OBJECTIVE: Major depressive disorder (MDD) is a prevalent and disabling psychiatric condition in Saudi Arabia, with genetic susceptibility remaining incompletely characterized. Reduced brain-derived neurotrophic factor (BDNF) activity has been implicated in MDD. The Val66Met polymorphism (rs6265), involving the substitution of valine (Val, G allele) with methionine (Met, A allele), impairs activity-dependent BDNF secretion. This study examined the frequency of Val66Met and its association with MDD in a Saudi cohort.
MATERIALS AND METHODS: A case-control study was conducted, including 87 patie…

Vitamin D signaling has emerged as a significant modulator of neurobiological processes implicated in major depressive disorder (MDD). Beyond its classical functions in calcium homeostasis and bone health, accumulating evidence highlights antioxidant, neurotrophic, and anti-inflammatory effects mediated by vitamin D-dependent pathways in the central nervous system. By influencing neurotransmitter synthesis, neuroinflammatory and redox pathways, as well as neuroplasticity, vitamin D signaling may contribute to mechanisms underlying depressive symptoms. Therefore, this review aims to provide a c…

Major depressive disorder (MDD) has been increasingly characterized as a network dysconnectivity syndrome. Although single-subject morphological networks are advantageous in studying the brain connectome, extant research on MDD is limited by either small samples or a lack of integration of multi-feature across different morphological features. We used the largest structural MRI data from 1442 MDD patients and 1277 controls to construct individual-level cortical morphological networks based on cortical thickness (CT), cortical volume (CV), surface area (SA), and sulcal depth (SD). Group compari…

Despite the long history of norepinephrine hypothesis in depression, the neuropathology involving norepinephrine remains elusive. This study aims to map the whole-brain functional connectivity (FC) of the major norepinephrine nucleus locus coeruleus (LC) and further investigate the effect of escitalopram, an antidepressant with minimal direct norepinephrine effects, on the LCNE system in patients with major depressive disorder (MDD). Totally 253 MDD patients and 227 healthy controls (HCs) were recruited. The Philips-scanning dataset from Xiangya (52 patients and 88 HCs) served as the discovery…

BACKGROUND: Major depressive disorder (MDD) often presents alongside physical illnesses, such as a high incidence of subclinical hypothyroidism (SHypo) in patients, highlighting the common occurrence of these comorbidities. Recent research has indicated that the presence of comorbid SHypo in individuals diagnosed with MDD may result in notable alterations in both brain structure and function. This study aimed was to investigate the neurological mechanisms underlying this co-occurrence using a data-driven approach to analyze brain activity patterns.
METHODS: Twenty-nine patients diagnosed with…

Major depressive disorder (MDD) affects over 300 million people globally. The etiology of MDD is linked to circadian rhythm disruption, including the diurnal pattern of mood, cognition, and physiological processes. The revised Mood Rhythm Instrument (MRhI-r) was developed to assess self-perceived rhythmicity of symptoms and has previously been tested in nonclinical populations. This study evaluates the psychometric properties of the MRhI-r in a clinical MDD sample. Individuals with MDD and healthy controls completed the MRhI-r at baseline and after 2 weeks. Psychometric properties were assesse…

Sertraline, a selective serotonin reuptake inhibitor, is widely used as a first-line drug for anxiety and depressive disorders. The clinical efficacy and adverse reactions observed with antidepressants are closely related to the concentration of this drug in the patient's blood, but the vast majority of antidepressants demonstrate significant pharmacokinetic variability, leading to pronounced interindividual differences in the steady-state concentration of the drug in the blood and its efficacy even with the same dosing regimen. In this regard, it has become obvious that genetic markers alone …

BACKGROUND: Major depressive disorder (MDD) is a leading cause of disability worldwide. Repetitive transcranial magnetic stimulation (rTMS) guided by structural magnetic resonance imaging (sMRI) has emerged as a promising non-invasive treatment. However, the neural mechanisms underlying its efficacy remain unclear.
METHODS: Thirty-three MDD patients and forty-two healthy controls underwent resting-state functional MRI and structural MRI scans. Among the MDD patients, thirty were rescanned after receiving 10 sessions of sMRI-guided rTMS over 2 weeks. Amplitude of low-frequency fluctuations (AL…

The purpose of this study is to investigate interhemispheric differences (or conversely, symmetry) in the EEG alpha wave in patients with major depressive disorder (MDD) and determine whether the alpha band interhemispheric symmetry changes due to repetitive transcranial magnetic stimulation (rTMS). Twenty patients (n=20) diagnosed with MDD underwent 36 sessions of rTMS. Surface electroencephalography (EEG) was acquired from each subject at baseline and after the 36 sessions of rTMS. Additionally, conventional clinical MDD assessments were conducted pre- and post-treatment and scores from the …

Major depressive disorder (MDD) is characterized by deficits in sensory processing and higher-order executive functions, reflecting dysfunction in the hierarchical organization of the brain. However, current methods for investigating brain hierarchy in MDD have not fully integrated multimodal data, and the underlying biological mechanisms remain poorly understood. We acquired diffusion tensor imaging and functional magnetic resonance imaging (fMRI) data from 100 participants with MDD and 77 healthy controls (HCs). The structural-decoupling index (SDI) was employed to quantify the hierarchical …

Major depressive disorder (MDD) is a substantial public health challenge. Pharmacogenomics (PGx), which identifies genetic variations that predict drug treatment outcomes, may have utility for clinical practice, but adequate representation of all populations is needed. As precision medicine in psychiatry moves towards the use of Artificial Intelligence (AI) and Machine Learning (ML) to predict treatment outcomes using PGx data, representation of diverse populations will be especially important in order to mitigate algorithmic bias and achieve equitable and generalizable findings. This work sou…
