An enabler or a barrier: implications of local government autonomy for effective primary health care (PHC) reforms in Nigeria.

INTRODUCTION: in July 2024, Nigeria´s Supreme Court granted autonomy, including financial, to local government, a decision with major implications for primary health care (PHC). Although local governments are constitutionally responsible for PHC, weak fiscal independence and administrative capacity have hindered performance. This study examines opportunities and challenges that LGA autonomy poses for PHC reforms, particularly within the Primary Health Care Under One Roof (PHCUOR) policy. METHODS: a qualitative exploratory policy analysis was conducted using key informant interviews (n=288) an

The interplay of poverty risk-employment trajectories in couples around the transition to parenthood in Germany.

The transition to parenthood is a critical period that exacerbates gendered economic inequality, with mothers more likely than their partners to experience employment disruptions and income losses. This study examines individual poverty risk among partnered individuals (N = 1237) in Germany from a life course perspective, analyzing how gendered career patterns around first births between 1992 and 2013 intersect with changes in individual poverty risk, i.e. under the assumption of no income pooling. Applying multichannel sequence analysis (MCSA) to data from the Socio-Economic Panel, the findin

Service utilisation changes in the transition to permanent supportive housing: The role of the housing environment and case management.

This study examines whether routine, low-cost service use changes in the transition from homelessness to permanent supportive housing (PSH) and explores whether housing model, neighbourhood and level of case management engagement affect utilisation of routine services. Data come from a prospective longitudinal study of adults experiencing homelessness who entered PSH in Los Angeles between 2014 and 2016 and participated in four interviews: pre-housing (i.e., while experiencing homelessness), and 3, 6 and 12 months after move-in. Mixed effects logistic regression assessed the effects of demogra

Socioeconomic challenges during the COVID-19 pandemic for Veterans with psychosis or recent homelessness.

The COVID-19 pandemic and social distancing have directly impacted the socioeconomic well-being of most Americans. Veterans with psychosis (PSY) and Veterans who were recently housed (RHV) through a supportive housing programme may be especially vulnerable to experiencing negative socioeconomic effects of the pandemic. In this study, we investigated socioeconomic experiences and challenges during the pandemic in these two putatively vulnerable Veteran groups and in Veterans with no history of PSY or homeless (i.e., control Veterans, CTL). A total of 231 Veterans (81 PSY, 76 RHV, 64 CTL) partic

Focus group study of public opinion about paying living kidney donors in Australia.

BACKGROUND AND OBJECTIVES: The unmet demand for kidney transplantation has generated intense controversy about introducing incentives for living kidney donors to increase donation rates. Such debates may affect public perception and acceptance of living kidney donation. This study aims to describe the range and depth of public opinion on financial reimbursement, compensation, and incentives for living kidney donors. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Twelve focus groups were conducted with 113 participants recruited from the general public in three Australian states in February 20

Family Physicians' Perspectives on Providing Living Kidney Donor Care.

RATIONALE & OBJECTIVE: Living kidney donors (LKDs) perceive gaps in care along their donation journeys, such as poor coordination and commun … providers, poor health record transmission, limited availability of mental health services, limited financial support for patients and providers). The participants had 3 key recommendations: (1) support PCPs with pragmatic approaches and on-demand resources; (2) empower LKDs to be active participants in their care; and (3) implement strategies to address LKD care p …

A technique for implantation of the CentriMag left ventricular assist device to allow ambulation and rehabilitation in patients with heart failure.

BACKGROUND: The therapeutic options for heart failure include inotropic agents, intraaortic balloon pumps, and left ventricular assist devices (LVAD). Implantable LVADs are not appropriate for all patients. The short-term devices require patients to stay in bed, connected to cannulas, which are usually inserted using a median sternotomy. This approach requires a subsequent sternotomy, midline cannulas (which can make sitting difficult), and immobility. We began using a right thoracotomy with cannulas placed through intercostal spaces for selected patients in need of temporary LVAD support. ME

Financial determinants of effective hypertension and diabetes care in rural primary health facilities in Kisumu, Kenya: a mixed-methods study.

BACKGROUND: Noncommunicable diseases (NCDs), including hypertension and diabetes, account for approximately 27% of all deaths in Kenya, with 26% of adults having elevated blood pressure. Despite devolution of health services to county governments in 2013, financing for NCD management at the primary health care (PHC) level remains weak. This study examines financial determinants shaping hypertension and diabetes care in PHC facilities within a devolved county health system in rural Kisumu County, Kenya. METHODS: We conducted a convergent parallel mixed-methods cross-sectional study in seven pub

Management of complications following aesthetic procedures can lead to significant additional cost.

Liposuction is one of the most common surgical interventions in aesthetic surgery, technically easy to perform and is also carried out by many non-plastic surgeons. However, this operation can have severe complications, and dealing with them can impose a great financial burden on the tax payer-supported national health-care systems. We report here about a patient, who was hospitalised for complications after a failed office-based liposuction. The ensuing costs of treatment are also discussed.

