Experience with the surgical treatment of 263 patients with acalculous cholecystitis is presented. Planned and postponed operations were fulfilled on 188 patients. 75 operations were performed for urgent indications. Gangrenous cholecystitis was found in 25 of the patients, phlegmonous cholecystitis--in 46 patients, catarrhal acalculous cholecystitis in 4 patients. Three patients died from severe concomitant diseases. The authors point to the considerable incidence of destructive forms of acute cholecystitis (especially in elderly and senile patients) which requires active surgical measures af…

The course of the disease analyzed in 104 patients has shown the correlation between severity of acute cholecystitis and the degree of diabetes mellitus, i. e. the "syndrome of mutual aggravation". The active- temporizing tactics is recommended. The data obtained show the timely operation with a radical removal of the inflammation focus in combination with an adequate correction of carbohydrate metabolism by high doses of insulin to be a successful method of treatment of acute cholecystitis in patients with diabetes mellitus.

A 53-year-old man who was a healthy, diet-controlled adult-onset diabetic underwent elective cholecystectomy eight weeks after an episode of acute cholecystitis. The gallbladder showed granulomatous inflammation and a single, mixed gallstone. Torulopsis glabrata was isolated from bile. This yeast of low pathogenicity is usually of clinical significance only in patients with serious associated illness. The patient remains well, and no reservoir for the organism has been identified in him. This is the first report of this organism causing cholecystitis confirmed by both culture and histologic ex…

A case of cholecystitis in an intrahepatic gallbladder with concurrent choledocholithiasis is reported. The patient initially presented with pyrexia of unknown origin and subsequently with suppurative cholangitis; the diagnosis was resolved pre-operatively using contemporary techniques of gallbladder delineation. Simple drainage of the gallbladder with choledocholithotomy proved effective.

It has been established that disorders in the blood circulation of the gallbladder play the dominating part in the development of posttraumatic cholecystitis. The disease is found more often to develop in elderly and senile people, mainly in men. The pathogenetically grounded treatment is considered to be the application of means improving hemodynamics and microcirculation in the gallbladder. The use of these means is indicated prophylactically in great traumas. Cholecystectomy is a necessary measure when the conservative treatment proves to be ineffective and when symptoms of generalization o…

Eighteen cases of gangrenous cholecystitis were reviewed to better define causes of morbidity and mortality. All patients in the series presented with signs and symptoms of acute cholecystitis. Cholecystectomy was done as an emergency procedure in 12 patients, while in six patients cholecystectomy followed a failure of nonoperative therapy. Gangrenous cholecystitis in this series was commonly associated with empyema and perforation of the gallbladder. Three of six patients having cholecystectomy after failure of nonoperative treatment died, while only one of 12 patients with immediate cholecys…
