Between Cradles and Compassion: A Conditional Obligation to Adopt Instead of Procreating.

Such prominent authors as Tina Rulli have argued in favour of pro tanto duties to adopt instead of procreating. This article interrogates-and then subsequently extends support for-such a view. Specifically, this paper develops and defends a conditional moral obligation to adopt rather than procreate for agents who intend to become parents by drawing on antinatalist philosophy. It argues that even non-antinatalists should recognize what I call "antinatalistic pressures": reasons arising from the harms and risks of existence that shift the justificatory burden onto would-be procreators. I formul

In Algorithms We Trust? Stakeholder Perspectives on Algorithmic and AI-Based Survival Calculators.

BACKGROUND: The integration of digital algorithms and artificial intelligence (AI) into healthcare has enabled personalized medicine, e.g., prognostic calculators that predict survival/mortality estimates and patient risk stratification. Alongside these advancements, understanding patients' trust in and perspectives on the use of these tools in their healthcare is essential. METHODS: Interviews were conducted with advanced heart failure patients (and caregivers) who received algorithmic personalized survival estimates (PSEs) in the context of education and decision making about left-ventricul

Patient Objectification in Psychiatry.

Psychiatry is characterized by several distinctive features: (1) it sometimes provides treatment against a patient's will or preferences; (2) it intervenes in patients' minds through physico-chemical means, such as psychotropic medication; (3) it treats patients' utterances not as reports of facts but as signs of illness; and (4) it may attribute patients' inappropriate words or actions to mental disorder, thereby exempting them from the responsibility ordinarily borne by a moral agent. This paper reexamines the ethical unease inherent in these psychiatric practices through the lens of patient

Unsung Healthcare Heroes for Children.

Schools play a vital role in communities by supporting the health, well-being, and development of children. This commentary explores the perspective of school nurses, who are frequently the sole medical providers responsible for the health of hundreds to thousands of children across multiple schools. They play a critical role in identifying medical and social issues, dealing with pushback when enforcing health protocols, and providing life-saving care under challenging conditions. Examples of their work include managing chronic conditions like asthma and diabetes, conducting contact tracing du

Structuring access and responsibility in transfusion refusal: a document analysis of relative non-transfusion policies in Japan.

Refusal of blood transfusion is often framed as a conflict between competent patients' refusal and clinicians' responsibility to prevent death or serious harm. In Japan, many hospitals have adopted relative non-transfusion policies, which seek to avoid transfusion where possible while reserving emergency transfusion when necessary. Existing debates have asked whether such policies are ethically or legally justified. This study examines how they structure access and responsibility as hospital policies. We conducted a document analysis of publicly available transfusion refusal policies from all

Lack of Rigour on COVID-19 Vaccines.

Several years ago Lipworth, et al. (2023) indicated that emerging evidence reveals a multitude of mistakes and a lack of scientific rigour on all things COVID-19, including issues around the vaccines. Examining some of the most recent evidence, I explain that Lipworth, et al. were correct in hypothesizing that numerous mistakes have been made regarding the vaccines' development, recommendation, media coverage, and mandating.

An ethical defense of safer supply.

Safer supply is a medical form of harm reduction in which a patient is prescribed a drug to which they are addicted. Despite ample high-quality evidence that it is a safe and effective last-line treatment for severe, treatment-refractory substance use disorders, safer supply is not widely used or advocated. This is likely due in part to ethical concerns, but I contend that safer supply is ethical by both general moral standards and the professional ethical standards of healthcare. Recent defenses of various harm reduction interventions argue that they are permissible regardless of the moral qu

The ethical nature of total care: a hermeneutic reconstruction of Cicely Saunders' perspective.

Care has become a key concept in the theoretical literature in nursing and various health professions. However, while there is generally an agreement on the purposes of care, the same cannot be said about the nature of care. Clarifying this point is fundamental to analyzing and understanding the conceptual and ethical presuppositions of medical science and healthcare professions and to determining what to focus on to improve the quality of care. In this sense, important help comes from the discipline of palliative care. For the first time in the debate about caring, Cicely Saunders introduced

Capturing Perceived Utility from Qualitative Studies of Genomic Sequencing: Translating a Conceptual Model into a Working Codebook.

INTRODUCTION: As genomic sequencing (GS) is integrated into clinical medicine, understanding how patients and families perceive its utility is critical. Conceptual models for perceived utility have been developed, however, direct application of such models to independently-collected qualitative data remains rare. To fill this gap, we translated a comprehensive model of perceived utility (Smith et al. 2022) into a codebook and revised it via application to a qualitative dataset capturing parent-reported utility of GS. METHODS: Using the domains and subdomains from Smith et al.'s conceptual mod

Using an Equivalent Protections Framework to Improve Oversight of International Human Subjects Research.

Current US military international research protocols involving human subjects are reviewed, approved, and overseen by both US and regulatory bodies. In some cases host nation research partners are disadvantaged due to slower processing times and redundant protocol reviews when competing for opportunities to conduct clinical trials. This article considers international legal and regulatory tensions between protecting host nations' patient-subjects and supporting host nation researchers' ability to conduct phase 2 and phase 3 clinical trials in support of important military and global health obj

The Roles of Military Health Diplomacy and Health Security When Conducting Global Health Engagements That Primarily Promote National Security.

