Foot drop as a harbinger of aortic occlusion.

A man presented with foot drop and 2 months later an infrarenal occlusion of his abdominal aorta was discovered. Initial evaluation found no structural etiology for his nerve injury. His neurologic deficits progressed until gangrenous changes developed in his feet leading to the discovery of aortic occlusion. This case demonstrates that peripheral nerve injury in the legs may signal underlying large vessel occlusive disease. We discuss the localization and potential mechanisms of ischemic nerve injury.

The role of foot surgery in patients with diabetes.

In conclusion, surgery on the diabetic foot should be looked upon as a required addition to quality conservative care. The success that can be gained from proper performance of these procedures can be rewarding and often improves the ability to salvage limbs that may have otherwise been lost because of ulceration, infection, instability, or pain.

Morbidity after crush injuries to the foot.

By retrospective review of hospital records and by follow-up clinical examinations, we evaluated 58 patients with crush injuries to the foot treated at our institution between 1986 and 1990. All patients had received initial treatment according to a standardized protocol determined by the type and magnitude of the injury. Patients were examined at a mean interval of 3.3 years (range 2-4) after injury, and the functional outcome was determined according to a foot trauma rating scale. Based on this scoring system, 46% of the patients had good functional outcome, 29% had fair results, and 25% had…

A skin suture for foot and ankle surgery.

Severe swelling and skin necrosis are not uncommon following foot and ankle surgery, particularly trauma surgeries. A skin suture technique is presented that has the advantages of being simple, providing good skin eversion and apposition with minimal tension and risk of ischemia, and being easy to remove. The suture technique has been used by the authors for the past 2 years on all foot and ankle trauma and reconstructive cases in which severe swelling was anticipated. There have been no instances of skin necrosis, significant epidermolysis, or dehiscence, and the appearance of the scar is acc…

Reconstruction of the foot with microvascular free flaps.

Reconstruction of the foot using microvascular flaps has been widely performed in the last 15 years but the choice of flap to repair some areas in the foot remains controversial. We present a series of 128 free flaps to the foot performed during the period of 1975-1990. One hundred and seventeen had a successful outcome (91%). The etiology of the problem was traumatic in 94, congenital in 10, tumor in 9, and chronic ulcerations due to vascular problems in 15. The indications for a specific flap depended on the site and extension of the foot problem, and were divided into four groups: 1. Dorsum…

An adult with juvenile hyaline fibromatosis of the foot.

An unusual consideration in the differential diagnosis of benign, solid tumors of the foot is juvenile hyaline fibromatosis, which only superficially resembles the better known myofibromatosis. The onset of the typical cutaneous lesions of this affliction, more frequently found in the head, trunk, or upper extremity, usually begins in childhood, with a progressive increase in their size and number. No cure is known, but early surgical excision of smaller lesions will minimize bony erosion and skin ulceration that otherwise risks toe amputation, thereby preserving function and a reasonable esth…

[Fractures of the foot (excluding the talus and calcaneus)].

The submitted retrospective investigation analyzes a group of 108 casualties with injuries of the skeleton of the foot who were hospitalized in the years 1985-90. Excluded was the group of patients with fractures of the talus and calcaneus because they consider problems of these fractures specific. The structure of the group is influenced by the specialty of the authors' workplace--i.e. treatment of multiple injuries. The characteristic of the group, the mechanism of the injury, data concerning time, a summary of the diagnoses, primary treatment and complications are summarized in tables. Twen…

[Calcitonin in treatment of diabetic foot syndrome].

A case of a 36-year patient with persistent diabetes mellitus of type I and gangrene of the foot is presented. Calcitonin has been used to treat osteitis and inflammation of the adjacent soft tissues. Despite performed surgery and conservative treatment, destruction of the bones has been progressing, and could lead to the high amputation of the extremity. Calcitonin produced an increase in bone, massive calcification around bone stumps, and anastomosis within one year. An acceleration of the wound healing has also been noted. Taking calcitonin mechanism of action and the clinical course of thi…

Foot and ankle problems in rheumatoid arthritis.

