BACKGROUND: Ventriculo-atrial (VA) conduction can have negative consequences for patients with implanted pacemakers and defibrillators. There is concern whether impaired VA conduction could recover during stressful situations. Although the influence of isoproterenol and atropine are well established, the effect of adrenaline has not been studied systematically. The objective of this study was to determine if adrenaline can facilitate recovery of VA conduction in patients implanted with pacemakers.
METHODS: A prospective study was conducted on 61 consecutive patients during a 4-month period (A

Seven out of 829 pediatric cardiac patients (0.84%) were found to have sinus node dysfunction (SND) over the past seven years. Of the seven patients, three had structurally normal hearts. One of these three patients had long QT syndrome. In four patients, structural heart disease was noted. In three of these four patients the sinus node dysfunction was attributed to cardiac surgery. The age of onset of SND ranged from four months to eight years. Presenting symptoms and signs included syncope, near-syncope, seizure and congestive heart failure. Two patients were asymptomatic. Five patients had

Since Sealy's pioneering surgical intervention for Wolff-Parkinson-White syndrome, surgical electrophysiologic interventions have been developed for all supraventricular arrhythmias. The surgical rationales are based on the site of origin of the arrhythmic mechanism and the associated pathology that characterizes the "arrhythmogenic substrate." The Wolff-Parkinson-White syndrome is characterized by an accessory atrioventricular (AV) connection distinct from the AV node-His bundle system. It is associated with AV reentrant tachycardia or atrial fibrillation, or both, with fast ventricular respo

Immediate and long-term effects of a transverse circular incision of the entire right atrium, including the atrial septum, on internodal conduction were studied in 7 dogs. Electrocardiograms were followed up to 35 weeks, and the atrial epicardial excitation sequence was evaluated in all 7 dogs. The findings were compared with data from 3 normal dogs and 3 other dogs which underwent total atrial transverse incisions. All dogs with total atrial separation developed complete atrioventricular (A-V) block immediately after the operation and died within 3 days. All dogs with right atrial interruptio

Morbidity and mortality of total cavopulmonary connection (modified Fontan procedure) may be decreased in many patients with single ventricle in whom the risk of surgery is high by performing the operations in a staged fashion. Each operative intervention, however, exposes the sinoatrial node region to risk of injury, and a multistaged approach may increase the risk of altered sinoatrial node function in these patients. The purpose of this study was to compare the prevalence of perioperative arrhythmias in patients undergoing either a primary or staged approach to the Fontan operation. Records

Congenital heart block (CHB) is a syndrome of uncertain pathogenesis leading to cardiac conduction disturbances in the foetus and newborns. It has been proposed that maternal antibodies transmit immunological injury in the developing foetal heart, thus causing irreversible damage of the atrioventricular node, leading to third-degree atrioventricular block. However, some genetic or environmental factors may also be involved. We have searched for genetic markers that play a role in immune response and that would be pathognomonic for the disease, either in mothers by regulating their immune respo…

The purpose of an electrophysiological study (EPS) is to determine the mechanism of arrhythmias and to decide the means of therapy for that arrhythmia. The electrode catheters are introduced percutaneously into the atria or ventricle to record the intracardiac electrogram or to stimulate those areas by electrical current. In bradycardia, atrioventricular block and sick sinus syndrome are the good indications of EPS. To determine the indication of pacemaker implantation, recording the block site of atrioventricular block is useful, and sinus node recovery time and sinoatrial conduction time in

An apparently healthy man of 26 years of age suddenly died. He was known to have had a small ventricular septal defect and complete right bundle branch block from early childhood. At post-mortem examination the small ventricular septal defect was found associated with an aneurysm of the membranous septum. Histological examination showed a normal atrioventricular node and bundle, adjacent to the aneurysm. There was fibrous interruption at the commencement of the right bundle branch, which was considered the basis of the bundle branch block. It was also considered that the right bundle branch bl…

Atrial fibrillation is the most common cardiac arrhythmia requiring treatment. Limitations of medical treatment have prompted development of nonpharmacologic therapies for this arrhythmia. These are aimed at ventricular rate control during atrial fibrillation, termination of the arrhythmia, and/or prevention of recurrences. Ventricular rate control can be achieved with transcatheter ablation or modification of the atrioventricular node. The MAZE operation is effective in preventing arrhythmia recurrence, but because it requires cardiac surgery, its appeal is limited. Development of the techniq…

