Dorso-ventral osteophytes of interphalangeal joints correlate with cartilage damage and synovial inflammation in hand osteoarthritis: a histological/radiographical study.

OBJECTIVE: To detect dorsally located osteophytes (OP) on lateral x-ray views and to correlate their presence with the extent of structural joint damage, determined by histologic grading (cartilage damage and synovial inflammation) and radiographic scoring in hand osteoarthritis (HOA). METHODS: Distal interphalangeal (DIP) and proximal interphalangeal (PIP) joints were obtained from post mortem specimens (n = 40). Multiplanar plain x-rays were taken (dorso/palmar (dp) and lateral views). Radiographic OA was determined by the Kellgren and Lawrence classification. Joint samples were prepared fo

Anti-Jo-1 antibody-positive arthropathy presenting as severe osteolysis of the distal interphalangeal joints.

Antisocial personality disorder (ASPD) is a relatively common problem, but the neuropsychological profile of affected individuals has seldom been studied outside of criminal justice recruitment settings. Non-treatment-seeking young adults (18-29 years) were recruited from the general community by media advertisements. Participants with ASPD (n = 17), free from substance use disorders, were compared with matched controls (n = 229) using objective computerized neuropsychological tasks tapping a range of cognitive domains. Compared with controls, individuals with ASPD showed significantly elevate

[Bursitis intermetacarpophalangea calcarea (case report)].

A case of bursitis intermetacarpophalangea calcarea is presented which, after clinical and X-ray examination, raised problems regarding differential diagnosis. Because of the rarity of the findings, the anatomical localization as well as the surgical procedure is presented. Calcifications which may show in tendons and ligaments are discussed. In cases of acute calcareous infiltration in hands, surgery very rarely is indicated. By contrast, in cases of bursitis intermetacarpophalangea calcarea, surgery should be performed as soon as possible to preserve normal hand function.

Atypical erosive osteoarthritis and Sjögren's syndrome.

An unusual, destructive form of erosive hand arthropathy is described in five postmenopausal women. Although early clinical and radiological findings were most in keeping with a diagnosis of erosive osteoarthritis (EOA), hand involvement progressed to a distribution intermediate between EOA and rheumatoid arthritis (RA). Although asymptomatic, all patients had features of Sjögren's syndrome (SS), including keratoconjunctivitis sicca (KCS) and lymphocytic foci in labial biopsy specimens. These cases, which are clinically unlike either RA or EOA, may represent a unique arthropathy associated wit

Observer variation in quantitative assessment of rheumatoid arthritis. Part I. Scoring erosions and joint space narrowing.

Six observers, including two bone and joint radiologists, two general radiologists, and two senior radiology residents, compared scores to quantitate radiographic findings in the hands and wrists of patients with rheumatoid arthritis. In the scoring system used, erosions and joint-space narrowing are graded separately. This scoring system differs from other methods in that equivocal findings are not scored, while ankylosis, subluxation, and dislocation are scored, and data from postoperative joints are included. Total radiographic scores were highly significantly correlated for all observers (

[Volar flexion of the fingers. Its value in diagnosis of leprous paralysis of the intrinsic muscles of the fingers and in the study of the results of palliative interventions].

Active metacarpophalangeal flexion with simultaneous active extension of interphalangeal joints places the fingers in the position in preparation for prehension. This volar flexion of the fingers is due to the action of their intrinsic muscles, and this movement, which is irreplaceable, has been used by the author for the last twenty years as a specific exploratory test of intrinsic finger muscles. It can be investigated as an emergency procedure in cases of ulnar nerve wounds at the elbow or injury to the upper forearm before suture, and clearly demonstrates the presence of an ulnar claw hand

Neuropathy, myopathy and destructive arthropathy in primary hypothyroidism.

A 75-year-old woman with untreated primary hypothyroidism was found with peripheral neuropathy (including carpal tunnel syndrome), severe myopathy (high levels of creatine phosphokinase) and destructive arthropathy, affecting fingers, toes and the left knee. Radiographs of her knee showed destructive lesions of the tibial plateau similar to a pathologic compression fracture, while the joints of the fingers and toes showed all signs of severe erosive osteoarthritis (OA), radiographically documented over a period of 7 years. It is suggested that hypothyroidism is causally related to the developm

Pustulotic arthro-osteitis with distal phalanx osteitis.

A 68-year-old Japanese woman was diagnosed with palmoplantar pustulosis. She presented with anterior chest pain and swelling, as well as pain in the right first finger. Initially, antibiotics was ineffective, the patient was diagnosed with non-bacterial osteomyelitis. Computed tomography of the chest revealed a sternal bone thickening pattern, and technetium-99m bone scintigraphy showed accumulations in the left sternoclavicular joint and sternal angle. Additionally, contrast-enhanced magnetic resonance imaging revealed osteitis in the sternal scapes and body. Based on these findings, the pati

Brachialis muscle transfer to the forearm for the treatment of deformities in spastic cerebral palsy.

The use of the brachialis muscle for tendon transfers in cerebral palsy has not been described previously. In this study, the brachialis muscle was used for transfer in 11 patients with spastic cerebral palsy for the restoration of forearm supination, wrist extension, or finger extension. Four patients underwent brachialis rerouting supinatorplasty. Active supination increased in two (60° and 50°), minimally increased in one (5°), and did not change in one patient. Five patients had a brachialis to extensor carpi radialis brevis transfer. The mean gain in postoperative active wrist extension w

Proximal Interphalangeal Joint Arthroplasty for Fracture.

