Allergen immunotherapy for birch pollen-allergic patients: recent advances.

As of today, allergen immunotherapy is performed with aqueous natural allergen extracts. Recombinant allergen vaccines are not yet commercially available, although they could provide patients with well-defined and highly consistent drug substances. As Bet v 1 is the major allergen involved in birch pollen allergy, with more than 95% of patients sensitized to this allergen, pharmaceutical-grade recombinant Bet v 1-based vaccines were produced and clinically tested. Herein, we compare the clinical results and modes of action of treatments based on either a birch pollen extract or recombinant Bet

[Comparative study of 3 types of lancets for performing prick tests].

Skin Prick Test is considered the cutaneous test of choice in rutinary clinical practice as well as in research. In the last 15 years a large number of lancets for puncture test have been developed looking for a higher level of standardisation and reproducibility. We have compared three lancets, two of them, NeoAbelló (NA) and Dome/Hollister Stier (DHS), used in puncture test and a third one, Blood Lancet (BL), in the modified prick test. Twenty-two patients sensitised to Phleum pratense were tested with the three lancets at three extract concentrations and histamine 10 mg/mL in duplicate. We

Resolution of Plaque Keratopathy in a Patient with Atopic Keratoconjunctivitis After Treatment with Leflunomide.

PURPOSE: To report a case of resolution of corneal findings in a patient with atopic keratoconjunctivitis after treatment with leflunomide. METHODS: Case report. RESULTS: A 57-year-old male presented with ocular signs and symptoms consistent with severe atopic keratoconjunctivitis. His case was distinguished by impressive sub-epithelial Salzmann-like nodules in the shape of petaloid plaques in both eyes. These keratinized plaques persisted despite topical steroids, tacrolimus ointment, and routine subconjunctival triamcinolone injections. The patient was started on leflunomide 10 mg daily fo

Vernal keratoconjunctivitis in Down syndrome: a case report.

BACKGROUND: Down syndrome (DS) or Trisomy 21 is the most common chromosomal disease and is characterized by possible heart defects, cognitive impairment and visual disorders. CASE PRESENTATION: We describe for the first time a 17-year-old Caucasian girl suffering from Down syndrome associated with vernal keratoconjunctivitis (VKC), a rare disorder of the anterior segment of the eye, characterized by intense photophobia, redness, watering eyes and itching due to an inflammatory-allergic reaction of the cornea and conjunctiva. On slit-lamp examination, the girl showed conjunctival hyperemia, pa

Dysbiosis of Ocular Surface Microbiota in Patients With Refractive Allergic Conjunctival Diseases.

PURPOSE: We investigated ocular surface microbiota dysbiosis in patients with refractory allergic conjunctival diseases (ACDs; stratified into mild and severe groups) treated with topical tacrolimus. METHODS: Patients (n = 21) with refractory ACDs (including vernal and atopic keratoconjunctivitis) actively treated with topical tacrolimus and 6 healthy controls were evaluated. Based on clinical scores and expression of specific cytokines on the ocular surface, patients with ACDs were divided into mild and severe groups using cluster analysis. The microbial composition of tear specimens collect

Allergy against Steroids in Ocular Inflammation.

Allergy to steroids is an uncommon and harmful condition that leads to a decrease in treatment options and increase in morbidity due to the suboptimal disease control. It can manifest as an immediate response or a delay manifestation. Worsening of underlying condition and not treatment response can indicate corticosteroids hypersensitivity as well. Despite its low prevalence, all clinicians should be aware of this condition and know the treatment options. This narrative review attempts to update information about corticosteroid allergy and we present a case of a patient with the diagnosis of V

Ocular allergy as a risk factor for dry eye in adults and children.

PURPOSE OF REVIEW: To provide an overview of the pathogenic mechanisms underlying the correlation between ocular allergy and dry eye disease (DED), highlighting how the first condition may be a risk factor for the second one. RECENT FINDINGS: Recent advances in our comprehension of the pathogenesis of ocular allergy and DED allow identifying several pathways of interaction between these two conditions. A growing body of evidence supports the role of ocular allergy as a risk factor for DED. Ocular allergy, particularly the severe forms of keratoconjunctivitis, can impact on different key mecha

[Giant papillary conjunctivitis].

Giant papillary conjunctivitis is an inflammation of the conjunctiva, which is associated with immunological-allergic disorders, but is difficult to integrate as a defined type of illness. The deposits of contact lenses are responsible in predisposed wearers. They induce a special immune answer to their biochemical ingredients. In addition, roughness of the superficial corneal layers and the conjunctiva, even without any contact lenses after filtrating glaucoma surgery, leads to mechanically induced papillary formations. In former days these symptoms of building giant papillae were seen mostly

Molecular and cellular biomarkers in dry eye disease and ocular allergy.

PURPOSE OF REVIEW: To summarize recent studies in biomarkers in dry eye disease (DED) and ocular allergy in the last 18 months. RECENT FINDINGS: New significant correlations with clinical symptoms and signs have been described for some tear molecules (proteins, cytokines/chemokines, metalloproteinases, mucins and lipids). Correlation with disease severity has been shown for some of them. New proposed DED biomarkers include some proteins, such as S100A8, S100A9, lipocalin-1, secretory phospholipase A2 and some cytokines and chemokines; mucin-1 mRNA conjunctival expression levels are shown to b

[Therapeutic options in vernal keratoconjunctivitis].

Vernal keratoconjunctivitis (VKC) is a chronic, seasonally exacerbated allergic inflammation of the lids, conjunctiva, and cornea. The pathogenesis of VKC is much more complex than a mere type I hypersensitivity reaction. T-cell-mediated responses, eosinophil attraction, and nonspecific immunological reactions are key mechanisms involved. Cytokines, chemokines, proteases, and growth factors play a major pathogenetic role. Therapeutic options in VKC include mast cell stabilizers, antihistamines, corticosteroids, and immunosuppressive drugs. Newer, more selective therapeutic strategies such as a
