[Potential risks, adverse effects and drug interactions associated with herbal medicine in dental patients].

Herbal medicine is an increasingly common form of alternative therapy in the United States. Most herbal products are considered dietary supplements and thus are not regulated as medicines. They are marketed without prior approval of their efficacy and safety by the Food and Drug Administration (FDA). Some herbal medications have potentially harmful side effects as well as adverse interactions with conventional drugs. The adverse reactions involve all systems, age groups and severity. It is important for dentists and other health care providers to obtain adequate information as to what herbal m

Anti-retrovirals and immunosuppressive drug interactions in a HIV-positive patient after liver transplantation.

We report a case of drug-related toxicity after liver transplantation for hepatocellular carcinoma in a HIV-HCV co-infected patient. Before transplant the patient was on a triple antiretroviral therapy (zidovudine and lamivudine and efavirenz) with a stable CD4+ cell count >500 cells/microL. Liver transplantation was performed with a liver graft showing a 10% of macrosteatosis and with a graft-to-recipient body weight ratio of 1.3. Immunosuppression was achieved with tacrolimus, azathioprine and steroids. The antiretroviral therapy was resumed in the first postoperative day as the early graft

Application of differential flow microcalorimetry for study of drug interactions in the blood system.

A compact differential flow microcalorimeter has been developed to investigate biomolecular reactions, especially drug interactions in the blood system. The calorimeter is an adiabatic type and consists of a twin-cell structure, each mixing part having a volume of 60 microliters. Both the precision and accuracy of the instrument have been evaluated by dilution of sucrose solutions to be 0.1-0.5% at a heat effect of 100-10 microW. The resolution is approximately 0.5 microW (less than 10(-3) Torr). The heat produced in erythrocyte hemolysis induced by chlorpromazine hydrochloride (CPZ) and the b…

Drug interactions with oral contraceptives: compilation and analysis of an adverse experience report database.

Over 700 alleged OC/drug interactions were reported for antituberculous drugs, other antibiotics, anticonvulsants, antidepressants, and analgesics. Fewer than ten reports of OC/drug interactions were found involving antihistamines, thyroid hormone, vitamin C, antacids, ulcer medication, or diuretics. These may represent a set of OC/drug interaction problems that need to brought into medical awareness. Pregnancy is the first event reported when OCs appear to interact with another drug. However, menstrual disturbances are reported more often. BTB is the most frequently reported menstrual disturb…

MetalloDock: Decoding Metalloprotein-Ligand Interactions via Physics-Aware Deep Learning for Metalloprotein Drug Discovery.

Accurate prediction of metalloprotein-ligand interactions is critical for metalloprotein-targeted drug discovery. Conventional docking tools and existing deep learning (DL) models fail to reliably capture metal-ligand interactions, hampering the discovery of potent metalloprotein inhibitors. Here, we propose MetalloDock, the first DL-based docking framework specially designed for metalloprotein targets. By innovatively integrating an autoregressive spatial decoding engine with a physics-constrained geometric generation paradigm, MetalloDock can precisely reconstruct metal coordination geometri

Potential Drug Interactions in Psychiatric Patients Undergoing Pangenotypic Therapy for Hepatitis C Virus Infection.

Over the past decade, significant progress has been made in the treatment of chronic hepatitis C virus (HCV) infection. The introduction of direct-acting antivirals (DAAs) has revolutionized the treatment of HCV infection, offering nearly 100% efficacy. Furthermore, additional therapeutic regimens with pangenotypic efficacy have been registered. These drugs are also characterized by a few adverse events and good treatment tolerance. As DAA therapy is now accessible to virtually all patients, including those with multimorbidity who often take multiple medications, drug interactions (DDIs) have …

In Vitro and Clinical Evaluation of Potential Interactions of Bemnifosbuvir with Drug-Metabolizing Enzymes.

Bemnifosbuvir is a novel oral guanosine nucleotide prodrug with potent pan-genotypic inhibitory activity against hepatitis C virus. In vitro studies assessing the inhibition or induction potential of bemnifosbuvir on the CYP450 and UGT1A1 enzymes demonstrated that bemnifosbuvir is a weak inducer and a reversible and time-dependent inhibitor of CYP3A4. These results prompted further evaluation in a Phase 1 clinical study in healthy participants who received midazolam (a sensitive CYP3A4 substrate) without and with simultaneous or staggered doses of bemnifosbuvir. A single simultaneous 550 mg be

Physicochemical interactions between oral drug products: An integrated in vitro and PBPK modeling approach.