Proximal interphalangeal joint arthroplasties can be performed in the setting of acute comminuted fracture, chronic fracture presentations, and posttraumatic arthritis. These surgeries provide excellent pain relief and patient satisfaction but patients should be cautioned not to expect an improvement in motion postoperatively. Despite high rates of minor complications and radiographic loosening, these implants have good rates of long-term survival with most revisions occurring in the early postoperative period. They provide viable alternatives to arthrodesis, osteotomy and amputation in the ap

Proximal interphalangeal joint arthroplasty with the Tactys® prosthesis: Clinical and radiological outcomes at a mean 3.1 years' follow-up.

Preliminary results with the Tactys® modular gliding prosthesis for proximal interphalangeal joint (PIPJ) replacement were encouraging, with notable improvement in clinical and functional scores at 1 and 2 years' follow-up. However, a recent study found a trend for deterioration over time. We wanted to check this by analyzing medium-term outcomes. Sixty-four arthroplasties were performed in 48 patients in single facility between January 2015 and January 2020. Clinical, functional, and radiographic outcomes were analyzed at short- and medium-term follow-up for 15 of these arthroplasties. Mean f

National Prevalence of Complications and Cost of Small Joint Arthroplasty for Hand Osteoarthritis and Post-Traumatic Arthritis.

PURPOSE: Osteoarthritis (OA) of the hand is commonly treated using implant arthroplasty. Despite the increasing prevalence of hand OA, population-based evidence regarding the complication profile and associated cost for patients undergoing proximal interphalangeal (PIP) joint and metacarpophalangeal (MCP) joint arthroplasty are lacking. Therefore, we aimed to evaluate the complication profiles and variation in cost of care for patients undergoing PIP and MCP joint arthroplasty. METHODS: We analyzed insurance claims from 2009 to 2016 using the Truven MarketScan Databases for adult patients und

Cerclage Wire Fixation for Fracture-Dislocations of the Proximal Interphalangeal Joint.

Proximal interphalangeal (PIP) joint fracture dislocations are challenging injuries to treat. Multiple and varied treatments have been proposed. We present the use of cerclage wiring as a helpful technique in these challenging scenarios. The technique has the benefit of securing fracture fragments from the volar or dorsal base of middle phalanges or a comminuted fracture involving the entire articular surface. We report on the use of cerclage wires in eight patients (average: 43 years of age). Three patients had volar base fractures, three dorsal base fractures, and two impacted fractures invo

Brachial plexus block with the nerve stimulator: motor response characteristics at three sites.

BACKGROUND AND OBJECTIVES: Differences in motor response patterns, minimum electrical currents, and success rates using a nerve stimulator for brachial plexus block were determined for the interscalene, supraclavicular, and axillary approaches. METHODS: Supervised, resident physicians performed 106 brachial plexus blocks. For each block, the minimum stimulating current, the strongest joint motion at the minimum current immediately before local anesthetic injection, presence or absence of other joint motion, and anesthetic success or failure were recorded. RESULTS: In the interscalene group (

Complications of open trigger finger release.

PURPOSE: Open release of A1 pulleys for trigger finger has been thought of as a relatively benign procedure with a low complication rate. Few studies have examined the rate of complications in trigger finger release. The objective of this study was to retrospectively review the complications documented for a cohort of patients who received open trigger finger releases. METHODS: We conducted a retrospective chart review of 43 patients who had had 78 open trigger finger releases by a single surgeon. Any postoperative complications that were documented were recorded. Complications were then divi

Traumatized index finger pollicization for thumb reconstruction.

PURPOSE: When the index finger is injured or severed in conjunction with a traumatic amputation of the thumb, transfer of the injured index finger can restore the important function of the thumb. The purpose of this study is to evaluate the results of the transfer of an injured index finger for traumatic loss of the thumb. METHODS: Seven patients treated by pedicled transfer of a traumatized index finger after amputation to the ipsilateral thumb were reviewed retrospectively. Postoperative evaluations included thumb range of motion, opposition and pinch function, grasp and pinch strength, sen

Spontaneous flexor digitorum profundus tendon rupture of the little finger in patients with rheumatoid arthritis: A report of two cases with different mechanisms.

Subcutaneous flexor tendon rupture in patients with rheumatoid arthritis is rare compared with extensor tendon rupture, with a reported incidence of approximately one-tenth of that of extensor tendons. The pathophysiology of flexor tendon rupture is not fully understood. We report two cases of spontaneous flexor digitorum profundus tendon rupture of the little finger in patients with rheumatoid arthritis despite low disease activity and the absence of advanced joint destruction. Both patients presented with an inability to actively flex their little fingers without any history of significant t

The Top-Hat Flap: A New Technique for Segmental Nailbed Reconstruction.

SUMMARY: The finger or thumb tip was used as a homodigital neurovascular island flap ("top-hat flap") to reconstruct 28 segmental defects of the distal phalanx, nailbed, or pulp over a 7-year period with a minimum follow-up of 12 months. All flaps survived and all fractures united. A total of 75% of patients achieved full range of motion at the distal interphalangeal joint, 71% achieved 2-point discrimination of less than 6 mm, and 92% returned unrestricted to their preinjury job less than 3 months after injury. The average digit impairment score, based on the American Medical Association's Gu
