly common in school-going children and early part of adult life, peak incidence reaching in the teens and early twenties. Luminal obstruction of the vermiform appendix is thought to be essential for the development of appendicular ischemia, gangrene, and perforation. The treatment of choice in acute appendicitis is emergency appendectomy. Appendicitis particularly puts the surgeon in a dilemma whenever the patient presents late by around four to seven days. In case of delay in presentation, complications like appendicular lump formation o…

Appendical fistulae formation as a complication of primary Crohn's disease prior to surgical management: report of a case.

We report herein the unusual case of a 55-year-old man who presented with primary Crohn's disease of the appendix with fistulae. In this patient there was no history of previous appendectomy. Although fistula formation around the orifice of the appendix after appendectomy for primary Crohn's disease of the appendix has been observed in a number of long-term follow-up studies, preoperative fistula formation is extremely rare.

Appendiceal goblet cell adenocarcinoma with perineural invasion extending into the ileocecal lesion.

BACKGROUND: Appendiceal goblet cell adenocarcinoma (GCA) is a rare subtype of primary appendiceal adenocarcinoma with an incidence of 1-5 per 10,000,000 people per year. Appendiceal tumors are often diagnosed after appendectomy for acute appendicitis. Notably, however, there is currently no standard treatment strategy for GCA, including additional resection. We report a case of appendiceal GCA with perineural extension into the cecum, in which ileal resection was considered effective. CASE PRESENTATION: A 41-year-old man was diagnosed with acute appendicitis and underwent appendectomy. Histop…

Clinical relevance of endoscopic peri-appendiceal red patch in ulcerative colitis patients.

BACKGROUND: Increasing evidence is suggesting appendectomy as an alternative treatment for ulcerative colitis (UC), especially in case of histological appendiceal inflammation. Therefore, preoperative identification of appendiceal inflammation could be beneficial. This study aimed to assess the prevalence of peri-appendiceal red patch (PARP) on colonoscopy. In addition, prognostic relevance of PARP for disease course and its predictive value for histological appendiceal inflammation in patients undergoing appendectomy was assessed. METHODS: UC patients undergoing colonoscopy in 2014/2015 were…

Effectiveness of laparoscopic approach for acute appendicitis.

BACKGROUND: Whether a laparoscopic approach is more effective for acute appendicitis than standard open appendectomy remains controversial. METHODS: Clinical records of the 95 consecutive patients who underwent appendectomy for acute appendicitis between April 2002 and December 2005 were reviewed retrospectively. Laparoscopic appendectomy was performed for 62 patients (LA group), while 33 underwent open appendectomy (OA group). At our institution, LA is a standard operative procedure for acute appendicitis, and OA underwent because of social reasons. Characteristics, operative factors, and po…

Appendiceal duplication an unusual cause of abdominal pain: A case report.

Appendiceal duplication is exceedingly rare, with a prevalence of 0.004% to 0.009% in appendectomy specimens. Appendiceal duplications can occur alone or in conjunction with cecal duplication. The persistence of the temporary embryologic second cecal appendix is hypothesized to cause appendiceal duplications. We present a case of appendiceal duplication in a 26-year-old Ethiopian female patient who had been experiencing abdominal pain in the right lower quadrant for 1 week. She developed anorexia, a loss of appetite, and a low-grade fever as a result of this. She reported direct and rebound mi…

Single-center analysis of appendiceal neoplasms.

The purpose of the present study was to evaluate the recent trend of diagnosing appendiceal diseases through the analysis of appendectomy materials, stressing the importance of their pathological examination. A clinicopathological assessment of patients undergoing an appendectomy was conducted, based on the pathological examination of resected appendiceal lesions. Using a pathological database of surgical specimens from patients who underwent an appendectomy between March 2002 and September 2014, a retrospective, single-center analysis was performed. Among the 803 patients identified, 752 with…

Bilateral renal vein thrombosis as a complication of gangrenous appendicitis.

Bilateral renal vein thrombosis after an appendectomy has never been reported in the pediatric literature. We describe the case of a 10-year-old boy who developed this very unusual complication following appendectomy for gangrenous appendicitis with peritonitis. Color duplex Doppler is the most appropriate investigation to allow correct diagnosis and immediate medical treatment. Peritoneal dialysis is a simple and effective tool to prevent permanent damage to renal function.

Hepatic actinomycosis presenting as liver mass: case report and review of the literature.

Primary hepatic actinomycosis is an uncommon entity. We report the case of a young man with past history of an appendectomy who presented with fever, weight loss and a liver mass that was initially suspected to be neoplastic. Percutaneous liver biopsy revealed actinomyces-like organisms. The patient was treated with intravenous and oral penicillin with good therapeutic response. Review of the literature and correlation between hepatic actinomycosis and previous appendectomy are considered.
