In utero treatment of severe fetal anemia resulting from fetomaternal red blood cell incompatibility: a comparison of simple transfusion and exchange transfusion.

OBJECTIVE: To compare in utero exchange transfusions (IUET) and in utero simple transfusions (IUST) for the treatment of fetal anemia resulting from red blood cell fetomaternal incompatibility. STUDY DESIGN: Retrospective comparative study from January 2006 through December 2011. The two techniques were compared for effectiveness, complications, and neonatal outcomes. RESULTS: 36 patients had 87 IUETs and 85 patients 241 IUSTs. Gestational age at the first transfusion was similar in both groups (IUET: 27±3.8 weeks; IUST: 27±4.7 weeks; NS) as was the initial fetal hemoglobin level (IUET: 6.4±

Mortality Risk Across Early RBC-Equivalent Transfusion Burden in the Whole Blood Era.

BACKGROUND: Concepts of massive transfusion and transfusion-associated futility were largely established during the component therapy era. Increasing whole blood use may alter the relationship between transfusion burden and mortality. We characterized the continuous association between RBC-equivalent transfusion burden and in-hospital mortality in a contemporary national trauma cohort. STUDY DESIGN: Adult trauma patients receiving ≥1 unit of packed red blood cells within 4 hours of admission were identified in American College of Surgeons TQIP from 2020 to 2023. RBC-equivalent transfusion bur

Allosensitization From Blood Transfusion in Pediatric Solid Organ Transplant Candidates: Impact of Irradiation.

BACKGROUND: Up to 30% of patients develop anti-HLA antibodies following a single transfusion with leukoreduced packed red blood cells (pRBCs). While some speculate that irradiation of pRBCs could reduce the risk of allosensitization, its effect in clinical practice remains unclear. METHODS: A single-center, retrospective study was conducted with 93 pediatric solid organ transplant (SOT) candidates from January 2013 to July 2022. All participants had a baseline calculated panel reactive antibody (cPRA) of 0% and underwent repeat cPRA testing at least 28 days after pRBC transfusion to evaluate

Hemodynamic effects of red blood cell transfusion in patients with hemato-oncologic diseases.

INTRODUCTION: The hemodynamic effects of red blood cell (RBC) transfusion in patients with hemato-oncologic diseases can be adverse. We measured acute hemodynamic changes with a novel non-invasive monitoring device. METHODS: Twenty-six patients (9 female, 17 male; median age 73) with hematological malignancies or solid tumors were included. Peripheral and central hemodynamics were assessed before and after RBC transfusion using the VascAssist2.0 device. RESULTS: Baseline hemoglobin was 7.8 g/dL (range 6.2-8.7). Post-transfusion, significant hemodynamic changes were observed: heart rate decre

Post-operative atrial fibrillation, intraoperative autologous blood salvage and transfusion in cardiac surgery.

BACKGROUND AND OBJECTIVES: Postoperative atrial fibrillation is the most common complication following cardiac surgical procedures. The objective of this study was to assess a possible association of postoperative atrial fibrillation, intraoperative autologous blood salvage, and blood transfusion. METHODS: A total of 357 adult patients undergoing cardiac surgery with the use of intraoperative autologous blood salvage were included. They were divided into two groups: one with 161 patients (45%) who only received their recovered autologous blood, and another with 196 patients (55%) who addition…

Calling for Diversity: Improving Transfusion Safety Through High-Throughput Blood Group Microarray Genotyping.

Blood transfusions, conducted between donors compatible in their red blood cell (RBC) antigens, play a life-saving role in transfusion medicine. Genetic differences at blood group loci between ethnicities result in diversity and altered frequency of RBC antigens that need to be considered in blood transfusions. Consequently, comprehensive and accurate blood group antigen typing is especially relevant for interethnic blood transfusions and for minorities underrepresented in the donor population. Blood group microarray genotyping is a cost-efficient and scalable method for comprehensive blood gr…

An Overview of Transfusion-Transmitted Infectious Agents among Blood Donors in Saudi Arabia.

