Infratentorial intratumoral hemorrhage.

Infratentorial intratumoral hemorrhage is infrequently reported. Owing to crowding of the vital structures in the posterior fossa, a high percentage of bleeding tumors in this region are fatal. Only six such cases of bleeding ependymomas in the posterior fossa have been documented; four of the patients died. We report a seventh case of intratumoral hemorrhage in a 4th-ventricle ependymoma. This patient survived. Reported cases of infratentorially bleeding tumors of various histologic types are reviewed. Their pathogenesis, clinical presentation, and management are discussed.

Mesenchymal Stem Cells Expressing CES1 and Soluble TRAIL Activate CPT-11 and Induce Apoptosis in Lung Cancer Brain Metastatic Lesions.

UNLABELLED: We aimed to develop a novel therapeutic strategy for lung cancer brain metastases by leveraging the tumor-tropic properties of genetically engineered Wharton's Jelly-derived mesenchymal stem cells (WJ-MSC) as vehicles for dual-agent gene therapy across the blood-brain barrier. WJ-MSCs were transiently engineered using lipid nanoparticle technology to coexpress soluble TRAIL (sTRAIL) and the prodrug-activating enzyme carboxylesterase 1 (CES1). In vitro analyses assessed transfection efficiency, therapeutic protein expression, apoptosis induction, and maintenance of stemness. Tumor-h

A Murine Model of Glioblastoma Initiating Cells and Human Brain Organoid Xenograft for Photodynamic Therapy Testing.

Glioblastoma (GB) is one of the most aggressive brain tumors, characterized by high infiltrative capacity that enables tumor cells to invade healthy brain tissue and evade complete surgical resection. This invasiveness contributes to resistance against conventional therapies and a high recurrence rate. Strategies capable of eliminating residual tumor cells are urgently needed. Photodynamic therapy (PDT) using 5-aminolevulinic acid (5-ALA), an FDA- and EMA-approved compound, induces selective accumulation of the photosensitizer protoporphyrin IX (PpIX) in metabolically active tumor cells, enabl

Anterior subtemporal approach for posterolateral brainstem cavernomas: report of ten cases.

BACKGROUND: The neuronavigation-assisted anterior subtemporal approach is proposed in this article as an alternative to surgery of posterolateral brainstem cavernomas. Brainstem cavernomas represent a neurosurgical challenge because of the high morbidity and mortality rate related to their surgical removal. Several nerve nuclei, ascending and descending fibers make this region at high risk of serious postoperative deficits. METHODS: Between 1998 and 2010, 24 patients underwent surgical removal of brainstem cavernomas in our institution. Ten of these patients presented a cavernous malformation

[Evaluation of the cochlear efferent system in patients with cerebello-pontine angle tumor].

The aim of the study was to estimate a cochlear efferent system in the group of patents with cerebello-pontine angle tumor (CPA). The size of an efferent effect was defined as the difference of the amplitudes in click evoked otoacoustic emissions (CEOAEs) during without contralateral acoustic stimulation (CAS) and with broad-bond noise (BBN) was estimated. The research was also conducted on the influence of tumor dimensions, age, sex, and the hearing level in tonal audiometry on the size of the efferent effect. The research was carried out on a group of 14 patients with cerebello-pontine angle

A study of safe entry zones via the floor of the fourth ventricle for brain-stem lesions. Report of three cases.

Direct surgery for intra-axial lesions of the brain stem is considered a hazardous procedure, and morbidity of varying degrees cannot be avoided even with partial removal or biopsy. The main causes of morbidity relate to direct damage during removal of the lesion, selection of an entry route into the brain stem, and the direction of brain stem retraction. The authors examined the possibility of making a medullary incision and retracting the brain stem, taking into account the symptomatology and surgical anatomy, and found two safe entry zones into the brain stem through a suboccipital approach

Brainstem auditory evoked potentials: intraoperative monitoring technique in surgery of posterior fossa tumors.

Brainstem auditory evoked potentials (BAEPs) were used to monitor eight neurosurgical procedures involving posterior fossa tumors to assist the neurosurgeon in preservation of hearing postoperatively. The technique included placement of recording electrodes over the Cz (vertex) and both earlobes. Stimulation was accomplished intraoperatively with a specifically designed intraauricular click stimulator that did not interfere with surgical access to the suboccipital region. Continuous BAEP recording was performed with particular attention to the sequence of preincision, opening of the dura, tumo

[Acute space-occupying processes in the posterior cranial fossa. Clinical aspects and computerized tomographic findings].

