The epidemiology and management aspects of 212 consecutive cases of foreign bodies of the ears and nose in children presenting to an urban pediatric walk-in and emergency care facility were retrospectively reviewed. The items most commonly removed from children's external auditory canals were roaches, paper wads, toy parts, earring parts, hair beads, eraser tips, and food. High-level resistance to gentamicin among clinical isolates of enterococci has been found with increasing frequency in recent years. In this issue, Zervos and colleagues report findings from a prospective study in which they assessed the frequency of colonization and infection with such organisms at a university medical center, demonstrating probable person-to-person spread. In a pilot study 29 cases of suppurative otitis were treated with ciprofloxacin. Acceptable clinical results were obtained in case of chronic otitis media without cholesteatoma and in case of otitis externa. Cholesteatoma and mastoid cavity cases gave no satisfactory results. Guidelines are proposed to define the specific indications and place of this drug in suppurative otitis.

Jewelry wearing among very young children is increasing. Ear piercing is being practiced even in neonates. Accompanying these phenomena is the ever-present risk of ingestions and aspirations of small jewelry items, notably earring pieces. We present two cases that illustrate some of the real and potential hazards of these small jewelry pieces. Relatively safe jewelry items and specific safety measures are discussed.

The skin response to UVB irradiation in mice was evaluated by means of ear swelling. A developmental aberration significant enough to cause a congenital auricular atresia may well entrap ectodermal keratinous elements medial to an atresia plate, resulting in a primary cholesteatoma. We report such a case and discuss the proposed pathogenesis and pertinent embryology. A two-stage surgical procedure is offered as a reasonable management alternative when the cholesteatoma is well encapsulated.

A 5-year-old female with a so-called "salivary gland choristoma" in the middle ear is reported, believed to represent the nineteenth such case in the literature. The nineteen cases are reviewed. The only consistent symptom was deafness. Aural discharge, tinnitus and otalgia occurred occasionally. A case of ankylosing spondylitis with aortic valve disease and hearing loss is described. A series of radiographic and audiometric investigations showed the hearing loss to be of a conductive type. It seemed most likely that the hearing loss was related to an inflammatory involvement of the ossicular joints due to the primary disease. No other case of conductive hearing loss has previously been reported due to otoarthritis in ankylosing spondylitis. This is important both theoretically and practically.

Noise-induced hearing loss was assessed in unilaterally sympathectomized rats exposed one month to 100 dB Leq(lin) noise. The systolic blood pressure of these animals was in the range of 110-140 mmHg. Auditory sensitivity was determined electrophysiologically by brainstem response audiometry to 1/3-octave filtered clicks. The results showed that the loss of sensitivity was about 10 dB smaller (p less than 0.01) on the sympathectomized side.
