The transanal approach is a relatively new technique for surgery of Hirschsprung's disease. The purpose of this experimental study was to evaluate and compare the transanal endorectal pull-through when it takes place with laparoscopic assistance, according to Soave-Georgeson procedure and without laparoscopic assistance. We used two separated groups of animals. In the 1st group the transanal endorectal pull-through was performed by retrograde transanal mucosectomy with laparoscopic devascularization of the colon and in the 2nd group the transanal endorectal pull-through was performed without l

Intestinal obstruction and perforation are always a challenge for the surgeon, not only in respect to the surgical option offered to the patient, but also to the ability to accurately diagnose and stage the disease. The understanding of the underlying pathophysiological mechanism is also very important in order to classify each patient in order to receive the more appropriate treatment. Mechanisms of obstruction and perforation, methods of diagnosis as well as prevention and treatment of the disease were reviewed.

The management of acute necrotizing pancreatitis has changed significantly over the past years. In contrast to the early surgical intervention of the past, there is now a strong tendency towards a more conservative approach. Initially, severe acute pancreatitis is characterized by the systemic inflammatory response syndrome. Early management is non-surgically and solely supportive. A specific treatment still does not exist. In cases of necrotizing disease, prophylactic antibiotics should be applied to reduce late septic complications. Today, more patients survive the first phase of severe panc

There appears to be a trend in the management of Hirschsprung's disease toward a one-stage surgical correction in the newborn period. After the development of the one-stage laparoscopic-assisted endorectal pull-through in 1995, the minimally invasive approaches have come to dominate the surgical management of this disease. The laparoscopic-assisted pull-through is ideally suited for those patients with aganglionosis confined to the rectum, sigmoid, or proximal left colon. At the time of the operation, a laparoscopic biopsy is recommended to identify, with certainty, the site of the transition

It is of great importance that anaesthetic regimens match surgical procedures in regard to surgical time, in reducing organ dysfunction elicited by the anaesthesia and surgical trauma and by providing optimal post-operative pain treatment, leaving the possibility of early mobilization. New, rapidly eliminated anaesthetic drugs are, by virtue of their pharmacodynamic and pharmacokinetic profiles, optimal for use; combined with continuous thoracic epidurals with local anaesthetics and low-dose opioids, these drugs may permit reduction of various post-operative complications. Minimally invasive s

BACKGROUND: Fecal incontinence is a disabling condition causing social isolation, whose real incidence is difficult to be exactly valued because patients are often reluctant to speak about it, even with their physician, or because they don't trust on surgical procedure. Aim of this study is report our experience in sphincteric reconstruction by direct sphincteric repair in patients suffering from post-traumatic fecal incontinence.
METHODS: Seven patients suffering from fecal incontinence due to traumatic lesions of anal sphincterial apparatus were surgically treated between 1981-2000; in the

The goal of the authors was to prepare a thorough survey of the publications about the application of low molecular weight heparines in deep venous thromboses and pulmonary embolism after abdominal surgery. Several low molecular weight heparines have been introduced in clinical practice since 1980. Till then only standard heparin has been used for deep thromboses prevention. Both drugs have the same or similar anticoagulant or antithrombotic effect. Low molecular weight heparines are more widely used than standard heparin because of the possibility to be administered once daily subcutaneously.

PURPOSE: We describe the use of a gastric segment in performing the antegrade continence enema procedure in patients with refractory fecal incontinence.
MATERIALS AND METHODS: The antegrade continence enema procedure was performed in 4 women and 3 men with refractory neurogenic fecal incontinence. Preoperative evaluation included defecography and anorectal manometry. Operative technique involves tunneling a 10 cm. segment of tubularized stomach isolated along the greater curve with preservation of the right gastroepiploic vessels through the anterior tenia of the colon just distal to the sple

The marked advantages and merit of pre-term and particularly pre-labor (PTPL) cesarean section (C-section) in the avoidance, and indeed, virtual elimination of severely disabling gastroschisis (GS) complications in infants diagnosed prior to birth by ultrasound has unfortunately remained controversial in the 10 to 12 years since it was first reported and strongly recommended by numerous authors. During this period, GS has remained one of the four major causes of the short-gut syndrome (SGS) in infancy and childhood and a major cause of prolonged, costly, complicated, and hazardous neonatal int

BACKGROUND: Perioperative hypotension in patients undergoing gastrointestinal surgery is a known and well-described condition. Timely treatment of perioperative hypotension, regardless of the reason, is important in order to reduce the associated postoperative complications. The treatment consists of vasoactive agents and/or fluids; however, the ideal vasoactive agent in gastrointestinal surgery has yet to be identified.
METHODS: We will conduct a scoping review of studies assessing the use of vasoactive agents in patients undergoing gastrointestinal surgery. We will investigate in which gast

The treatment of multimorbid patients in oncological surgery of the upper gastrointestinal tract requires a differentiated consideration of every single risk factor in order to provide a holistic assessment. This article focuses on pre-existing diseases that are particularly relevant for elective esophageal and gastric surgery and have practical clinical consequences. In this way a differtiation is made between metabolic, vascular, cardiopulmonary and organ-specific risks. The aim of this work is to provide practical guidelines for complex and multimorbid cases. Given the multifactorial interr

BACKGROUND: Crohn's disease (CD) of the duodenum is uncommon, and the goal of treatment is often avoidance of surgery. However, as many as one-third of patients with CD will eventually require surgery for their disease. This study aimed to compare the outcomes and safety of surgical treatments of duodenal CD.
METHODS: In this meta-analysis, a search was conducted for studies that reported outcomes of surgical treatment of duodenal CD. The primary outcome was treatment failure. The secondary outcomes were postoperative complications and reoperation rate.
RESULTS: A total of 10 studies that in

Malnutrition, sarcopenia, and obesity can negatively impact the course of gastrointestinal surgery, increasing the risk of postoperative complications such as anastomotic dehiscence, reoperations, increased mortality and morbidity, and prolonged hospitalizations, leading to higher healthcare costs. Weight loss greater than 10% in the previous six months is a prognostic indicator of mortality in gastrointestinal surgeries and one of the few modifiable variables. Preoperative malnutrition prevalence ranges from 17% to 20%, increasing the risk of infectious complications, especially in malignant