Ascending transtentorial herniation and compression of the brain stem are very severe sequelae of acute space-occupying lesions of the posterior fossa such as hematoma, infarction and tumours. The symptoms, course and outcome of these acute space - occupying lesions are discussed on the basis of 50 patients. For correct diagnosis of transtentorial herniation, CT studies are essential. The signs can be divided into early and alarming, and the time course into a biphasic and foudroyant form. Early detection of transtentorial herniation or impending incarceration is of crucial importance for succ

Nuclear magnetic resonance imaging (NMR), (MRI), of brain stem tumours.

The NMR scans of twenty-six patients who were referred with the clinically suspected or histologically verified diagnoses of brain stem tumour were reviewed. Twenty-one patients (eleven children and ten adults) had scans that were positive for tumour. Nine of these had histological confirmation. Eleven were irradiated without histology on the basis of the clinical diagnosis. The remaining patient was treated expectantly. The other five patients (two children and three adults) had CT and NMR scans that were negative for tumours. Clinical follow-up of these five cases for 9 to 29 months disclose

Recurrent intracranial hypertension and a midbrain glioma.

At the age of 36, this patient's clinical picture satisfied the criteria for the diagnosis of benign intracranial hypertension. After an essentially symptom-free interval, she developed new symptoms 17 years later that proved to be the result of a associated with glial-lined cavities in the midbrain and malignant glioma in the splenium. The possibility of a relationship between these disparate events is considered.

Intra-operatively obtained human tissue: protocols and techniques for the study of neural stem cells.

The discoveries of neural (NSCs) and brain tumor stem cells (BTSCs) in the adult human brain and in brain tumors, respectively, have led to a new era in neuroscience research. These cells represent novel approaches to studying normal phenomena such as memory and learning, as well as pathological conditions such as Parkinson's disease, stroke, and brain tumors. This new paradigm stresses the importance of understanding how these cells behave in vitro and in vivo. It also stresses the need to use human-derived tissue to study human disease because animal models may not necessarily accurately rep

Radiological complete response in diffuse intrinsic pontine glioma: a case report.

The prognosis of DIPG is still dismal. Despite transient clinical improvement after treatment, there is usually minimal or no radiologic regression and radiological complete response is very rare. Here, we report a child with DIPG who had radiological complete response and long-term survival treated with radiotherapy and adjuvant temozolomide. The initial MRI of a 9-year-old boy demonstrated large pontine mass causing expansion with the largest diameter of 35 mm, partially encasing the basilar artery and indenting the anterior wall of the 4th ventricle suggestive of DIPG. He received radiother

Reirradiation for diffuse intrinsic pontine glioma in an upper middle-income country: does full dose improve outcomes?

PURPOSE: Reirradiation (reRT) during diffuse intrinsic pontine glioma (DIPG) progression has emerged as a potential treatment strategy for prolonging survival. METHODS: This retrospective, single-center observational study was conducted on children diagnosed with DIPG between 2010 and 2025, and data from 58 were collected. Doses for reRT were as follows: 50-54 Gy (1.8-2.0 Gy daily/fractions) or 39 Gy (3 Gy daily/fractions). Overall survival (OS) was defined as the time from diagnosis to the date of death and reRT OS, reirradiation starting to death. The Kaplan-Meier method and log-rank test w

Characterisation of the expression of P2X7 receptor, cancer stem cell markers and immunological mediators in human high-grade gliomas.

INTRODUCTION: Glioblastoma is the most aggressive primary brain cancer. It is considered an 'immunologically cold' tumour where immune infiltrates are polarised to drive immunosuppression-and therefore tumour proliferation. An important driver of neuroinflammation in glioma is the purinergic P2X7 receptor (P2X7R). While much of the complex glioma microenvironment has been characterised, studies expounding the associations between various cytokines/chemokines, immune cell markers, P2X7R expression and glioma stemness are lacking. Here we aimed to characterise the mRNA expression profiles of var

Cortical thickness deviations as biomarker for subtyping and prognosis in pediatric brainstem tumors.

Brainstem tumors exert profound effects on cortical organization and functionality across the whole brain. However, the precise implications of changes in cortical thickness (CTh) for patient stratification and prognostic assessment remain unclear. Our study seeks to address these gaps and provide clearer insights into the distant impact of brainstem tumors. This study involved 124 pediatric patients with brainstem tumors and 849 healthy controls. Using CAT12 segmentation on 3D T1-weighted MRI scans and Gaussian process regression modeling, we established a normative CTh model from healthy dat