"More work means less pay": a qualitative understanding of physicians' patient selection decision-making under DRG reform in Nanjing, China.

BACKGROUND: Patient selection, also known as 'cream-skimming', can lead to the systematic exclusion of high-cost, clinically complex patients from necessary care. It has been identified as an opportunistic response under Diagnosis-Related Groups (DRGs) payment systems globally. As China advances the rollout of DRG reform alongside commitments to promoting healthcare equity, understanding the ecology of physicians' cream-skimming behaviour and how to mitigate it is imperative. METHODS: We purposively sampled 21 physicians from two hospitals with different incentive structures in 2024 and condu

Development and Use of Gene Therapy Orphan Drugs-Ethical Needs for a Broader Cooperation Between the Pharmaceutical Industry and Society.

Gene therapy orphan medicinal products constitute a unique group of new drugs which in case of hereditary diseases are usually administered only once at an early age, in the hope to provide sufficient gene product to last for the entire life of the patients. The combination of an exceptionally large single payment and the life-long clinical follow-up needed for understanding the long-term benefits and safety of gene therapy, represent new types of scientific, financial, social and ethical challenges for the pharmaceutical industry, regulators and society. With special consideration of the uniq

Acceptability of financial incentives for health behaviour change to public health policymakers: a qualitative study.

BACKGROUND: Providing financial incentives contingent on healthy behaviours is one way to encourage healthy behaviours. However, there remains substantial concerns with the acceptability of health promoting financial incentives (HPFI). Previous research has studied acceptability of HPFI to the public, recipients and practitioners. We are not aware of any previous work that has focused particularly on the views of public health policymakers. Our aim was to explore the views of public health policymakers on whether or not HPFI are acceptable; and what, if anything, could be done to maximise acce

Overlooked and Undernourished: A Case Report of Scurvy Linked to Food Insecurity.

UNLABELLED: Food insecurity is a commonly underrecognized social determinant of health and is a risk factor for severe, though uncommon, nutritional deficiencies. This report describes a 60-year-old male with a history of asthma and housing instability who presented to the emergency department (ED) for shortness of breath and was found to have classic findings of vitamin C deficiency, colloquially known as scurvy. Examination revealed diffuse wheezing, poor dentition with palatal ecchymosis, and a perifollicular rash with scattered bruising on his bilateral lower extremities. Due to financial

A qualitative exploration of sonographers' views toward transvaginal ultrasound in Zimbabwe: A barriers and solutions approach.

INTRODUCTION: Given the increasing reliance on pelvic imaging and the superior diagnostic quality of transvaginal ultrasound (TVUS), the British Medical Ultrasound Society emphasizes that all eligible patients should be offered this procedure to ensure the highest standard of care. However, in Zimbabwe, anecdotal evidence indicates a reluctance among sonographers to offer it to patients. This study aimed to understand sonographers' views on the barriers to adopting TVUS and to identify potential solutions. METHODS: This study employed a qualitative exploratory design using semi-structured int

Bayesian Reconciliation of Return Predictability.

This article considers a stable vector autoregressive (VAR) model and investigates return predictability in a Bayesian context. The bivariate VAR system comprises asset returns and a further prediction variable, such as the dividend-price ratio, and allows pinning down the question of return predictability to the value of one particular model parameter. We develop a new shrinkage type prior for this parameter and compare our Bayesian approach to ordinary least squares estimation and to the reduced-bias estimator proposed in Amihud and Hurvich (2004. "Predictive Regressions: A Reduced-Bias Esti

Comprehensive portrait of the needs of caregivers of individuals living with a traumatic brain injury: a systematic mixed-methods review.

BACKGROUND: Caring for an individual living with traumatic brain injury may place physical and emotional demands on caregivers, yet evidence on their specific needs remains fragmented. Synthesising this evidence is therefore essential to address this gap and to inform the development of targeted support strategies. The aim of this study was to appraise and synthesise the evidence the needs of caregivers of individuals living with moderate-to-severe traumatic brain injury. METHODS: A mixed-methods systematic review was conducted in 2024 across MEDLINE, PsycINFO, CINAHL and Web of Science (PROS

Depression and Functioning during the COVID-19 Pandemic among Adults across Tunisia.

We aimed to understand how risk (trauma history, health problems, financial problems, family problems) and protective (friend support, family support) factors influenced daily functioning (e.g., self-care, mobility, social participation) among Tunisian adults during the COVID-19 pandemic, directly and through their impact on depression, with a focus on gender differences. We recruited a representative sample of 2014 participants (1024 males, 990 females) using random digit dialing of mobile phone numbers across all 24 governorates in Tunisia. Females reported higher depression, greater impaire