Data are slowly accumulating regarding the possible benefits and appropriate uses of permanent cardiac pacing in a variety of pathophysiologic states or syndromes other than the broad categories and indications of sinus node dysfunction and atrioventricular block. The subjective and physiologic indications for pacing in these disorders are not symptomatic bradycardia or fulfillment of previously accepted criteria for antibradycardia pacing, and pacing is not the first or only therapy used. Therefore, individual patient evaluation, often including documentation of the response to temporary paci

The author used P-synchronized stimulation of the ventricles to prevent tachycardia paroxysms in patients with frequent drug-resistant paroxysms of supraventricular tachycardia (SVT). Electrophysiologic examinations revealed additional conductive pathways functioning during tachycardia in the retrograde direction in 12 patients whereas in 6 cases tachycardia was accounted for by the mechanism of the re-entry of the excitation wave inside the atrioventricular node. The detected mechanisms of SVT development made it possible to use a method of its prevention by ventricular electrostimulation in …

The clinical electrophysiologic findings in four patients with biopsy-proven polymyositis and bifascicular block are described. Atrioventricular (AV) block developed in two of these four patients, and intracardiac electrophysiologic studies documented the site of spontaneous block to be distal to the His bundle in one, whereas in the second marked His-ventricular prolongation was found, and distal block developed at low paced atrial rates. Sinus node and AV nodal function were normal in all four. These electrophysiologic data provide the first documentation of the site of AV block in patients

The tachycardia-bradycardia syndrome (paroxysmal atrial fibrillation, flutter, or tachycardia followed by sinoatrial block or sinus arrest resulting in Stokes-Adams attacks) is an important clinical entity that requires familiarity by the clinician. Pathologic studies and physiologic mechanisms as revealed in the electrocardiogram indicate multiple disturbances in the conduction system of the heart (sinus node, atria, and atrioventricular junctional tissues). The electrocardiogram establishes the diagnosis. Pacemaker implantation with supplementary drugs has provided a satisfactory means of th

A 23-year-old gentleman presented with a history of palpitations. The 12-lead electrocardiogram showed no manifest ventricular pre-excitation. Echocardiogram was within normal limits. A retrograde study showed concentric activation of the atrium with decremental conduction. Atrial pacing from right atrial free wall showed progressive pre-excitation. No anterograde nodal duality was documented.

BACKGROUND: High-density three-dimensional (3D) mapping in the Triangle of Koch during sinus rhythm (SR) has demonstrated low amplitude, late activating, multi-component signals suggestive of the AV nodal (AVN) right inferior extension (RIE). Ablation at the site of these signals has been successful for the treatment of typical AV nodal reentrant tachycardia (AVNRT). The leftward inferior extension (LIE) of the AVN has been proposed as a participatory limb in AVNRT and described in the inferoseptal region of the left atrium (LA). We present the use of Ripple mapping of the LA in sinus rhythm t

Neostigmine is the anticholinesterase drug most commonly used to reverse blockade or speed up recovery from neuromuscular blockade from nondepolarizing neuromuscular blocking drugs. Because of its cardiac muscarinic effects, prior or simultaneous administration of glycopyrrolate or atropine is usually recommended. There have been a few case reports of bradycardia, atrio-ventricular (AV) block, and cardiac arrest following neostigmine/glycopyrrolate administration to reverse neuromuscular block affecting several patients. In this report, we describe a case of 21-year-old with a history of seizu

Atrio-Hisian bypass tracts are considered to be rare electrophysiologic curiosities. The prevalence and functional significance of these tracts are unknown. We examined the incidence of atrio-Hisian and nodo-Hisian bypass tracts, their electrophysiologic manifestations, and their physiologic role in supraventricular tachycardia in 200 consecutive patients referred for evaluation of supraventricular tachycardia. In one patient it was demonstrated for the first time that a concealed (retrograde only) nodo-Hisian bypass tract functioned as the retrograde limb of orthodromic reciprocating tachycar

A 13-year-old girl presented with incessant ventricular tachycardia complicating acute Coxsackie B3 myocarditis. Electrophysiologic assessment revealed that the tachycardia could not be terminated, overdrive suppressed or accelerated by programmed electrical stimulation, but was transiently slowed by intravenous adenosine triphosphate and had marked spontaneous and sympathoautonomic-mediated fluctuation in the tachycardia cycle length. These features were atypical of reentry and triggered automaticity and suggested that abnormal automaticity was the likely tachycardia mechanism. Intravenous am
