Fixed combination drug therapy.

Almost half of all marketed drugs are fixed combination preparations, yet the World Health Organization included only 7 in its list of 240 "essential' drugs. There has been little scientific study of these preparations but much emotion and rhetoric has been expended both for and against them. Potential advantages of fixed combination preparations include: increased compliance, synergy and increased efficacy, and reduced side effects and cost. Potential disadvantages include: inflexible fixed dose ratio, incompatible pharmacokinetics, increased toxicity, and physician and pharmacist ignorance o

Terlipressin-related Ischaemic Necrosis of the Skin: A Rare Complication.

UNLABELLED: Terlipressin is used for the treatment of bleeding oesophageal varices and hepatorenal syndrome in patients with cirrhosis. Adverse effects are usually minimal. However, potentially serious side effects such as skin necrosis involving the extremities, scrotum, trunk and abdominal skin can rarely occur. Our patient had greater skin involvement than other described cases. We present the case of a patient with extensive skin necrosis, unrelated to the infusion site, in the lower and upper limbs, scrotum and abdomen after the use of terlipressin. Skin necrosis secondary to terlipressin

Exploring Risk Drugs and Mechanisms in Dystonia: Insights from Pharmacovigilance and Proteogenomics.

INTRODUCTION: Dystonia is a movement disorder (MD), which is the third most common MD after Parkinson's disease and essential tremor. Although drugs are one of the main risk factors for dystonia, they are often not fully recognized. This study aims to identify drugs related to dystonia and explore the potential molecular mechanism of drug-induced dystonia. METHODS: We used disproportionality analysis to analyze the data of the FDA Adverse Event Reporting System (FAERS) to identify risk drugs associated with dystonia. The molecular target information of these drugs comes from the DrugBank data

Costs are a major driver of antibacterial drug prescriptions in Germany: market analysis from 1985 to 2022.

In recent years, several threatening developments regarding antibacterial drugs, such as rising bacterial resistance and delivery bottlenecks, have occurred. Since antibacterial drugs are crucial for modern medicine, understanding events and influencing factors relevant for long-term developments is essential. Therefore, we analyzed the number of prescriptions and costs, defined daily dose (DDD) and DDD costs of antibacterial drugs in Germany, based on the Arzneiverordnungsreport (AVR, Drug description report) between 1985 and 2022. Based on prescription rates in 2022, we selected the TOP15 an

Repurposing sulfonamide drugs as anticancer ligands and understanding its properties through density functional theory.

Drug repurposing represents a promising approach towards drug discovery that has the potential to improve patient outcomes and address unmet medical needs. This study attempted to repurpose existing sulfonamide drugs in search of novel anticancer drugs because of their effectiveness in treating bacterial infections. A search was made in DrugBank for Sulfonamide, and 25 drugs with functional groups like SH, OSO, CS, and -S- were chosen for our study. The drug properties, such as dipole moment, volume, polarisability, highest occupied molecular orbital (HOMO), lowest unoccupied molecular orbital

Essential tremor: treatment with propranolol.

A double-blind trial of propranolol, in a dosage of 120 mg a day, and placebo was conducted in 11 patients with essential tremor. An improvement in tremor was observed in all patients who received propranolol and was most pronounced in the upper extremities. The favorable clinical assessment was further confirmed by improvement in a grooved pegboard test and in the quality of handwriting as evaluated by two "blind" observers. No side effects were encountered. It is concluded that propranolol is at present the best available drug for the treatment of essential tremor.

Challenges in Designing Clinical Trials to Test New Drugs in the Pregnant and Breastfeeding Woman.

Until recently, pregnant or lactating women were systematically excluded from drug development. Currently, the ethical consensus is that women who are pregnant or lactating need to be involved in drug development. The rich experience of drug development in HIV/AIDS can be generalized. This article reviews contemporary, integrated, multidisciplinary best practice in the design and conduct of clinical trials that recruit women who are pregnant or lactating and shows how contemporary preclinical methods facilitate clinical studies while minimizing the use of nonhuman animal species. Zebrafish as an Experimental and Preclinical Model for Alzheimer's Disease.

The success rate of novel drug candidates in clinical trials relies on the safety and efficacy data of the preclinical studies. Although cell-based assays are widely used, the complexity of an in vivo system to mimic human disease pathophysiology is essential. Despite the wide usage of rodent models for preclinical drug discovery, increasing the repertoire of animal models that allow the investigation of various pathological mechanisms with a unique operational strength for drug discovery is required. Zebrafish, a teleost vertebrate, with its high similarity to human pathophysiology and unique

Drug-resistant malaria in children and in travelers.

Malaria remains a significant cause of childhood morbidity and mortality worldwide. Drug resistance in Plasmodium falciparum has become widespread in the past 30 years, and in some parts of the world multidrug resistance is common. Chloroquine resistance in Plasmodium vivax has recently been recognized in Indonesia. The mechanisms of drug resistance have been defined for the antifolate antimalarial agents but remain incompletely understood for the quinolines. Judicious use of antimalarial compounds will be essential to prevent the emergence and spread of further drug resistance. The history, g

The inhibitory effect of antiretroviral drugs on the L-carnitine uptake in human placenta.

In spite of remarkable reduction in the number of children born with HIV due to antiretroviral therapy, concerns remain on the short- and long-term effects of antiretroviral drugs at the feto-placental unit. Cardio- and skeletal myopathies have been reported in children exposed to antiretroviral drugs prenatally. These conditions have also been described in perturbed placental transfer of l-carnitine, an essential co-factor in fatty acid oxidation. Due to limited fetal and placental synthesis, carnitine supply is maintained through the placental carnitine uptake from maternal blood by the orga

Identifying Foundational Drug Knowledge in Pharmacy Education Using a Structured Consensus Framework.

BACKGROUND: Foundational drug knowledge is a central expectation for pharmacy practice-readiness, yet programs lack consensus-based guidance on which medications to prioritize or how intensively to assess them. Programs often use volume-derived drug lists as a pragmatic starting point, but such lists may not fully reflect the breadth of medications encountered across diverse practice settings. The purpose of the study is to identify a consensus-based foundational drug list reflective of the breadth of pharmacy practice. METHODS: A multi-phase consensus study was conducted using the Lawshe met

[Clinical evaluation of the hypotensive drug catapresan].

A clinical and laboratory examination was conducted with a hypotensive drug Katapresan used alone and in combinations with Reserpine and saluretics in 30 patients with stage II and III of essential hypertension. Katapresan was shown to be an active hypotensive agent, often effective in patients resistant to other drugs. For long-term therapeutic courses the most effective combination is Katapresan with Hypothiazide and Reserpine. Katapresan may result in a deceleration of atrioventricular conduction, a slowing down of the electric systole, a reduction of the contractile function of the myocard

Substance abuse and movement disorders.

The complex relation between movement disorders and substance abuse is reviewed. First, we discuss the wide variety of movement disorders that occur as a direct consequence of acute use or withdrawal of drugs of abuse, such as alcohol, cocaine, heroin, amphetamine and methcathinone. Second, we describe the recent advances in the comorbid relationship between alcoholism and two movement disorders: essential tremor and myoclonus-dystonia. Lastly, we discuss the abuse potential of the dopaminergic agents, apomorphine and levodopa, in patients with Parkinson's disease.