BACKGROUND: The safety of donated blood during transfusion is important; however, blood donation can entail risks owing to contamination of the blood with transfusion-transmitted infections (TTIs). Ensuring the safety of blood and blood components is a major global goal to guarantee their availability to patients. Unsafe blood not only affects the patients but also extends the risk to their partners, contributing to the spread of TTIs and posing a burden on society. This review emphasizes the trends and prevalence of TTIs among blood donors in Saudi Arabia over the years. METHODS: A systemic

Tactical transfusion-Importance and logistics of integrating prehospital blood in tactical EMS operations.

BACKGROUND: Tactical Emergency Medical Services (TEMS) encompasses the skillset of prehospital care that is required to support all phases of operations performed by tactical law-enforcement personnel. High-risk operations can result in critical injuries to those involved. OBJECT OF THE REVIEW: Although TEMS clinicians have been taught to address critical hemorrhage, airway, and breathing issues, there is a benefit in incorporating the use of prehospital blood to reverse hemorrhagic shock. Given the unique environment in which TEMS clinicians operate, this does require forethought, planning,

Sex differences in outcomes following whole blood transfusion for patients with severe hemorrhage.

BACKGROUND: Whole blood (WB) transfusion is increasingly used for trauma patients with hemorrhagic shock; however, sex-based differences in utilization and outcomes remain incompletely understood. The purpose of this study was to examine sex-based differences in WB utilization and survival using a large national trauma dataset. METHODS: We performed a retrospective cohort study using the 2021 to 2022 American College of Surgeons Trauma Quality Improvement Program database. Trauma patients ( 16 y) with blunt or penetrating trauma, shock index >1, and receipt of 4 units of blood within 4 hours

Transfusion risk assessment for cesarean delivery: a preoperative tool to guide blood utilization.

BACKGROUND: Cesarean delivery is performed in approximately one-third of pregnancies in the United States and is associated with an increased risk of intrapartum and postpartum hemorrhage. Routine crossmatching of blood for all cesarean deliveries requires substantial resources and may not be necessary for every patient. We aimed to identify preoperative and intrapartum predictors of intraoperative blood transfusion and develop a practical risk stratification tool to support perioperative blood preparation. MATERIALS AND METHODS: We conducted a retrospective cohort study including 3,033 cesar…

Trace Elements in Different Blood Products Used in Neonatal Transfusion: Arsenic and Selenium.

Arsenic (As) is a toxic trace element with neurodevelopmental, carcinogenic, and other adverse effects. Meanwhile, selenium (Se) is an antioxidant trace element with essential physiological roles in humans. The preterm neonate is the most heavily transfused patient. The multiple blood transfusions could expose this vulnerable group to trace elements with variable effects. This study aimed to quantify As and Se in various blood products that were used in neonatal blood transfusions alongside an estimate of a dose per transfusion. In addition to exposure quantification, database mining and molec…

Reduction of Blood Transfusion in Iron Deficiency Anemia (ReBIDA): A Quality Improvement Initiative.

Background: Iron deficiency anemia (IDA) is the most common form of pediatric anemia, with first-line treatment focusing on iron repletion through oral and/or intravenous iron. The American Society of Hematology (ASH)/the American Society of Pediatric Hematology and Oncology (ASPHO) Choosing Wisely Campaign recommends against packed red blood cell (PRBC) transfusion for asymptomatic IDA. PRBCs are a finite resource and carry treatment associated risk compared to iron therapies. The use of oral and intravenous iron is an effective, tolerated therapy modality for IDA which can be overlooked base

Suction drainage increases the need for blood transfusion in bilateral total knee arthroplasty.

BACKGROUND: In simultaneous bilateral total knee arthroplasties (TKA), the blood transfusion rate could be higher, considering the cumulative blood loss from both knees. Due to its continuous suction mechanism, closed drainage can increase the amount of postoperative bleeding and drainage. In this study, we evaluated the effect of suction drain on postoperative blood transfusion in patients undergoing simultaneous bilateral TKA. METHODS: Consecutive 400 primary simultaneous bilateral TKAs were studied. In the 'drain group' of 100 consecutive patients (200 TKAs), a suction drain was used, and …
