The antipanic efficacy of alprazolam and lorazepam was evaluated in 48 patients meeting DSM-III criteria for agoraphobia with panic attacks or panic disorder in a double-blind random assignment study. On the basis of rating scale scores, both drugs demonstrated similar efficacy in reducing panic attacks and phobic behavior compared with placebo baseline. The doses required to achieve response were approximately double those required for the treatment of generalized anxiety. A prospective double-blind cross-over trial of oral cisapride 10 mg and placebo was performed to determine the effects of cisapride on the transit of colonic contents in normal humans. Six male volunteers were studied twice using colonic transit scintigraphy. After passing a tube to the caecum, 50 mu Ci of 111Indium diethylene triamine pentaacetic acid were instilled into the bowel lumen. The movement of radiolabelled material was followed using a gamma camera interfaced to a digital computer. In an open trial 25 tremor patients were treated with clozapine in small doses (18-75 mg per day). The effect was measured with a new movement analyzer. Nine of 12 essential tremor patients were greatly improved. In six of nine patients with Parkinson tremor and in two combined essential tremor/Parkinson tremor patients tremor almost disappeared. Sedation is a major side effect, but decreases in most patients with time. The risk of agranulocytosis makes blood control necessary.

Thirty-seven patients with seborrheic dermatitis were treated topically with a 2% ketoconazole cream or its vehicle control in a double-blind study. The efficacy of transdermal scopolamine, oral meclizine, and placebo in protection against motion sickness was compared in a double-blind crossover study. Thirty-six healthy subjects were exposed to motion three times for 90 min in a ship-motion simulator. Transdermal applications were made and tablets were taken at least 12 and 2 hr before exposure to motion. Transdermal scopolamine provided better protection than placebo or meclizine. Dryness of mouth was the only side effect reported more frequently for one regimen, transdermal scopolamine.
