Treatment of closed subtalar joint dislocation: A case report and literature review.

Subtalar dislocation is defined as a separation of the talocalcaneal and talonavicular articulations, commonly caused by high-energy mechanisms, which include falls from height, motor vehicle crashes, and twisting leg injuries. The dislocations are divided into medial, lateral, anterior, and posterior types on the basis of the direction in which the distal part of the foot has shifted in relation to the talus. The most common type is medial dislocation resulted from inversion injury. Subtalar dislocation may accompany with other fractures. Physical examination must be performed carefully to as

[Fracture of the calcaneus].

Although screw or plate fixation of the fractured calcaneus was first described in the 1920s, surgical treatment using open anatomical reduction and stable internal fixation only commenced at the start of the 1980s. This treatment was made possible by the introduction of new imaging methods such as CT which allowed better detection of the fracture pathology and provided the basis for new surgical strategies. New procedures, including modified surgical approaches related to vascular anatomy, arthroscopic control of the subtalar joint during open or closed reduction, early free flap coverage in

[Humeral fractures with paralysis of the radial nerve].

Forty fractures of the humerus associated with radial nerve palsy were reviewed after an average follow-up of 11 years. Early exploration of the radial nerve and internal fixation of the fracture was performed in 20 patients, while the other 20 patients were treated by closed reduction and immobilization in a plaster cast. In four patients of the latter group the radial nerve did not recover and a delayed exploration was performed. In each treatment group functional recovery of the radial nerve was observed in 16 patients (80%). On the basis of these results the authors suggest treating fractu

The impact of helicopter emergency medical services and craniocervical traction on the early reduction of cervical spine dislocation in a rural area of Japan.

BACKGROUND: Several studies have shown an association between achieving decompression of the spinal cord within a few hours and neurological recovery, even in patients with complete paralysis due to cervical spine dislocation. This study aimed to clarify the impact of helicopter emergency medical services (HEMSs) and craniocervical traction on the rapid reduction of lower cervical spine dislocation in rural Japan. METHODS: The success rate of and factors inhibiting closed reduction, the time from injury to reduction and the functional prognosis of lower cervical spine dislocations treated bet

Supracondylar humeral fractures in children: beware the medial spike.

Supracondylar humeral fractures are common in children, but there are no classification systems or radiological parameters that predict the likelihood of having to perform an open reduction. In a retrospective case-control study we evaluated the use of the medial spike angle and fracture tip-skin distance to predict the mode of reduction (closed or open) and the operating time in fractures with posterolateral displacement. A total of 21 patients (4.35%) with a small medial spike angle (< 45°) were identified from a total of 494 patients, and 42 patients with a medial spike angle of > 45° were

Conservative Treatment of Isolated Dorsal Dislocations of Fourth and Fifth Carpometacarpal Joints: A Report of a Rare Case.

Carpometacarpal (CMC) dislocations without associated fractures of the adjacent bones are extremely infrequent injuries. Dorsal or volar dislocations occur after high-energy injuries and may lead to early post-traumatic arthritis and carpal instability. The purpose of this study was to present a case of dorsal dislocation of both the fourth and fifth CMC joints that were treated with closed reduction and casting. A 31-year-old man developed severe acute pain, functional limitation, and deformity of the wrist after falling from a height. The clinical examination revealed intense localized tende

Information on infraorbital nerve damage from multitesting of sensory function.

Sensory disturbance following orbitozygomatic complex fractures was studied in 65 patients from 4 treatment groups which represented potentially varying degrees of sensory disturbance. The fracture-type-dependent treatments were: no surgical intervention (n = 20), closed reduction with or without wire fixation (n = 17), open reduction with miniplate fixation (n = 15) and/or reconstruction of the orbital floor (n = 13). In order to assess the sensory function of different classes of afferent fibres, several methods of sensory testing were applied. On average 6.3 months after treatment, the pati

Operative treatment for proximal phalangeal neck fractures of the finger in children.

Displacement and inappropriate treatment of a proximal phalangeal neck fracture may result in malunion of the fracture with consequent loss of motion and gross deformity, especially in children. We performed a retrospective study of twenty-four patients who had undergone operative treatment for a proximal phalangeal neck fracture, with a mean follow-up evaluation of 14 months (range: 12-30 months). We analyzed the types of fractures, their causes, operative treatments, complications, and functional outcomes. The age of the patients ranged from 2 to 14 years (average: 4.8 years). Twenty of the

[Antelunar dislocation of the wrist. Apropos of 3 cases].

