Neuroimaging of middle cerebral artery anomalies: a report of three cases.

PURPOSE: This is a description of the main middle cerebral artery (MCA) anomalies from the neuroradiological and embryological point of view … these variants and the association with aneurysms on other cerebral arteries. The knowledge of the embryology of these variants is of paramount importance in order to make easier their identification and reporting and to enhance their understanding. CONCLUSIONS: MCA anomalies should be known and carefully checked in order to avoid misdiagnosis with disease …

Complex Facial Clefts.

The embryology, classification, evaluation, and reconstructive management of complex craniofacial clefts, a group of exceedingly rare and highly variable congenital anomalies, is presented. Using the Tessier system as a structural framework, the characteristic features of each cleft subtype and highlights, key diagnostic considerations, including assessment of soft-tissue deficiency, skeletal disruption, and associated functional concerns are described. Emphasis is placed on the application of fundamental reconstructive principles: respect for facial subunits, restoration of symmetry, and stag…

Bladder prolapse through a patent urachus presenting as an umbilical mass in the newborn: characteristic prenatal sonographic findings and the diagnostic benefit of postnatal cystography.

Bladder prolapse through a patent urachus is rare. We present a newborn with an unusual exophytic, erythematous umbilical mass. Voiding cystography readily demonstrated continuity of the bladder dome with the umbilical mass, confirming bladder prolapse through a patent urachus. The diagnosis of bladder prolapse was rapidly made in a second newborn with similar imaging and clinical findings and confirmed by cystography. We discuss the embryology of this condition including the association with a vesico-allantoic cyst in utero. Pre- and postnatal images are presented. The use of cystography in d…

The filum terminale internum and externum: A comprehensive review.

INTRODUCTION: The filum terminale has oven been overlooked in the literature probably due to its small size and historical lack of research on its true morphology. However, this structure's roll in the tethered cord syndrome has become more apparent. Therefore, the current comprehensive review seemed timely. METHODS: Using standard search engines, the history, embryology, anatomy, pathology and surgery of the filum terminale were reviewed. CONCLUSIONS: It is only recently that the true anatomy and pathological involvement of the filum terminale in the tethered cord syndrome have been elucida…

The missing falx: a potential surgical pitfall during interhemispheric transcallosal approach.

One of the standard surgical approaches to lateral ventricular tumors involves interhemispheric dissection along the falx cerebri. The falx cerebri thus provides an important landmark in guiding the surgeon to the midline. Unrecognized absence of falx may lead to surgical complications. A child with tuberous sclerosis presented with refractory epilepsy and a lateral ventricular tumor. The tumor was excised by an interhemispheric trans-callosal approach. The midline falx cerebri was absent anteriorly, rendering the identification of midline difficult. The embryology of a deficient falx and its …

Neonate With Imperforate Anus and Meconium Streak Along the Penile Shaft.

We present a neonatal male with a rectopenile fistula in the setting of imperforate anus. Reported cases of fistulae exiting on the penile shaft are exceedingly rare. The patient had a diverting colostomy performed within the first 48 hours of life and imaging later revealed no communication with the urinary tract. At 4 months of age the child was brought to the operative suite for penile exploration and posterior sagittal anorectoplasty. The patient's colostomy was reversed 4 months later. We discuss the embryology and management of imperforate anus as well as literature review.

Congenital agenesis of the superficial posterior compartment calf muscles in a 13-month-old infant.

Muscle agenesis may induce cosmetic and functional deficits, particularly if the muscle is an axial limb or a large muscle. Limb muscle agenesis is a rare condition. Here, the authors report the case of a 13-mo-old girl with unilateral atrophic calf and gait abnormality. Magnetic resonance imaging confirmed agenesis of the posterior superficial compartment of the calf. The patient showed an out-toeing calcaneal gait and fibular length discrepancy secondarily during growth. Normal embryology and the differential diagnostic point of foot deformity as well as the clinical implications of calf age…

The tricuspid valve in adult congenital heart disease.

The tricuspid valve is frequently affected in adults with congenital heart disease but is also frequently overlooked. Disease of this valve can occur primarily or develop secondary to changes in the right ventricle caused by other disease states. The embryology and anatomy of the tricuspid valve are important to understanding pathogenesis of valve dysfunction in congenital heart disease. Clinical findings can be subtle. Multimodality imaging may be necessary to fully assess the cause and impact of tricuspid valve lesions. More research is needed in pathophysiology, imaging, and treatment in th…

Polydactyly: a review.

Polydactyly of the hand is a difficult problem and poses a unique challenge for the hand surgeon. The embryology of limb development is complex, leading to a host of different phenotypes of polydactyly. Polydactyly can occur in any digit and is described as preaxial, postaxial, and central, based on location. Classification systems exist for each of these locations, which guide treatment options. Surgical treatment needs to address the aesthetic and functional aspect of hand reconstruction. Careful consideration and planning of surgical treatment individualized to each patient is required to o…

Management of supraglottic dysgenesis presenting as laryngomalacia.

Laryngomalacia is a common source of stridor and can lead to significant upper airway obstruction and feeding disturbances in infants. We describe a unique case of supraglottic dysgenesis presenting as laryngomalacia featuring a prominent "s-shaped" epiglottis with both posterior edges fused to the right aryepiglottic fold/arytenoid complex. Although this anomaly is not accounted for in any of the current laryngomalacia classification schemes, modified laser supraglottoplasty was a satisfactory approach leading to successful decannulation. Laryngeal embryology and possible timing of the pathog…

Dorsal dimelia of a thumb.

Dorsal dimelia (congenital palmar polyonychia) of the hand is defined as the presence of an ectopic palmar nail at the tip of the finger with evidence of dorsalisation of the palmar skin. All reported human cases involved the little or ring fingers and none of the affected digits were subjected to detailed examination. We report the first human case of dorsal dimelia involving a thumb, and provide gross and histological examination of the affected digit. We also review the embryology of this rare deformity and previously reported cases.

Congenital nasal malformations.

The nose is a prominent feature of the human face. Congenital malformations of the nose, whether functional or anatomic, affect the physiologic and psychologic wellness of children who have these anomalies. Congenital nasal abnormalities may be overt or subtle and can occasionally cause life-threatening emergencies at birth. A discussion of nasal embryology and development provides the basis for the discussion of some of the important congenital abnormalities seen in clinical practice. The final portion of the article is devoted to several of the more common syndromes in which nasal abnormalit…

A persistent left superior vena cava.

Persistent left superior vena cava is the most common form of anomalous venous drainage involving the superior vena cava, and represents persistence of the left horn of the embryonic sinus venosus, which normally involutes during normal development to become the coronary sinus. Almost always, a persistent left superior vena cava enters the right atrium through the orifice of an enlarged coronary sinus. In this case report of a 60-year-old male cadaver, we describe a persistent left superior vena cava and discuss its embryology and clinical significance.

Rare case of ectopic duplicated megaureter with dysplastic upper kidney moiety opening into ipsilateral ejaculatory duct.

A rare case of left mesonephric duct malformation consisting of a duplicated ectopic megaureter opening into the ejaculatory duct and ipsilateral upper moiety cystic renal dysplasia is reported to increase awareness among urologists and radiologists of this entity. Magnetic resonance imaging has been shown to be an excellent diagnostic tool for tracking of the trajectory of the ectopic ureter, thereby obviating the need for other invasive diagnostic techniques and permitting surgical correction of the anomaly. The embryology, clinical features, and diagnostic and therapeutic aspects of this ra…
