BACKGROUND AND OBJECTIVE: Percutaneous screw fixation (PSF) technique is widely used in treating displaced intra-articular calcaneal fractures (DIACFs), the optimal screw configuration remains controversial. In this study, a finite element analysis (FEA) was conducted to compare the biomechanical properties of five different screw configurations in the fixation of Sanders type ⅡB calcaneal fractures, aiming to provide a reference for clinical strategy selection.
METHODS: In this study, models of five different screw configurations for the fixation of Sanders type IIB calcaneal fractures were

Femoral neck fractures (FNFs) affect the young adult population and are intimately related to high-energy trauma. Despite innovations in osteosynthesis materials, the rate of complications remains at 10%-59% in Pauwels type III (PIII) fractures. The authors thus propose a fixation model with a novel self-compression screw, comparing it to a sliding hip screw plate associated with a derotation screw in the fixation of a PIII fracture with posterior inferior comminution. The finite element method (FEM) was used in this comparison along with a virtual femur model with structural characteristics s

BACKGROUND: Intramedullary screw fixation of fifth metatarsal Jones fractures often produces satisfactory results, however, nonunion and refracture rates are not negligible. The low-profile "hook" plate is an alternative fixation method that has been promoted to offer improved rotational control at the fracture site, but this remains to be proven. The purpose of this study was to document biomechanical performance differences between this type of plate and a contemporary solid, dual-pitch intramedullary screw in a cadaveric Jones fracture model.
METHODS: Simulated Jones fractures were created

STUDY DESIGN: Occipital-cervical (OC) stabilization using occipital condyle fixation with a polyaxial screw-rod construct is described.
OBJECTIVES: To describe a novel technique and initial radiographic results for posterior OC fixation using the occipital condyles for cranial fixation.
SUMMARY OF BACKGROUND DATA: Stabilization of the OC junction remains a challenge. Owing to the regional anatomy and the poor occipital bone purchase, multiple attachment points to the occipital bone are required to increase construct rigidity. To address these issues, we propose a novel OC fixation technique

INTRODUCTION: Dynamization of proximal femoral nailing by removal of distal interlocking is one of the recommended treatment options for nonunions of femur fractures. However, in certain inter-/subtrochanteric fractures, gliding of the nail along the femoral shaft is blocked by lateral femoral cortical support of the lag screw. For these cases, Biber et al. proposed lateral cortical notching (LCN), in which the supporting lateral bone is removed. This study investigates the biomechanical effect of LCN on gliding of proximal femoral nailing and stress distribution at the bone/implant interface.

OBJECTIVES: Osteoporotic bone predisposes to hardware loosening in patients with odontoid fracture treated with anterior odontoid screw fixation. Cement augmentation is an established method to increase screw anchorage in such cases. The aim of this study was to determine the intra- and perioperative surgical complications of this technique and a comparison with a non-cement augmented group.
PATIENTS AND METHODS: During the period January 2012 to November 2017, 24 elderly patients with odontoid fractures and reduced bone mineral density were treated with cement-augmented anterior odontoid scr

BACKGROUND: The failure of the conventional nailing of both forearm bones or isolated fractures of radius and ulna pose a potential problem of nail migration and rotational instability, despite the best reduction. The purpose of this paper is to evaluate the results of screw elastic intramedullary nail for the treatment of adult diaphyseal fractures of both forearm bones, which effectively addresses the problems associated with the conventional nailing systems for the forearm fractures.
MATERIALS AND METHODS: Seventy-six adults with forearm fractures (radius and ulna or isolated fracture of t

The author presents a simplified technique for midline screw-plate fixation in fusion procedures after anterior cervical discectomy, in which the plate is introduced over the Caspar distractor pins. The Uniplate system used, with a single screw in each vertebral body, minimizes bone damage to the vertebral body as the screws can be fixed in the holes previously used for the Caspar distractor pins. This simplified version of the classical anterior cervical fusion technique saves surgical time, facilitates screw insertion, and obviates the need for manipulations to stabilize the plate before the

BACKGROUND: Implant-supported prostheses are widely used in posterior mandibular restorations, where biomechanical factors play a critical role in long-term performance. Abutment design may influence stress distribution within implant-supported systems, potentially affecting both prosthetic components and surrounding bone. This study aimed to evaluate stress distribution in three-unit implant-supported prostheses with different abutment designs using three-dimensional finite element analysis.
METHODS: A three-dimensional finite element model of the posterior mandible with two implants (4.1 mm

Scaphoid fracture is, in most cases, usually still treated conservatively. The disadvantages of long-term immobilization are stiffness of the wrist joint, loss of strength and higher costs. The osteosynthesis of the scaphoid fracture with the Herbert bone screw restores the exact form and length and the normal position of the scaphoid to the other carpal bones. It prevents non-unions, arthrosis and other late complications. The Freehand method is used as a percutaneous technique without utilization of a jig. The scaphoid fracture is treated with closed reduction and osteosynthesis is performed

BACKGROUND: Hybrid fixators with quite different joint design concepts have been widely to suppress adjacent segment degeneration problems. The kinematic and kinetic responses of the adjacent and transition segments and contact behaviors at the bone-screw interfaces served as the objective of this study.
METHODS: The moderately degenerated L4/L5 and mildly degenerative L3/L4 segments were respectively immobilized by a static fixator and further bridged by the rod-rod (Isobar) and screw-spacer (Dynesys) fixator. The joint stiffness and mobility of the rod-rod system and the cable pretension of

BACKGROUND: Anterior transarticular screw (ATS) fixation is a useful surgical option for atlantoaxial (AA) stabilization. This report presents a revised ATS method for AA fusion.
METHODS: A 79-year-old male presented with AA instability attributed both to an old odontoid fracture and severe degeneration of the lateral atlantoaxial joints (LAAJs). ATS fixation was performed through the conventional anterior cervical approach. The longest screw trajectories were planned preoperatively using multiplanar reconstruction computed tomography (CT) scans, with entry points of the screws situated at th

BACKGROUND: Conventional PMMA-augmented pedicle screws (CPPS) remain susceptible to "reverse windshield-wiper" failure in severe osteoporosis. This study aimed to elucidate this failure mechanism and develop a novel pedicle-PMMA-augmented screw (PPAS) with enhanced biomechanical stability under FE analysis.
METHODS: An L4 vertebral finite element (FE) model incorporating threaded hollow-core CPPS with anterior injection holes was established, followed by simulation of PMMA diffusion characteristics. Under 100 N craniocaudal cyclic loading, screw stress distribution and displacement were recor
