The impact of functional architecture dynamics in bipolar disorder staging.

Bipolar disorder (BD) follows a neuro-progressive trajectory, yet current clinical staging models lack robust biological markers. Based on recent evidence, we hypothesized that BD stages are associated with alterations in the functional architecture of the Triple Network Model. We analyzed resting-state fMRI data from 123 individuals spanning five clinically defined BD stages. Using a hierarchical approach, we investigated how functional connectivity features characterize these stages, transitioning from coarse- to fine-grained analyses in both clinical classification (binary early-versus-late…

The recognition, diagnosis, and management of late-onset bipolar disorder.

INTRODUCTION: Late-onset bipolar disorder (LOBD) represents a clinically relevant but understudied subtype of bipolar. Compared with early-onset bipolar disorder, LOBD may present with distinct clinical features, increased somatic comorbidity, and potential associations with neurodegenerative and vascular processes. Specific diagnostic and therapeutic recommendations for LOBD are currently limited. AREAS COVERED: This narrative review examines the current evidence regarding epidemiology, pathophysiology, diagnostic workup, and management of LOBD. Literature was identified using PubMed to cond…

Inhibitory control in Bipolar Disorder disclosed by theta band modulation.

BACKGROUND: Cognitive inhibition is key to cognitive control in healthy and psychiatric conditions. Bipolar Disorder (BD) individuals display a range of inhibitory deficits and high levels of impulsivity across all stages of the disease, including euthymia. METHODS: We tested how the inhibition of heuristics in favor of analytical strategies influences the elaboration of sentences with logical quantifiers by means of a sentence-picture matching task in which the processing of quantified sentences containing the logical universal and particular quantifiers was required. Behavioral and brain os…

Immune, Developmental, and Synaptic Pathways Define Bipolar Disorder Clinical Heterogeneity.

IMPORTANCE: The clinical heterogeneity of bipolar disorder (BD) is a major obstacle to improving diagnosis, predicting patient outcomes, and developing personalized treatments. A genetic approach is needed to deconstruct the disorder and uncover its fundamental biology. Previous genetic studies focusing on broad diagnostic categories have been limited in their ability to parse this complexity. OBJECTIVE: To test the hypothesis that clinically distinct subphenotypes of BD are associated with different underlying common variant genetic architectures. DESIGN SETTING AND PARTICIPANTS: This multi…

Cost-utility analysis of adjunctive psychosocial therapies in bipolar disorder.

BACKGROUND: There are few economic evaluations of adjunctive psychosocial therapies for bipolar disorder. AIMS: Estimate the cost-utility of in-person psychosocial therapies for adults with bipolar disorder added to treatment as usual (TAU), from an Australian Government perspective. METHOD: We developed an economic model, estimating costs in 2021 Australian dollars (A$) and outcomes using quality-adjusted life-years (QALYs) gained and disability-adjusted life-years (DALYs) averted. The model compared psychoeducation, brief psychoeducation, carer psychoeducation, cognitive-behavioural therap…

Pathophysiologic Mechanisms of Cardiovascular Disease in Patients With Bipolar Disorder.

Bipolar disorder (BD) is a mental health condition characterized by periodic intense emotional states ranging between mania and depression. It has a strong association with comorbid cardiovascular disease (CVD), which contributes greatly to the mortality among this patient population. While a part of this association can be attributed to antipsychotic medication use, this systematic review highlights and offers a comprehensive analysis of other pathophysiological mechanisms that drive CVD in BD patients beyond pharmacologic and psychosocial factors. Through a systematic search of EMBASE, OVID,…

Retinal thickness: A window into cognitive impairment in bipolar disorder.

INTRODUCTION: Cognitive impairment (CI) in bipolar disorder (BD) significantly impacts overall functioning and quality of life. A better understanding of the neurobiological mechanisms associated with CI is needed. Studies on neurodegenerative diseases, such as Alzheimer's and Parkinson's diseases, have revealed promising findings related to retinal thickness alterations using optical coherence tomography (OCT). Similarly, retinal differences between healthy controls and individuals with BD or schizophrenia have been described. This study explores the utility of OCT in discerning retinal chang…

Potential causal association between the oral microbiome and bipolar disorder.

BACKGROUND: The oral microbiome can influence the growth, development, and regulation of the nervous system through various pathways; however, its relationship with bipolar disorder (BD) remains ambiguous. This study aims to investigate the causal relationship between the oral microbiome and BD through Mendelian randomization (MR) analysis. METHODS: Data regarding single nucleotide polymorphisms (SNPs) in GWAS summary statistics for oral microbiota and BD were obtained from two studies: one reported the association between the oral microbiome and the human genome, encompassing both the dorsum…

Psychoneural reduction revised: The case of suicidality in bipolar disorder.

This paper uses suicidality in bipolar disorder (BD) to illustrate that multidimensional frameworks account for complex phenomena with cognitive, psychological, socioenvironmental and physiological components better than their reductionist counterparts. We challenge level-based reductionist models that regard a particular dimension as fundamental. To comprehensively understand complex psychiatric phenomena, multidimensional models that acknowledge the heterogeneity of aetiological factors, rather than homogenising them under a single dimension, must be utilised. Multidimensional models allow a…

Genomics of Bipolar Disorder: What the Clinician Needs to Know.

Bipolar disorder (BD) affects approximately 2% of the global population, characterized by alternating episodes of mania or hypomania, and depression. It comprises two main types: bipolar I disorder, marked by severe manic episodes, and bipolar II disorder, defined by milder hypomanic episodes. Individuals often experience rapid cycling and significant comorbidities, leading to decreased productivity and increased mortality rates. Early diagnosis and intervention are crucial for better outcomes. Both genetic and environmental factors contribute to BD's etiology, with genetic research promising …

Can Sleep Parameters Predict Upcoming Mood Episodes in Bipolar Disorder?

BACKGROUND: Bipolar disorder (BD) is a recurrent disorder, characterised by episodes of (hypo)mania, depression and euthymia with variation in mood, cognition and sleep. Many patients identify changes in sleep before an episode; using daily sleep logs could help identify these changes. Such early warning signs can be a valuable tool for patients and clinicians alike in predicting and preparing for changes in mood. METHODS: In the BipoSense study, we followed patients with BD, who were in remission at the start of the study, daily for 1 year. Patients reported for each hour if they were awake …

Neurodevelopmental predictors of treatment response in schizophrenia and bipolar disorder.

BACKGROUND: Treatment resistance is a major challenge in psychiatric disorders. Early detection of potential future resistance would improve prognosis by reducing the delay to appropriate treatment adjustment and recovery. Here, we sought to determine whether neurodevelopmental markers can predict therapeutic response. METHODS: Healthy controls (N = 236), patients with schizophrenia (N = 280) or bipolar disorder (N = 78) with a known therapeutic outcome, were retrospectively included. Age, sex, education, early developmental abnormalities (obstetric complications, height, weight, and head cir…

Retinal Optical Coherence Tomography in Bipolar Disorder: A Scoping Review.

BACKGROUND: Bipolar disorder (BD) is a psychiatric condition with significant health implications due to its comorbidities, premature mortality, and functional impairments. Despite extensive research on treatment and rehabilitation, gaps remain in diagnosis and monitoring. Therefore, there is a need for biomarkers to identify individuals at risk for disease progression or exacerbation. Developmentally part of the central nervous system, the retina represents a possible marker for observing BD-related structural and functional alterations in the brain. SUMMARY: The retina's structure can be as…
