Synergistic regulation of aquatic microbiome assembly and function by carbon resource availability and a bivalve bioengineer.

While resource subsidies and biotic manipulation are prevalent in managed ecosystems, their interactive effects on microbial community assembly and function remain elusive. Here, we investigated how external carbon (molasses) and a filter-feeding ecosystem engineer (Anodonta woodiana) interactively shape bacterial communities in aquaculture systems using a factorial mesocosm experiment (n = 3 replicate tanks per treatment, sampled at three time points). Integrating 16 S rRNA sequencing, community-level physiological profiling, and null model analyses, we found that combined treatment was assoc

Antarctic rocky outcrops as microbial diversity and biogeochemical hotspots.

Climate-driven deglaciation is rapidly exposing rocky outcrops in polar-alpine landscapes, creating novel habitats for organisms. Yet, the contribution of these new habitats to microbial diversity and biogeochemical processes remains largely overlooked. Here, we assessed how Antarctic rocky outcrops shape soil microbial diversity, organic matter (OM) formation, and nutrient cycling by comparing thin soils developed on rocky outcrops with soils from surrounding areas on Livingston Island, Antarctica. In these soils, we characterized bacterial and fungal communities, OM composition and extracell

"There's no real urgency when it comes to us": Critical discourse analysis of Black communities' lived experience with opioid overdose response in Indianapolis area.

This study aims to examine how Black communities construct their discourses about opioid overdose response from lived experiences in the Indianapolis area. We randomly selected and analyzed 20 out of 50 interviews as data saturation was reached. Our analysis was informed by critical discourse analysis (CDA) and intersectionality as an overarching framework for the interpretation process. Each interview lasted about 60 min and consisted of ten open-ended questions. Participants were Black residents over 18 years old, who lived in four zip code areas in Indianapolis that had the highest rates of

Voices from the Ground: Community Perspectives on Preventing Unintentional Child Injuries in Low-Income Settings.

Unintentional injuries significantly contribute to mortality and morbidity among children under five, with higher prevalence in low- and middle-income countries (LMICs). Deprived communities in these regions face increased injury risks, yet there is limited research on child safety tailored to their unique challenges. To address this gap, we conducted focus group discussions in rural Uganda, involving parents, village health workers, community leaders, teachers, and maids. The objective was to understand community perceptions around child safety and determine what culturally and age-appropriat

Communicating Effectively With People Experiencing Homelessness to Prevent Infectious Diseases.

BACKGROUND: People experiencing homelessness (PEH) are disproportionately affected by many infectious diseases, including coronavirus disease 2019 (COVID-19). However, communication efforts during public health emergencies like the COVID-19 pandemic often do not consider the unique needs of PEH. We examined how PEH seek and receive health information and how traditional health communication methods resonate with them. METHODS: We conducted in-person focus groups with PEH in 4 jurisdictions (Cincinnati, Ohio; Denver, Colorado; Sacramento, California; and the Bronx, New York) during July 2021.

Short-term outcomes of a randomized, mixed-methods trial to improve disclosure of hereditary cancer test results and follow-up communication with family members.

PURPOSE: This mixed-methods study evaluated an online, theory-guided family communication intervention (GeneSHARE) designed to increase the initial sharing of genetic test results and follow-up communication with family members among individuals with a germline pathogenic or likely pathogenic variant (GPV) in a hereditary cancer gene. METHODS: This randomized trial consisted of 528 participants with a GPV who self-reported having at least 1 untested, at-risk relative, of whom a subset of these participants was randomized to the GeneSHARE intervention and received resources designed to facilit

Australian pharmacist immunisers in vaccine conversations with patients: a qualitative study to explore communication support needs.

BACKGROUND: Since the COVID-19 pandemic, uptake of childhood and adolescent vaccines recommended under the National Immunisation Program (NIP) in Australia has declined, while adult NIP vaccine uptake remains stable but suboptimal. Over the past 10 years, pharmacists have taken on a larger role in NIP vaccination, yet their communication and information needs are not well understood. Supportive vaccine conversations are key to improving uptake, but can be difficult, especially with hesitant patients. This study explored Australian pharmacist immunisers' experiences and support needs around vac

Reducing Maternal Mortality by Strengthening Community Maternal Support Systems: Findings from a Qualitative Baseline Study in Northern Nigeria.

The "three delays model" illustrates how issues around obstetric emergency can lead to maternal deaths. This study applied in-depth interviews of key community gatekeepers in 16 rural communities across two states in northern Nigeria to evaluate the presence and functionality of key community maternal support systems for reducing maternal mortality. Findings show that only one out of the 16 communities had all the key support systems. Five rural communities reported that pregnant women have standing permission to visit health facilities during obstetric emergencies. A quarter of the communitie

A Phenomenological Study of Primary Healthcare Nurses' Experiences in Assessing and Managing Diabetic Foot Complications.

