Navigating isolation- barriers and multidimensional support needs in symptom self-management for HSCT patients within laminar airflow rooms: a descriptive qualitative study.

BACKGROUND: Hematopoietic stem cell transplantation (HSCT) is a crucial therapy for hematological malignancies and other conditions. However, the intensive therapy and required isolation place significant physical and psychological burdens on patients. Although effective symptom self-management during isolation is pivotal, the current efforts are often challenged, leading to distress and feelings of loss of control. Nevertheless, research specifically addressing the self-management needs and barriers faced by patients with HSCT confined to laminar airflow rooms is limited. This study aimed to

Integrating Artificial Intelligence, Circulating Tumor DNA, and Real-World Evidence to Optimize Hematologic Clinical Trials: Toward Adaptive and Learning Trial Designs.

The integration of emerging technologies and real-world data is transforming the landscape of hematologic clinical trials. Artificial intelligence (AI) offers remarkable capabilities for predictive modeling, patient stratification, and adaptive trial design, while circulating tumor DNA (ctDNA) provides a minimally invasive biomarker for disease monitoring, the early detection of relapse, and treatment response assessment. Concurrently, real-world evidence (RWE) complements traditional clinical trial data by capturing treatment effectiveness, safety, and patient outcomes in broader, heterogeneo

A global, regional, and national survey on burden and Quality of Care Index (QCI) of hematologic malignancies; global burden of disease systematic analysis 1990-2017.

BackgroundHematologic malignancies (HMs) are a heterogeneous group of cancers that comprise diverse subgroups of neoplasms. So far, despite the major epidemiologic concerns about the quality of care, limited data are available for patients with HMs. Thus, we created a novel measure-Quality of Care Index (QCI)-to appraise the quality of care in different populations.MethodsThe Global Burden of Disease data from 1990 to 2017 applied in our study. We performed a principal component analysis on several secondary indices from the major primary indices, including incidence, prevalence, mortality, ye

Optimisation or Restriction? Unpacking the Logic Behind NICE Decisions.

BackgroundThe National Institute for Health and Care Excellence (NICE) regularly issues optimised technology appraisal (TA) recommendations, whereby medicines are recommended for a subset of their licensed population. These decisions play a growing role in shaping patient access, yet the underlying drivers and implications of optimisation remain poorly understood.MethodsWe conducted a mixed‑methods analysis of all optimised NICE TAs published in 2023 and 2024. Fifty‑six appraisals met our inclusion criteria for qualitative assessment of committee documentation, while 30 provided sufficient dat

Social relationship challenges perceived by cancer patients and survivors: a qualitative meta-synthesis.

BackgroundA cancer diagnosis can significantly disrupt patients' social lives, leading to strained relationships and increased social isolation. Many encounter uneasy interactions or feel judged by others, amplifying loneliness and a sense of exclusion. While existing research has predominantly focused on clinical outcomes, the social relationship challenges faced by cancer patients have not been fully explored. Consequently, this study aims to synthesize qualitative studies that examine the social relationship experiences of cancer patients and survivors, offering a deeper insight into this i

Barriers to Treg therapy in Europe: From production to regulation.

There has been an increased interest in cell based therapies for a range of medical conditions in the last decade. This explosion in novel therapeutics research has led to the development of legislation specifically focused on cell and gene based therapies. In Europe, the European medicines agency (EMA) designates any medicines for human use which are based on genes, tissues, or cells as advanced therapy medicinal products or advanced therapy medicinal products (ATMPs). In this article we discuss the hurdles to widespread adoption of ATMPs in Europe, with a focus on regulatory T cells (Tregs).

Systemic Lupus Erythematosus in Primary Care: An Update and Practical Messages for the General Practitioner.

Systemic Lupus Erythematosus (SLE) is a complex chronic autoimmune disease that manifests a wide range of organ involvement. Traditionally, the diagnosis and management of SLE is provided at secondary and tertiary centers to ensure prompt initiation of treatment, adequate control of flares and prevention of irreversible organ damage. Notwithstanding, the role of primary care in SLE is also emerging as there are still significant unmet needs such as the diagnostic delay at the community level and the high burden of therapy- and disease-related comorbidities. In the present review, we summarize

Trends in volumes and survival after hematopoietic cell transplantation in racial/ethnic minorities.

AbstractThere has been an increase in volume as well as an improvement in overall survival (OS) after hematopoietic cell transplantation (HCT) for hematologic disorders. It is unknown if these changes have affected racial/ethnic minorities equally. In this observational study from the Center for International Blood and Marrow Transplant Research of 79 904 autologous (auto) and 65 662 allogeneic (allo) HCTs, we examined the volume and rates of change of autoHCT and alloHCT over time and trends in OS in 4 racial/ethnic groups: non-Hispanic Whites (NHWs), non-Hispanic African Americans (NHAAs), a

Light Chain Stabilization: A Therapeutic Approach to Ameliorate AL Amyloidosis.

Non-native immunoglobulin light chain conformations, including aggregates, appear to cause light chain amyloidosis pathology. Despite significant progress in pharmacological eradication of the neoplastic plasma cells that secrete these light chains, in many patients impaired organ function remains. The impairment is apparently due to a subset of resistant plasma cells that continue to secrete misfolding-prone light chains. These light chains are susceptible to the proteolytic cleavage that may enable light chain aggregation. We propose that small molecules that preferentially bind to the nativ

The biology of stress in cancer: Applying the biobehavioral framework to adolescent and young adult oncology research.

The stress response influences the development and trajectory of cancer through a host of complex neuroimmune mechanisms. Basic, translational, and clinical research has elucidated these biobehavioral connections and offers a new paradigm for scientific investigation and patient care. Using a biobehavioral approach could offer new diagnostic and therapeutic opportunities in oncology, and this approach will be particularly impactful for adolescent and young adult (AYA) patients with cancer. To date, nearly all biobehavioral oncology research has been done in the adult population. And yet, AYAs
