Liver Transplantation for Propionic Acidemia and Methylmalonic Acidemia: Perioperative Management and Clinical Outcomes.

Propionic acidemia (PA) and methylmalonic acidemia (MMA) comprise the most common organic acidemias and account for profound morbidity in affected individuals. Although liver transplantation (LT) has emerged as a bulk enzyme-replacement strategy to stabilize metabolically fragile patients, it is not a metabolic cure because patients remain at risk for disease-related complications. We retrospectively studied LT and/or liver-kidney transplant in 9 patients with PA or MMA with additional focus on the optimization of metabolic control and management in the perioperative period. Metabolic crises w

Cardiac peroxisome proliferator-activated receptor gamma is essential in protecting cardiomyocytes from oxidative damage.

OBJECTIVES: Peroxisome proliferator-activated receptors (PPAR) alpha and beta/delta are essential transcriptional regulators of fatty acid oxidation in the heart. However, little is known about the roles of PPARgamma in the heart. The present study is to investigate in vivo role(s) of PPARgamma in the heart. METHODS: A Cre-loxP mediated cardiomyocyte-restricted PPARgamma knockout line was investigated. In these mice, exon 1 and 2 of PPARgamma were targeted to eliminate PPARgamma from cardiomyocytes. RESULTS: PPARgamma null mice exhibited pathological changes around 3 months of age, featuring

Sudden cardiac death in familial hypertrophic cardiomyopathy: are "benign" mutations really benign?

Familial hypertrophic cardiomyopathy (FHC) is an autosomal dominant disorder characterised predominantly by left ventricular hypertrophy and sudden cardiac death. Mutations in the cardiac beta-myosin heavy chain gene have been identified in several families and designated as "benign" or "malignant". We describe a family (family L) with a "benign" mutation in which early sudden cardiac death has occurred. The family was studied by clinical, electrocardiographic and echocardiographic assessment. DNA analysis involved screening for the six most common cardiac beta-myosin heavy chain gene mutation

Ongoing enterovirus-induced myocarditis is associated with persistent heart muscle infection: quantitative analysis of virus replication, tissue damage, and inflammation.

Coxsackievirus B3-induced myocarditis in different immunocompetent mouse strains was used as a model to investigate interrelationships between virus replication and development of chronic enteroviral heart disease. Using in situ hybridization to detect enteroviral RNA, we show that heart muscle infection is not only detected in acute myocarditis but is also detected during the chronic phase of the disease. Coxsackievirus B3 could evade immunological surveillance in a host-dependent fashion, thus inducing a persistent infection of the myocardium in association with ongoing inflammation. Pattern

Single-base polymorphism of the ACE2 and TNF-alpha genes in patients with the cardiac and indeterminate forms of Chagas disease.

INTRODUCTION: Chagas Disease (CD) is caused by the protozoan Trypanosoma cruzi and is endemic to 21 Latin American countries. It is estimated that 6 to 7 million people in Latin America are infected. Clinical manifestations occur in two phases, acute and chronic. The chronic phase may present as indeterminate, cardiac, digestive, or mixed. Few studies have investigated why some infected individuals remain asymptomatic, while others develop more severe clinical forms of the disease. The present study aimed to evaluate the frequency of Single Nucleotide Polymorphisms (SNPs) in the ACE2 and TNF-a

Exercise Recommendations in Pediatric HCM: Variation and Influence of Provider Characteristics.

Pediatric Hypertrophic Cardiomyopathy (HCM) is associated with sudden cardiac death (SCD) that can be related to physical activity. Without pediatric specific guidelines, recommendations for activity restriction may be varied. Therefore, our aim is to determine the current practice and variability surrounding exercise clearance recommendations (ER) in pediatric HCM referral centers as well as provider and patient characteristics that influence them. We designed a survey that was distributed to the Pediatric Heart Transplant Study (PHTS) providers and members of the Pediatric and Adult Congenit

Increased collagen within the transverse tubules in human heart failure.

