AIMS: Macrophage migration inhibitory factor (MIF) is a pro-inflammatory cytokine found pre-formed in the urothelium. During inflammation, MIF is released into the bladder lumen and bladder MIF mRNA is upregulated. Since MIF also has tautomerase activity and blocking tautomerase activity also blocks MIF's biological activity, we hypothesized that blocking MIF's tautomerase activity would prevent bladder inflammation. We present a patient who developed severe lower urinary tract symptoms following resection of a Ta low grade bladder urothelial carcinoma with immediate post-resection instillation of mitomycin-C. Urine cultures were negative. Radiographic imaging demonstrated a bladder mass. On biopsy she was found to have eosinophilic cystitis. We report the clinical scenario and discuss treatment options. A review of reported cases in the English literature is provided.

Interstitial cystitis (IC) is a disabling bladder disorder that leads patients on an endless journey in search of treatment options to control their painful symptoms. Although there are some medical treatments available, no one treatment is completely effective. The Neuman model provides the rationale for the use of holistic prevention interventions with IC patients. Holistic interventions can help IC patients to retain, attain, and maintain system stability. A case study of 1 IC patient, who successfully used holistic approaches to achieve system stability, is described.