Global health engagement (GHE) has emerged as a central instrument of foreign and defense policy across multiple nations, reflecting the growing convergence of health and national security. This manuscript examines the ethical and strategic roles of military health diplomacy and health security in GHE activities primarily oriented toward advancing national security objectives. It argues that military GHE is ethically justified only when it promotes health security alongside, rather than subordinate to, national security interests. Drawing on contemporary examples, including U.S. military medic

Organ Procurement Organization Perspectives on Normothermic Regional Perfusion Practices: A Qualitative Interview Study.

BACKGROUND: Normothermic regional perfusion (NRP) increases the availability of quality transplantable organs. However, there are unresolved ethical challenges and variability in approaches to NRP facilitation across U.S. organ procurement organizations (OPOs). We sought to assess the landscape of OPO member perceptions on facilitating NRP. METHODS: Semi-structured interviews with OPO executives assessed their experiences collaborating with hospital partners, communicating with donor families, and training hospital personnel to implement NRP. Qualitative themes were elicited through deductive

Bodily Reflections on Living Organ Donors Through the Lens of Confucian Body Philosophy.

The body of a living organ donor is not merely a medicalized object but also a vessel for nurturing familial ethical bonds and reconstructing the self. The Confucian perspective on the body offers a unique lens for interpreting the donor's physical and existential situation. Confucianism interweaves the act of bodily donation with the harmony and continuity of the moral individual within the family, the production of ethical relationships, and the formation of self-awareness, thereby providing a multidimensional interpretation of the donor's bodily experience. Through their bodily sacrifice, l

The Fiduciary Argument Against Conscientious Refusal in Healthcare.

The question of when, if ever, healthcare professionals with conscientious objections should be accommodated by permitting them to refuse to provide medical interventions to which they morally object is a heated issue that has received significant attention in the medical ethics literature. While many professional medical associations have long held that such objections may be accommodated in a broad range of cases, a vocal and growing minority has been fervently arguing that the medical profession should cease accommodating conscientious objections and should rarely, if ever, permit clinician

Exploring Research Participants' Perceptions and Attitudes Toward Research and Informed Consent in Myanmar.

Informed consent is fundamental to ethical research involving human participants; however, little is known about the public's understanding of research participants' rights in Myanmar. This multicenter cross-sectional study examined patients' perceptions of medical research, attitudes toward information disclosure during the informed consent process, and recognition of participants' rights using a structured, self-administered questionnaire completed by 284 adult patients from three hospitals. Most respondents viewed medical research positively but demonstrated varying levels of trust and unde

Likelihood ratios and the dependence of clinical evidence: should individual LRs be taken at face value?

Likelihood ratios (LRs) are often used to quantify the evidential weight of clinical findings. In diagnostic accuracy studies, however, LRs are typically reported for individual findings, not for conjunctions of findings. Since LRs for conjunctions of findings are often unavailable, influential Evidence-Based Diagnosis texts suggest that individual LRs may be multiplied to obtain joint LRs, but only under the assumption of approximate conditional independence. This assumption can be analytically convenient, but often implausible in clinical practice, where findings arise from shared pathophysi

Assisted Dying in Unjust Conditions.

The phenomenon of euthanasia and/or assisted suicide (EAS) in unjust conditions has received much attention in the popular press, along with a handful of responses in academic venues. These responses typically defend the permissibility of EAS in unjust conditions by appeal to anti-discrimination or harm reduction arguments, which frame the issue in terms of exclusion from current EAS law. We first briefly examine these arguments within such a framing, and then argue that it fails to account for the deep moral concern elicited by EAS in unjust conditions. To address this moral concern more full

Is Unconsciousness the Intention? Physicians' Perspectives on Palliative Sedation Outside Specialized Palliative Care: A Qualitative Study.

Background The principle of double effect is often invoked to interpret clinical intentions in palliative sedation (PS), considering that intended effect may differ from foreseen effect. Despite clarifications, intentions concerning patient consciousness remain ambiguous. Moreover, little is known about how non-palliative care specialists understand and apply these intentions. Methods This study aims to explore physicians' intentions regarding PS, with a particular focus on the relationship between their intentions and patients' consciousness. This qualitative study is based on twelve semi-str

"It's an Uphill Battle. We Have a Long Way to Go": Perspectives of Canadian Individuals Living with Genetic Conditions Detectable by Prenatal Cell-Free DNA Screening.

BACKGROUND: In Canada, prenatal cell-free DNA screening is mainly used as a second-tier screening test that targets specific aneuploidies, namely trisomy 21, 18 and 13. METHODS: Within the PEGASUS-2 study, we conducted in-depth interviews with 11 individuals living with a genetic condition currently or potentially detectable by this screening method, to explore their opinions regarding its potential implementation as a first-tier test, and the expansion of its scope. RESULTS: Qualitative analysis revealed that, although some individuals appreciated an early result with the test, they general
