OBJECTIVES: The objective of the present study was to assess the validity of ultrasound diagnosis of shoulder disorders in relation to examiner experience.
METHODS: A total of 239 patients referred to us for shoulder arthroscopy from October 2001 to June 2004 were prospectively studied by ultrasound. The following ultrasound diagnoses were evaluated: total and partial rotator cuff tears, calcific tendinitis, biceps tendon injuries and subacromial bursitis. Examiner A established the ultrasound diagnoses and examiner B performed the surgery during week A, whereas examiner B did the examination…

Accessing the subacromial space remains a fundamental yet challenging step in shoulder arthroscopy, particularly during rotator cuff repair. Limited space, bursitis, and adjacent tissue inflammation often complicate visualization and scope advancement. This technical note presents a reproducible and efficient 2-portal technique for subacromial access using a metal arthroscopic sheath, trocar, and rigid cannula. The procedure begins with creation of a standard posterior portal through which the sheath is advanced beneath the acromion using wrist-guided flexion-extension to identify anatomic lan…

BACKGROUND: Neuritis of the infrapatellar branch of the saphenous nerve can result from iatrogenic injury, entrapment, bursitis, or patellar dislocation. Currently, there is an unmet clinical need for treating refractory neuritis nonsurgically.
CASE PRESENTATION: Three patients presented with persistent anterior knee pain caused by neuritis of the infrapatellar branch of the saphenous nerve that had got excellent but only temporary relief from steroid and local anesthetic nerve block. The neuropathic pain diagnostic Douleur Neuropathique 4 questionnaire and painDETECT questionnaire confirmed …

Injections are valuable procedures for managing musculoskeletal conditions commonly encountered by family physicians. Corticosteroid injections into articular, periarticular, or soft tissue structures relieve pain, reduce inflammation, and improve mobility. Injections can provide diagnostic information and are commonly used for postoperative pain control. Local anesthetics may be injected with corticosteroids to provide additional, rapid pain relief. Steroid injection is the preferred and definitive treatment for de Quervain tenosynovitis and trochanteric bursitis. Steroid injections can also …

Adhesive capsulitis is a painful and disabling shoulder condition characterized by progressive synovial inflammation, capsular fibrosis, and global restriction of motion. Conventional treatment strategies, including physical therapy and landmark-guided intra-articular corticosteroid injection, often yield variable outcomes. Ultrasound-guided interventions have emerged as a precision-based therapeutic approach, enabling real-time visualization of pathological structures, accurate needle placement, and dynamic assessment of capsular mechanics. This narrative review summarizes current evidence on

BACKGROUND: The association between adhesive capsulitis and hypothyroidism is well-known. However, its relationship with hypertension and diabetes remains debated. While, rotator cuff (RC) tendinosis is frequently observed, to our knowledge, there is no study indicating its link to adhesive capsulitis. This study aimed to assess the prevalence RC tendinosis in adhesive capsulitis and examine associations between adhesive capsulitis and hypertension, diabetes, thyroid dysfunction, and inferior glenohumeral ligament (IGHL) thickness.
METHODS: This retrospective study included 503 patients with

OBJECTIVE: To evaluate the rabbit modle of frozen shoulder induced by persistent strain injuries and ice compression.
METHODS: Twelve clean, healthy male New Zealand rabbits with a mass of (2 500±500) g were selected and randomly divided into a blank group and a control group with 6 rabbits in each group. In the control group, the rabbits were modeled with persistent strain injuries and ice compression, the general conditions of the rabbits and the active and passive activities of the shoulder joint were observed and their body weights were recorded. MRI was performed on the affected shoulder

OBJECTIVES: The primary objective of this study is to evaluate, using ultrasound measurements, the correlation between coracohumeral ligament (CHL) thickness and restricted shoulder range of motion (ROM) in patients with adhesive capsulitis (AC). The secondary objective is to investigate the correlation between CHL thickness and disease duration.
DESIGN: Prospective cross-sectional survey.
SETTING: Clinical research of a tertiary care hospital.
METHODS: Overall, 65 patients with clinically diagnosed AC were enrolled. Ultrasound measurements of CHL thickness in the axial oblique plane were o

In this study data from 54 patients with persisting primary frozen shoulder were collected and evaluated retrospectively. All included patients underwent a specific kind of surgical treatment of the shoulder. Three different surgical techniques were compared to each other. A group of 21 patients received a combination of arthroscopic capsular release and subacromial decompression. 18 patients were treated by subacromial decompression combined with mobilization under anesthesia and 15 patients underwent selective arthroscopic capsular release. We evaluated glenohumeral range of motion in every

OBJECTIVES: To determine whether current care for common shoulder problems in Australian general practice is in keeping with rheumatologist expectations and the best available evidence.
METHODS: We performed a mailed survey of a random sample of 3500 Australian GPs and an online survey of all 270 rheumatologists in Australia in June 2009. Each survey included four vignettes (first presentation of shoulder pain due to rotator cuff tendinopathy, acute rotator cuff tear in a 45 year-old labourer and early and later presentation of adhesive capsulitis). For each vignette, GPs were asked to indica

BACKGROUND: Adhesive capsulitis is a common but poorly understood disorder of the shoulder. Various treatments have been developed to manage this condition, but the efficacy of these treatments is controversial. We developed an ultrasound-guided, minimally invasive interventional technique to manage adhesive capsulitis of the shoulder using a specially designed needle.
OBJECTIVE: To describe the methodological approach of our intervention.
STUDY DESIGN: Technical report.
SETTING: Pain management clinic.

The musculoskeletal system can be affected by diabetes in a number of ways. The shoulder is one of the frequently affected sites. One of the rheumatic conditions caused by diabetes is frozen shoulder (adhesive capsulitis), which is characterized by pain and severe limited active and passive range of motion of the glenohumeral joint, particularly external rotation. This disorder has a clinical diagnosis and the treatment is based on physiotherapy, non-steroidal anti-inflammatory drugs (NSAIDs), corticosteroid injections and, in refractory cases, surgical resolution. As with adhesive capsulitis,
