Proximal ulnar fractures in adults: a review of 163 cases.

The aim of this study was to report the epidemiological characteristics and the experience of 5 departments of trauma, in France, in the management of fractures of the proximal ulna. 163 patients with fractures of the proximal ulna with a mean age of 49.9 years (range 16-97) were managed. The most common mode of injury was a motor vehicle collision (48%). 18% sustained associated injuries to the ipsilateral limb. Open fractures were present in 42 patients (25%). A total of 109 patients had a fracture of the olecranon, with the Mayo 2A and B types most frequently seen (66%). The patients were i

Acute disassembly of a bipolar radial head arthroplasty.

The GUEPAR(®) implant is a metallic bipolar radial head prosthesis designed to treat comminuted radial head fractures when anatomic realignment of the articular surface of the radiocapitellar joint is not possible. We report herein the rare case of an acute complete disassembly of this implant, discuss the reason for this occurrence and review the literature. In the presented case, the complete removal of the prosthesis provided a satisfactory outcome with an excellent Mayo Elbow Performance Score at 12months follow-up.

The sequelae of a missed Essex-Lopresti lesion.

Radial head fractures are the most common type of elbow fracture in adults. Unrecognised disruption of the intraosseous membrane at the time of injury can lead to severe wrist pain from proximal radial migration especially if the radial head is excised. In this case, despite anatomical reduction and internal fixation of the radial head fracture, longitudinal forearm instability developed after delayed radial head resection was performed 7 months post-injury. A Suave-Kapandji procedure was performed due to ongoing wrist pain. Because of the previous radial head resection, this led to a floating

Bruck syndrome (osteogenesis imperfecta with congenital joint contractures): review and report on the first North American case.

We describe a patient who was born with flexion contractures and pterygia at the elbows, clubfeet, torticollis, and several rib fractures. During infancy and childhood, multiple fractures of the lower limbs occurred with minimal trauma and led to disabling deformities. When evaluated at age 19 years, he was normally intelligent, but extremely short, with severe kyphoscoliosis compromising his pulmonary function. Pterygia limited elbow extension to 90 degrees, and severe lower limb deformities prevented ambulation. He did not have blue sclerae, dentinogenesis imperfecta, or hearing loss. X-ray

Distal humeral AO type C fracture--analysis of LCP fixation.

BACKGROUND: Distal humeral fractures of type C by AO are serious musculoskeletal injuries, affecting the function of the elbow and upper limb. The authors present the results of treatment of these injuries with the use of two LCP plates. MATERIAL AND METHODS: We report on 25 patients aged 26-75 years who were operated on between 2008 and 2010. We examined 20 patients. Treatment results were evaluated basing on patient histories (Oxford Elbow Score), clinical examination (Mayo Elbow Score) and radiographs. We rated the percentage loss of movement in relation to the healthy limb. RESULTS: Bony

[Progress on the treatment of terrible triad injury of elbow joint].

As to terrible triad injury of elbow joint, because of the complexity of the surrounding anatomy of the elbow joint, the limited biomechanical study, the difficult treatments, and the poor prognosis, the choice of the treatment scheme is still controversial. This article summarizes the anatomy, injury mechanism, surgical treatment and prognosis of elbow joint by retrieving a large number of references. Currently, there are various surgical treatments for triad injury of elbow joint, but due to individual differences, the clinical efficacy is mostly uncertain. In the treatment of triad injury o

[Volkmann's contracture and suprcondylar fractures of the humerus in children].

The most frequent cause of the compartment syndrome with subsequent Volkmann's contracture of the upper extremity in children is a supracondylar fracture of the humerus. Often it is being associated by some with the concurrent injury and obliteration of the brachial artery. In the majority of cases of Volkmann's contracture in children with dislocated supracondylar fractures of the humerus, described in the available literature, the development of Volkmann's contracture was preceded by reduction of the fracture and fixation with bandage or in cast with the elbow in flexion. The aim of this stu

Failure of metal radial head replacement.

There is little information available at present regarding the mechanisms of failure of modern metallic radial head implants. Between 1998 and 2008, 44 consecutive patients (47 elbows) underwent removal of a failed metallic radial head replacement. In 13 patients (13 elbows) the initial operation had been undertaken within one week of a fracture of the radial head, at one to six weeks in seven patients (seven elbows) and more than six weeks (mean of 2.5 years (2 to 65 months)) in 22 patients (25 elbows). In the remaining two elbows the replacement was inserted for non-traumatic reasons. The mo

[Concomitant fractures of the capitulum humeri and the caput radii].

This case report describes the osteosynthetic treatment and postoperative course of a fracture of the capitulum humeri and a concomitant fracture of the head of the radius with a follow-up over 3 months. Simultaneous fractures of the capitulum humeri and the head of the radius are rare injuries of the elbow. Due to the complex anatomical relationships this type of fracture poses a big challenge for treating traumatologists.

Transverse coronoid fracture: when does it have to be fixed?

BACKGROUND: After elbow fracture-dislocation, surgeons confront numerous treatment options in pursuing a stable joint for early motion. The relative contributions of the radial head and coronoid, in combination, to elbow stability have not been defined fully. QUESTIONS/PURPOSES: The purpose of this study was to evaluate the effect of an approximately 50% transverse coronoid fracture and fixation in the setting of an intact or resected radial head on coronal (varus/valgus) and axial (internal and external rotational) laxity in (1) gravity varus stress; and (2) gravity valgus stress models. Radial Head Arthroplasty: A Systematic Review.