Volar perilunar dislocation of the carpus is rare. We report three cases here. The patients were 18, 23 and 31 years of age. The etiology was a traffic accident in all cases and palmar hyperflexion of the wrist was noted in all three. Palmar dislocation was associated with a scaphoid fracture in two cases. In the third case, there was an associated fracture of the radial styloid and capitate. The patients were operated on after a delay of 48 h in two cases and four weeks in one. The dorsal approach was used in two cases and the palmar approach in the other. Reduction, fracture osteosynthesis a

Radial Nerve Palsy After Humeral Shaft Fractures: The Case for Early Exploration and a New Classification to Guide Treatment and Prognosis.

Radial nerve palsies are a common complication associated with humeral shaft fractures. The authors propose classifying these injuries into 4 types based on intraoperative findings: type 1 stretch/neuropraxia, type 2 incarcerated, type 3 partial transection, and type 4 complete transection. The initial management of radial nerve palsies associated with closed fractures of the humerus remains a controversial topic, with early exploration reserved for open fractures, fractures that cannot achieve an adequate closed reduction requiring fracture repair, fractures with associated vascular injuries,

Tibial-talar dislocation without fracture: treatment principles and outcome.

The incidence of tibial-talar dislocations without fracture is unknown and has been sparsely reported in the literature. The diagnosis of the injury is straightforward with the appropriate examination and roentgenograms. Good to excellent results can be achieved with open or closed dislocations treated by closed reduction and immobilization for 4-6 weeks. Although open dislocations require irrigation, debridement, and possibly delayed closure, controversy exists with regard to acute ligament repair. Because good to excellent results are possible without acute ligament repair, and delayed repai

Rupture of the posterior tibial tendon associated with closed ankle fracture.

Rupture of the posterior tibial tendon has rarely been associated with closed ankle fractures. All previous cases have been in association with pronation-external rotation type fractures in which the medial malleolus has been fractured. This case represents the first such report of a severed posterior tibial tendon in the absence of a medial malleolar fracture and reemphasizes the importance of critically evaluating intraoperative radiographs following the open reduction and internal fixation of closed ankle fractures to assess the possibility of soft tissue interposition. In the event of an a

Primary total talus arthroplasty for Hawkins type IV talar neck fracture dislocation.

A woman in her 40s was involved in a motor vehicle collision and sustained a closed Hawkins type IV talar neck fracture dislocation. The injury was treated with reduction, percutaneous pinning and spanning external fixation, followed by definitive treatment with total talus arthroplasty (TTA) 2 months following injury. This is a unique example of definitive management for a severe talar neck fracture dislocation with arthroplasty in the subacute setting. TTA is perhaps a primary option for these injuries at high risk for avascular necrosis, non-union, malunion and post-traumatic arthritis.

Association of Acute Trapeziometacarpal Joint Dislocation and Trapezium Fracture in a 14-Year-Old Boy: A Case Report.

Fracture-dislocation of the trapeziometacarpal (TM) joint in adolescent patients is a rare injury, with only 3 cases reported in literature to our knowledge. Its low incidence, together with the complexity of the anatomy and biomechanics of TM joint, may represent a challenge for surgeons in choosing the best treating option. Here, we report a case of a TM fracture dislocation in a 14-year-old boy treated with percutaneous Kirschner wire pinning. The results we obtained endorsed our choice in patients for whom closed reduction is achievable.

Post-traumatic nonunion of the clavicle in a 4-year-old boy and the importance of vitamin D level testing.

Clavicle fractures in children are common and usually go on to achieve solid union with closed treatment. A limited number of pediatric clavicle fracture nonunion cases have been reported in the literature, none of which were directly associated with hypovitaminosis D. We report the youngest case to our knowledge in a 4-year-old vitamin D-deficient male with a 6-month-old right midshaft clavicle fracture nonunion that was treated successfully with vitamin D supplementation, followed by open reduction and internal fixation with autologus iliac crest bone grafting.

Severely displaced scaphoid fracture treated by arthroscopic assisted reduction and osteosynthesis.

The severely displaced scaphoid fracture, when combined with a scapholunate dissociation, is a very rare condition that commonly is treated by open surgery. The authors present a case treated by arthroscopic-assisted closed reduction, using percutaneous pinning for the scapholunate dissociation and a Herbert/Whipple screw insertion for the scaphoid fracture. The patient was able to return to work four months after surgery. At twenty-seven months after surgery, both the patient's grip power and range of motion in the wrist were acceptable. The authors recommend this procedure as a minimally inv

Posterior dislocation of the radial head without fracture of the ulna. A case report.

Posterior radial head dislocation without fracture of the ulna in an adult is extremely rare. A case is presented in which immediate closed reduction, a short period of immobilization, and early active range of motion resulted in a very good outcome. Early detection by thorough physical examination and careful radiographic analysis is crucial for a good final result.
