Early prediction of femoral head avascular necrosis following neck fracture.

Femoral neck fracture puts at risk functional prognosis in young patients and can be life-threatening in the elderly. The present study reviews methods of femoral head vascularity assessment following neck fracture, to address the following issues: what is the risk of osteonecrosis? And what, in the light of this risk, is the best-adapted treatment to avoid iterative surgery? Femoral head vascularity depends on retinacular vessels and especially the lateral epiphyseal artery, which contributes from 70 to 80% of the femoral head vascular supply. Fracture causes vascular lesions, which are in tu

[Subcapital osteotomy of the femoral neck in the treatment of coxa vara in adolescence--long-term results].

The authors present a group of 16 adolescents suffering from coxa vara treated in the years 1967-1987 for a severe slipped capital femoral epiphysis by subcapital osteotomy. The authors deal with the etiology, symptomatology and treatment with special regard to the severe slip. Spica cast was applied in most operations and no internal fixation was used. In the so far latest female patient the femoral head was fixed after osteotomy by Kirschner wires according to Dyas Aries. In the discussion the authors report considerably lower percentage of complications in the longterm examination, mainly a

Finite element analysis of proximal femur bionic nail for treating intertrochanteric fractures in osteoporotic bone.

This study compared the biomechanical characteristics of proximal femur bionic nail (PFBN) and proximal femoral nail antirotation (PFNA) in treating osteoporotic femoral intertrochanteric fractures using finite element analysis. Under similar bone density, the PFBN outperforms the PFNA in maximum femoral displacement, internal fixation displacement, stress distribution in the femoral head and internal fixation components, and femoral neck varus angle. As the bone density decreases, the PFBN's biomechanical advantages over PFNA become more pronounced. This finding suggests that the PFBN is supe

Late sequelae of juvenile rheumatoid arthritis of the hip: a follow-up study into adulthood.

A 4-13 year follow up study of 29 patients with juvenile rheumatoid arthritis into adulthood revealed several structural deformities of the hips: coxa magna, short femoral neck, subluxations associated with large cyst-like erosions of femur and acetabulum near the ligamentum teres and a flattened, wide femoral head (without steroid treatment). The deformities evoke the possibility of ischemia of the femoral head in the presence of active, compressive synovial proliferation during the growth period in a joint with largely intra-articular nutrient vessels.

Association between fracture location and one-year mortality in patients aged 65 years and older after lower extremity trauma: a retrospective cohort study.

PURPOSE: Fractures in older patients are associated with increased mortality. Our study evaluated 30-day and one-year mortality among older patients based on fracture location. METHODS: A descriptive retrospective cohort analysis was conducted including all patients aged 65 years or older hospitalized at a level I trauma center with a pelvic or lower extremity fracture between January 1, 2016 and December 31, 2019. Mortality rates were compared with age- and gender-matched data from the Swiss general population. In addition, an exploratory multivariable logistic regression analysis was perform

Administration of Prophylactic Enoxaparin on the Morning of Surgery Does Not Increase Risk of Blood Transfusion or Wound Drainage Following Internal Fixation of Geriatric Femur Fractures.

BACKGROUND: Despite standard use of chemoprophylaxis, 30-day incidence of venous thromboembolism after geriatric, those older than 60 years, femur fracture surgery is reported to be up to 10%. Missing one dose of enoxaparin has been proven to increase the risk of developing venous thromboembolism. It is commonplace to hold preoperative chemoprophylaxis the morning of surgery because of concern for intraoperative bleeding or wound drainage. We sought to determine whether administration of prophylactic enoxaparin the morning of surgery resulted in an increased rate of blood transfusion or wound

Femoral osteochondroplasty can be performed effectively without the risk of avascular necrosis or femoral neck fractures in an experimental ovine FAI model.

OBJECTIVE: The experimental induction of cam-type femoroacetabular impingement (FAI) in sheep is established. To tap the full potential of this ovine model, one should be able to perform a femoral osteochondroplasty safely. This study was based on previous cadaver experiments on the blood supply to the ovine femoral head and on the biomechanical strength of the proximal femur following offset creation. We hypothesized that offset creation in this ovine FAI model does not lead to (1) avascular necrosis (AVN) of the ovine femoral head or (2) iatrogenic femoral neck fractures and (3) can be perfo

Proximal Femoral Deformity Following Threaded Prophylactic Fixation for Slipped Capital Femoral Epiphysis: Risk Stratification Using the Modified Oxford Score.

BACKGROUND: This study assesses the effect of skeletal maturity on the development of iatrogenic proximal femoral deformity following threaded prophylactic screw fixation in patients presenting with unilateral slipped capital femoral epiphysis (SCFE). METHODS: Children who underwent threaded screw prophylaxis of the uninvolved hip (Group P) and those who were observed with no prophylaxis (Group N) on presentation with unilateral SCFE were compared. Skeletal maturity was assessed with the Modified Oxford Score (MOS). Proximal femoral morphology was characterized by femoral neck length, femoral

Coxa plana--the fate of the physis.