BACKGROUND: Despite the expanding body of literature on radial head arthroplasty, the increasing understanding of elbow anatomy, biomechanics, and kinetics, and the evolution of surgical techniques and prosthesis designs, there is currently no evidence to support one type of radial head prosthesis over another. The purposes of the present report were to review the literature and to explore the association between prosthesis design variables and the timing of surgery and the outcome of modern radial head arthroplasty. METHODS: The literature search was limited to studies involving skeletally m

Paediatric bilateral 'floating elbow'.

A child sustained an ipsilateral supracondylar humerus (SCH) and distal both-bone forearm fractures bilaterally, in addition to facial injuries, following a fall from height. He was managed surgically by closed reduction and pinning for both SCH and distal end radius fractures bilaterally. At the final follow-up, all the fractures had united uneventfully, and he had no functional limitations or cosmetic concerns. We conclude that a floating elbow in the paediatric population is an uncommon injury, and the bilateral scenario is even rarer. One should be vigilant for compartment syndrome; early

Analysis of twenty-five cases of terrible triad injury of the elbow surgically treated with a single lateral approach.

BACKGROUND: The objective was to analyze the long-term clinical outcome of terrible triad injury of the elbow following surgical intervention with a single lateral approach. METHOD: We treated twenty-five patients with terrible triad injury of the elbow from July 1, 2017, to June 30, 2020, and performed post hoc analysis. All patients underwent standardized surgery which entailed: plate fixation for the ulnar coronoid process, fixation or replacement of the radial head, and repair of the lateral ligament but not the medial collateral ligament. RESULTS: We followed up each patient for an aver

K-wire versus screws in the fixation of lateral condyle fracture of humerus in pediatrics: a systematic review and meta-analysis.

BACKGROUND: Lateral condyle fracture of the humerus is the second most common elbow fracture in children. Non-displaced lateral condyle fractures can often be managed with cast and conservative care, while reduction and fixation are often used to treat displaced lateral condyle fractures. Traditionally, K-wire fixation has been used for displaced lateral condyle fractures, but recently fixation using screws has been advocated in some studies. Therefore, we performed a meta-analysis to determine the difference in outcomes and complications between the two different fixation methods for the trea

Incidence and analysis of simultaneous bilateral radial head and neck fractures at a level I trauma center.

BACKGROUND: Reviewing the current literature, very few reports are given on simultaneous bilateral radial head and neck fractures. There are no reports on this entity's incidence. Thus, the purpose of this study was to analyze the incidence and outcome of simultaneous bilateral radial head fractures. METHODS: This study reviewed the clinical records and trauma database of this Level I Trauma Center and identified all adult patients with fractures of the radial head or neck who were admitted between 1992 and 2007. From a database of 2,296 adult trauma victims with radial head or neck fractures

Case Report: An isolated medial condylar fracture of the distal humerus in an adolescent baseball pitcher.

BACKGROUND: Thrower's fracture, a non-traumatic humerus fracture related to the throwing motion, has been reported in a few studies in the literature. To the authors' knowledge, there has been no report on an isolated medial condylar fracture of the distal humerus induced by the throwing motion to date. In this study, we provide a potential explanation to the pathomechanism of such a rare type of thrower's fracture. CASE PRESENTATION: We present a rare case of a 17-year-old Asian adolescent male who suffered an isolated medial condylar fracture of the distal humerus after baseball pitching. A

Radial Head Dislocation with Ipsilateral Proximal Shaft of Radius Fracture: A Case Report.

Radial head dislocations are uncommon in adults. They are commonly seen in children and are generally associated with proximal ulna fracture. Radial head dislocation with associated proximal radial shaft fracture is rarer than isolated radial head dislocation in adults. Due to the rarity of this complex injury, in the absence of keen observation and meticulous attention, the correct diagnosis might be missed leading to unsatisfactory management and related complications. Here, a similar case of radial head dislocation with associated proximal radial shaft fracture has been presented.

Intra-articular fractures of the upper extremity: new concepts in surgical treatment.

New techniques of internal fixation, postoperative rehabilitation, and emphasis on functional as well as radiographic outcome have refined the surgical treatment of complex fractures of the glenoid, humeral head, supracondylar and intracondylar humerus, olecranon, radial head, distal radius, and distal radioulnar joint over the past decade. Early stabilization and rehabilitation of these injuries leads to soft-tissue stabilization and facilitates the patient's ability to place the hand in three-dimensional space.

Technique for ensuring ulnohumeral reduction during application of the Richards compass elbow hinge.

The compass elbow hinge is an externally applied device that allows both active and passive range of motion after placement. It is effective for treatment of posttraumatic elbow contractures and of the arthritic elbow with posttraumatic arthritis and also in cases of acute instability despite radial head replacement or fracture fixation and ligamentous repair. The device is technically challenging to apply and requires that the assistant maintain ulnohumeral reduction during application, particularly of the ulnar pins. We describe a simple technique that ensures reduction and shortens surgical