BACKGROUND: In South Africa, diabetic foot complications (DFC) often present for the first time at the primary healthcare (PHC) level, where nurses are central to diabetes management and prevention. Despite their key role, there are limited data on nurses' experiences in managing DFC, particularly in resource-constrained primary healthcare (PHC) settings. This study's objective was to explore and describe nurses' experiences in managing DFC at the PHC level, with the aim of informing future interventions and policy development. METHODS: A descriptive phenomenological study was conducted with

Review article: Impact of pandemics on rural emergency departments: A scoping review.

Pandemics can cause much distress to communities and present a major burden to the resources and functioning of hospitals. This scoping review aimed to identify, evaluate and summarise current literature regarding how pandemics impact rural EDs in terms of staff wellbeing, structure, function and resources. A systematic search of six databases using search terms including pandemic, ED and rural and remote was undertaken. Articles were included if they were peer-reviewed, written in English, original research, published between January 2010 and October 2021 and discussed the impact of pandemics

Research Priority Setting Is Not Research: A Call for Ethical Clarity in Australia.

In Australia, research priority setting (RPS) may be misclassified as research requiring ethics approval, despite its purpose as a collaborative, pre-research involvement activity. Treating RPS as research creates delays, diverts resources and undermines partnership principles central to inclusive, community-centred priority setting. Recent revisions to the National Statement on Ethical Conduct in Human Research clarify that pre-research consultation does not require ethics review, creating an opportunity for consistent national practice. We call for institutions to recognise RPS as involvemen

Housing for care, connection, and health equity.

Researchers and policymakers have used a four-pillar framework- condition, consistency, context, and cost-to describe the characteristics of housing that are important for health equity. We propose adding a fifth pillar: care and connection. Housing for care and connection refers to the housing design, institutional policies, and housing programs that strengthen social connections, caregiving relationships, access to resources, and a sense of self in community. Attending to these needs in housing is especially important for people who are in transition in and out of homelessness, living in pov

Top Ten Tips Palliative Care Clinicians Should Know About Navigating the Needs of Adults with Intellectual Disabilities.

As many people with intellectual disabilities (ID) live longer, the need for access to quality palliative care (PC) rises. People with ID realize significant barriers and inequities in accessing health care and PC. The need for integrated disability and PC services with extensive collaboration is great. The following tips are for PC clinicians caring for people with ID, their families, caregivers, and the community. While patient-centered care is difficult to distill into "tips," this article, written by an interdisciplinary team of PC and ID specialists, offers resources and references to imp

Economics of decriminalizing mental illness: when doing the right thing costs less.

The United States' criminal justice system has seen exponential growth in costs related to the incarceration of persons with mental illness. Jails, prisons, and state hospitals' resources are insufficient to adequately treat the sheer number of individuals cycling through their system. Reversing the cycle of criminalization of mental illness is a complicated process, but mental health diversion programs across the nation are uniquely positioned to do just that. Not only are these programs providing humane treatment to individuals within the community and breaking the cycle of recidivism, the p

Access to Affordable Housing Promotes Health and Well-Being and Reduces Hospital Visits.

Clinical interventions can only partially mitigate homelessness and housing insecurity, which are threats to health and well-being. Clinicians have several opportunities to address these problems: They can refer patients who are homeless or housing insecure to support services, advocate for their employer or care group to commit resources to end homelessness and housing insecurity, and/or work with government and private sector community organizations to address and eliminate these problems. Citing examples from around the US, we will illustrate how clinics, hospitals, health plans, and public

Drug-Resistant Tuberculosis: Challenges and Progress.

Antimicrobial resistance is a natural evolutionary process, which in the case of Mycobacterium tuberculosis is based on spontaneous chromosomal mutations, meaning that well-designed combination drug regimens provided under supervised therapy will prevent the emergence of drug-resistant strains. Unfortunately, limited resources, poverty, and neglect have led to the emergence of drug-resistant tuberculosis throughout the world. The international community has responded with financial and scientific support, leading to new rapid diagnostics, new drugs and regimens in advanced clinical development

The Great East Japan Earthquake, tsunami, and Fukushima Daiichi nuclear power plant accident: a triple disaster affecting the mental health of the country.

The Great East Japan Earthquake in 2011 caused 2 other serious disasters: a tsunami and a nuclear power plant accident. A chronic shortage of mental health resources had been previously reported in the Tohoku region, and the triple disaster worsened the situation. Eventually a public health approach was implemented by providing a common room in temporary housing developments to build a sense of community and to approach evacuees so that they could be triaged and referred to mental health teams. Japan now advocates using psychological first aid to educate first responders. This article extracts

Expanding harm reduction services through a wound and abscess clinic.

A wound and abscess clinic, held concurrently with a syringe exchange, provided economical treatment and aftercare for injection-associated soft tissue infections. During 20 two-hour clinic sessions, 173 treatment episodes were logged, and the visit cost was estimated at $5 per patient. Increased patient-clinician interactions provided opportunities beyond those afforded by the syringe exchange for patients to obtain resources and referrals to services such as HIV counseling and testing, medical care, and drug treatment. Distribution of cards advertising the clinic was substantially less effec