Eighty patients who had unilateral coxa plana and who had been treated conservatively were followed to a mean age of sixteen and one-quarter years, and their cases were reviewed for radiographic evidence of a disturbance of the femoral capital growth plate. This physeal involvement was inferred by the presence of one or more of five findings: premature physeal closure, overgrowth of the greater trochanter, change in physeal shape, lateral protrusion of the capital nucleus, and medial bowing of the femoral neck. Premature physeal closure, which was more common in the girls than in the boys, occ

Treatment of nonunion of femoral neck fracture by valgus osteotomy in 33 cases.

In spite of advances in treatment, femoral neck fractures are complicated fractures with a relatively high incidence of nonunion. Between 1990 and 2004, 33 patients with nonunion of femoral neck fractures were treated by valgus osteotomy in our department. The mean age of patients at the time of operation was 38 years (range 16-60 years). Reasons for nonunion were implant failure in 21 and osteomalacia in two patients. The remaining ten patients were treated nonoperatively. Average neck-shaft angle was 109° (78°-125°), and the average shortening of the involved limb was 2.5 cm (0.5-4.5). After

Recurrent femoral neck fractures: keep an eye out for Cushing's syndrome.

Prolonged glucocorticoid overexposure leads to Cushing's syndrome (CS). It is characterised by diverse metabolic, cardiovascular and bone mineral manifestations. A middle-aged woman presented for a preoperative evaluation of a pathological fracture of the left neck of the femur. She had a history of a hip fracture on the opposite side 3 years back. She exhibited several features specific to CS and was subsequently diagnosed with Cushing's disease. She underwent transsphenoidal adenoma resection and achieved remission. Glucocorticoids primarily affect the trabecular bone compared with cortical

Intracapsular fracture of the proximal femur in a bilateral above-knee amputee: A case report with technical tips for intraoperative positioning and literature review.

Intracapsular fractures of the proximal femur are one of the most common fractures of the lower limbs. Most cases require osteosynthesis with suitable implants, and intraoperative positioning of the patient on the fracture table is a prerequisite to facilitate fracture manipulation, traction, reduction and fluoroscopy assessment. However, positioning the limbs of bilateral above-knee amputees for internal fixation of related proximal femoral fractures is a difficult task, which requires customized inventory for effective limb positioning and fracture manipulation. This study reported a rare ca

Combination of lumbar erector spinae plane block and transmuscular quadratus lumborum block for surgical anaesthesia in hemiarthroplasty for femoral neck fracture.

Femur neck fractures may occur in elderly patients with multiple co-morbidities. Spinal or general anaesthesia may not be safe in such patients, leading to a search for other safer alternatives. Herein, we report a case in which a never previously reported combination of quadratus lumborum block (QLB) and erector spinae plane block (ESPB) was successfully used as the main anaesthetic method for hemiarthroplasty. An 86-year-old female patient with severe aortic stenosis was scheduled for internal fixation or hemiarthroplasty due to right femoral neck fracture. Following sedoanalgesia, the patie…

The Effect of Proton Pump Inhibitors on Bone Mineral Density at Specific Anatomical Sites: A Systematic Review and Meta-Analysis.

There have been some concerns about the potential adverse consequences of proton pump inhibitors (PPIs) on bone health, specifically with respect to bone mineral density (BMD). The objective of the present investigation was to evaluate the impact of PPIs on BMD in the lumbar spine, femoral neck, and total hip regions. On the basis of PRISMA, a systematic review and meta-analysis were performed. PubMed, Scopus, Cochrane Library, and Google Scholar were used to find eligible observational studies published between January 2010 and January 2025. The ROBINS-I tool was employed to evaluate potentia

Characteristics of Femoral Shaft Fractures That Predict Ipsilateral Femoral Neck Fractures.

INTRODUCTION: This study aims to characterize radiographic features and fracture characteristics in femoral shaft fractures with associated femoral neck fractures, with the goal of establishing predictive indicators for the presence of ipsilateral femoral neck fractures (IFNFs). METHODS: A retrospective cohort was collected from the electronic medical record of three level I trauma centers over a 5-year period (2017 to 2022) by current procedural terminology (CPT) codes. Current CPT codes for combined femoral shaft and IFNFs were identified to generate our study group. CPT codes for isolated

Exploration of the synergistic role of cortical thickness asymmetry ("Trabecular Eccentricity" concept) in reducing fracture risk in the human femoral neck and a control bone (Artiodactyl Calcaneus).

The mechanobiology of the human femoral neck is a focus of research for many reasons including studies that aim to curb age-related bone loss that contributes to a near-exponential rate of hip fractures. Many believe that the femoral neck is often loaded in rather simple bending, which causes net tension stress in the upper (superior) femoral neck and net compression stress in its inferior aspect ("T/C paradigm"). This T/C loading regime lacks in vivo proof. The "C/C paradigm" is a plausible alternative simplified load history that is characterized by a gradient of net compression across the e