AIMS: In heart failure transverse-tubule (t-tubule) remodelling disrupts calcium release, and contraction. T-tubules in human failing hearts exhibit increased labelling by wheat germ agglutinin (WGA), a lectin that binds to the dystrophin-associated glycoprotein complex. We hypothesized changes in this complex may explain the increased WGA labelling and contribute to t-tubule remodelling in the failing human heart. In this study we sought to identify the molecules responsible for this increased WGA labelling. METHODS AND RESULTS: Confocal and super-resolution fluorescence microscopy and prote

Cellular regulation of iron assimilation.

Cells of plants, most microorganisms, and animals require well-defined amounts of iron for survival, replication, and differentiation. The metal is an important component of such processes as synthesis of DNA, RNA, and chlorophyll; electron transport; oxygen metabolism; and nitrogen fixation. Because of the insolubility of iron in aerobic environments at neutral and alkaline pH values, cells have had to devise specific strategies to assimilate the metal. These include (1) development of systems for reducing ferric ions to the more soluble ferrous ions at the cell surface, (2) employment of sma

The effect of a change in fluid intake advice on outcomes in chronic heart failure: a prespecified subanalysis of the FRESH-UP study.

BACKGROUND AND AIMS: The Fluid REStriction in Heart failure versus liberal fluid UPtake (FRESH-UP) study (NCT04551729) randomized patients with chronic heart failure (HF) to liberal fluid intake (LFI) vs fluid restriction (FR). Pre-trial fluid management may have impacted trial outcomes. METHODS: The open-label FRESH-UP study randomized 504 patients with chronic HF to an advice of LFI or FR of 1500 ml/day. Primary outcome was the Kansas City Cardiomyopathy Questionnaire Overall Summary Score (KCCQ-OSS) after 3 months adjusted for baseline. Key secondary outcome was thirst distress. Patient-re

Cardiac remodelling in a baboon model of intrauterine growth restriction mimics accelerated ageing.

KEY POINTS: Rodent models of intrauterine growth restriction (IUGR) successfully identify mechanisms that can lead to short-term and long-term detrimental cardiomyopathies but differences between rodent and human cardiac physiology and placental-fetal development indicate a need for models in precocial species for translation to human development. We developed a baboon model for IUGR studies using a moderate 30% global calorie restriction of pregnant mothers and used cardiac magnetic resonance imaging to evaluate offspring heart function in early adulthood. Impaired diastolic and systolic card

Cell-resolved testing of network-pharmacology candidates in dilated cardiomyopathy: PI3K-AKT-associated transcriptional differences are distributed across cardiac lineages and are not enriched above a genome-wide background.

Network pharmacology nominates targets but cannot show that they are altered in the relevant cell, distinguishable from a hypothesis-free analysis of the same data, or linked to disease. Taking the candidates nominated for Yangxin Decoction in dilated cardiomyopathy (DCM) and viral myocarditis, we asked how many survive independent functional-genomic testing. Few did. A direct-evidence intersection returned 61 genes, but across four bulk cardiac datasets the set was differentially expressed no more often than the transcriptome-wide background in any human cohort, and AKT1 was significant in no

Alcohol increases circulatory disease mortality in Russia: acute and chronic effects or misattribution of cause?

BACKGROUND: There is a consensus that the large fluctuations in mortality seen in Russia in the past two decades can be attributed to trends in alcohol consumption. However, the precise mechanisms linking alcohol to mortality from circulatory disease remain unclear. It has recently been argued that a substantial number of such deaths currently ascribed to cardiovascular disorders are misclassified cases of acute alcohol poisoning. METHODS: Analysis of routine mortality data and of a case-control study of mortality among working-age (25-54 years) men occurring in the Russian city of Izhevsk, w

Obesity-associated cardiac pathogenesis in broiler breeder hens: Development of metabolic cardiomyopathy.

Feed intake is typically restricted (R) in broiler hens to avoid obesity and improve egg production and livability. To determine whether improved heart health contributes to improved livability, fully adult 45-week-old R hens were allowed to consume feed to appetite (ad libitum; AL) up to 10 wk (70 d). Mortality, contractile functions, and morphology at 70 d, and measurements of cardiac hypertrophic remodeling at 7 d and 21 d were made and compared between R and AL hens. Outcomes for cardiac electrophysiology and mortality, reported separately, found increased mortality in AL hens in associati

Novel point mutations in the mitochondrial DNA detected in patients with dilated cardiomyopathy by screening the whole mitochondrial genome.