The purpose of this study was to examine the prevalence of foot and ankle problems in 99 patients with clinically proven rheumatoid arthritis. Patients were recruited from outpatient rheumatology clinics; no attempt was made to select patients on the basis of the severity of their disease, duration of disease, or symptom constellation. Each patient was examined by an investigator utilizing a predesigned protocol to assess their functional status, functional capacity, and overall joint involvement. Ninety-three of 99 patients had complaints referable to the foot or ankle at some time since diag…

Puncture wound pseudomonal osteomyelitis of the foot.

Puncture wounds of the foot are commonly seen in the hospital emergency room. They are usually managed appropriately; however, a percentage go on to skin infection with subsequent joint and osseous involvement. This is particularly true in the adolescent patient. This manuscript was written to inform the foot specialist on the etiology, medical, and surgical management of osteomyelitis secondary to Pseudomonas aeruginosa. Also, two pediatric case histories that occurred within 6 months of each other are presented.

Diagnosis and treatment of pediatric foot deformities.

In children, the lower extremities have extremely wide variations of normal, including calcaneovalgus foot, metatarsus adductus and flatfoot. The first two conditions often resolve or improve spontaneously over time and may be considered abnormal if they persist into adulthood. Serial casting, splints and surgery are rarely necessary. Corrective shoes, orthotic inserts and some forms of splinting have not proved to be effective. The primary care physician must establish an early and correct diagnosis, perform a general screening examination to rule out accompanying musculoskeletal deformities,…

Soft-tissue reconstruction of the foot and ankle.

Successful reconstruction of the severely injured foot and ankle remains a challenge for the surgeon. Frequently, bone and joint injuries to this area with overlying soft-tissue injuries or losses require the coordinate management by orthopedic and reconstructive specialists. Healthy, well-functioning feet are necessary to perform daily activities. Because of the dense packaging of the specialized interdependent structures of the feet, injuries to this area commonly cause significant (composite) wounds that, if not properly managed, can lead to progressive deformity and disability. Basic princ…

Magnetic resonance imaging of the foot and ankle.

Magnetic resonance imaging (MRI) of the foot and ankle is useful for the diagnosis and staging as well as the surgical planning of multiple disorders: first, for osseous injury, particularly stress fractures and occult fractures, which appear like bone bruises but behave more like fractures. Then, for the Achilles tendon, MR is useful for staging the chronicity and extent of damage, as well as whether the tendon can be surgically repaired or reconstructed. For the diagnosis of posterior tibialis tendon tears and injuries, MR is an important tool in surgical planning. Often, MR is helpful for t…

Varus foot in cerebral palsy: an overview.

In cerebral palsy, imbalance of the invertor and evertor muscles can result in varus alignment of the foot. The primary functional problems of varus foot are weightbearing instability and/or difficulty in foot clearance in swing phase. Additional problems include difficulties in shoe fitting, abnormal shoe wear, and unacceptable cosmesis. Gait studies have shed some light on the alterations of the muscle activity that can cause this problem but have fallen short of providing clear guidelines for treatment of this common problem. Confusion exists because dynamic electromyography does not give d…

Magnetic resonance imaging of the foot and ankle.

Technical considerations for the magnetic resonance imaging of the foot and ankle are discussed, including the selection of the appropriate surface coil, the importance of stabilizing the anatomic region, and the principles guiding the choice of pulse sequences. The anatomy of the foot and ankle are reviewed, and the normal appearance and important variations of the different structures on magnetic resonance images are discussed. Pathology of the foot and ankle are then discussed. Topics covered include osteochondral and radiographically occult fractures, tears and inflammation of the Achilles…

[Vascular complications in reconstructive surgery of the foot].

Even when we assess accurately the preoperative vascular damage of the blood vessel we intend to use for reconstruction, difficulties may arise during operation which cannot be resolved by routine methods. Microvascular surgery is an important part of many modern reconstruction procedures which can be used during reconstruction of the extremity. As an example the authors describe injuries of the left foot involving loos in a 41-year-old patient who was knocked down by a taxi. The defect denuded the bone and loss of soft tissues of the heel called for free transfer of a vascularized flap. Durin…

Foot orthosis for the severe flatfoot in sports.

The control of excessively pronating feet in athletes can be a prolonged and frustrating experience for both the doctor and the patient. The inverted orthotic technique has previously been described for making a device that has two basic applications: to control excessive subtalar joint pronation, especially where a Root or modified Root device cannot, and to invert the entire lower limb for such problems as medial knee pain. The authors present two case histories that highlight the indications of this orthotic device for the athletic population.