While pharmacodynamic and pharmacokinetic drug-drug interactions are well established, physicochemical interactions between co-administered oral drug products and their impact on drug absorption remain poorly understood. This study combined biorelevant in vitro testing and physiologically based pharmacokinetic (PBPK) modeling to investigate such interactions between three commercially available drug products SPORANOX (itraconazole), ACTOS (pioglitazone) and GEVILON (gemfibrozil). The in vitro experiments revealed contradictory effects. The apparent solubility of itraconazole was improved in th…

Tuning enantioselective drug adsorption in isoreticular homochiral metal-peptide frameworks through proximity pore interactions.

Pharmaceutical research emphasizes stereochemistry and the enantioseparation of racemic drugs because different enantiomers can exhibit varying pharmacological and toxicological properties when interacting with the body's metabolic pathways. Metal-organic frameworks (MOFs) are porous adsorbents that can be designed to possess homochirality within their structures, enabling tunable porosity and enantioselective adsorption of racemic drugs. In our study, we present the synthesis of five novel and homochiral Co-L-GG(R) MOFs (where L-GG = glycyl-L(S)-glutamic acid and R = bipyridyl (bipy) pillar l

Assessment of potential drug interactions in rheumatology: a comparative study of decision support software.

BACKGROUND: Rheumatologic diseases often require long-term, complex pharmacotherapy involving immunosuppressive and anti-inflammatory agents. This complexity leads to a high prevalence of polypharmacy and increases the risk of potential drug–drug interactions (pDDIs), which may reduce efficacy, cause adverse effects, prolong hospitalization, and increase costs. Clinical decision support systems (CDSSs) are used to detect pDDIs, but inconsistencies in knowledge bases, severity classification, and evidence sources challenge their reliability. OBJECTIVES: To identify pDDIs in rheumatology patient…

Pharmacogenomic and drug interactions risk in cardio-oncology: A precision medicine perspective for India.

Cardio-oncology patients may face complex treatment regimens due to the concurrent existence of cancer and cardiovascular disease, leading to a considerable polypharmacy burden. This significantly increases the prospect of drug-drug interactions (DDIs) and gene-drug interactions. The majority of these interactions arise from comparable pharmacokinetic and pharmacological pathways associated with drug transporters and cytochrome P450 enzymes. The significance of pharmacogenomics in tailored treatment strategies are emphasised by the fact that genetic variability enhances individual differences …

Outcomes following exposure to drug interactions with ibrutinib in patients with chronic lymphocytic leukaemia.

Ibrutinib is metabolized by cytochrome P450 3A (CYP3A) and poses challenges with drug interactions. We conducted a population-based cohort study of Ontario residents aged ≥66 years who initiated ibrutinib for chronic lymphocytic leukaemia (CLL) to evaluate the frequency of potential drug interactions involving moderate/strong CYP3A inhibitors and inducers and their association with overall survival (OS). Secondary analyses employed a nested case-control design examining hospitalizations for haemorrhage or infection as potential markers of ibrutinib toxicity. Among 642 ibrutinib recipients (med…

Molecular basis identification and hypnotic drug interactions for cognitive impairment related to sleep deprivation.

Chronic sleep deprivation can lead to cognitive impairment which makes it difficult to think, focus, and make comprehensive decisions. This in turn leads to the progression and increased risk of several diseases. This study aimed to explore potential drug targets and biomarkers underlying the increased disease risk due to sleep deprivation, including stress responses, immune dysfunction, and metabolic dysregulation. Four datasets namely GSE40562, GSE98566, GSE98582 for sleep deprivation, and GSE26576 normal brain cells were utilized to understand the molecular basis and potential drug targets

Gut-brain-immune interactions: exploring probiotics as a drug delivery platform for neurological disease.

The gut-brain-immune (GBI) axis, connecting gut microbes, neural tissue, and the cells of the immune system, plays a critical role in human health, particularly in relation to neurological diseases. Research in this field over the last few decades shows that disruptions in the microbiome have been linked to chronic inflammation, which may contribute to neurological conditions, including Parkinson's disease, Alzheimer's disease, and other mental health disorders. As we gain a greater understanding of the links between these systems, novel therapeutic strategies are being explored to treat disea

Unraveling Epistatic Interactions Between Sites Under Drug-Dependent Selection in the Mycobacterium tuberculosis Genome.

Many Mycobacterium tuberculosis genome sites experience different selective forces depending on whether a patient is treated with antibiotics. Here, we searched for pairs of such sites that evolve interdependently. We reconstructed the phylogeny of more than 11,000 Mycobacterium tuberculosis clinical isolates with known phenotypes for at least one of the 13 antitubercular drugs. By analyzing the distributions of substitutions and phenotypic state changes on the phylogeny, we identified sites where substitutions were associated with the acquisition of drug resistance or occurred preferentially