Dilated cardiomyopathy (DCM) is widely accepted as a pluricausal or multifactorial disease. Because of the linkage between energy metabolism in the mitochondria and cardiac muscle contraction, it is reasonable to assume that mitochondrial abnormalities may be responsible for some forms of DCM. We analysed the whole mitochondrial genome in a series of 45 patients with DCM for alterations and compared the findings with those of 62 control subjects. A total of 458 sequence changes could be identified. These sequence changes were distributed among the whole mitochondrial DNA (mtDNA). An increased

Nemaline myopathy and cardiomyopathy.

A case report is presented in which a 4-year-old male is diagnosed with hypertrophic cardiomyopathy, respiratory distress, muscle hypotonia, and psychomotor retardation. Electron microscopic study of skeletal muscle biopsy revealed pathologic changes typical of congenital nemaline myopathy, and biochemical analysis revealed a disorder of mitochondrial fatty acid oxidation. Therefore a previously undescribed combination of a structural and metabolic myopathy is reported.

25-Year Evolution of Apical Hypertrophic Cardiomyopathy in a Professional Athlete.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a leading cause of sudden cardiac death in young athletes. Risk stratification and disease evolution in athletes who continue competitive sport, particularly in apical phenotypes, remain incompletely defined. CASE SUMMARY: A 14-year-old academy football player was diagnosed with subclinical HCM during preparticipation screening and elected to continue competing despite contemporaneous guideline restrictions. Over 25 years of follow-up, he progressed to apical HCM with late gadolinium enhancement and mild ventricular ectopy but maintained excell

Indications, protocols, and interpretation of cardiovascular imaging for the evaluation and management of athletes. A Clinical Consensus Statement of the European Association of Preventive Cardiology (EAPC) and the European Association of Cardiovascular Imaging (EACVI) of the European Society of Cardiology (ESC). Part 2-Cardiovascular Magnetic Resonance, Cardiac CT and Nuclear Imaging.

The number of individuals engaging in sports continues to rise, and identifying those with cardiac substrates associated with increased risk of exercise-related adverse events is crucial. Athlete evaluation requires a refined diagnostic strategy to distinguish physiological cardiac remodelling from pathology. This joint European Association of Preventive Cardiology/European Association of Cardiovascular Imaging consensus provides a multimodality approach for advanced cardiovascular imaging in sports cardiology. Cardiovascular magnetic resonance, cardiac computed tomography, and nuclear imaging

Cardiopulmonary Exercise Testing Identifies Treatable ATTR Cardiac Amyloidosis: Beyond Natriuretic Peptides.

BACKGROUND: Transthyretin cardiac amyloidosis (ATTR-CM) is a progressive infiltrative cardiomyopathy where early disease-modifying therapy improves outcomes. However, treatment access may be restricted by biomarker thresholds not reflecting true functional impairment. CASE SUMMARY: A 73-year-old man with exertional dyspnea (NYHA functional class II) and confirmed wild-type ATTR-CM did not meet reimbursement criteria for tafamidis because N-terminal pro-B-type natriuretic peptide was below the required threshold (≥600 pg/mL), despite impaired functional capacity on cardiopulmonary exercise tes

Rbm24-mediated post-transcriptional regulation of skeletal and cardiac muscle development, function and regeneration.

RNA-binding proteins are critically involved in the post-transcriptional control of gene expression during embryonic development and in adult life, contributing to regulating cell differentiation and maintaining tissue homeostasis. Compared to the relatively well documented functions of transcription factors, the regulatory roles of RNA-binding proteins in muscle development and function remain largely elusive. However, deficiency of many RNA-binding proteins has been associated with muscular defects, neuromuscular disorders and heart diseases, such as myotonic dystrophy, amyotrophic lateral s
