Rigid internal fixation has long been the standard for fracture management; however, excessive construct stiffness can suppress interfragmentary strain, reduce callus formation, and impair secondary fracture healing. Low-elastic-modulus TiNbSn alloys have emerged as a promising alternative, offering mechanical behavior closer to that of cortical bone. This review synthesizes representative preclinical and computational evidence to clarify the mechanobiological rationale for TiNbSn alloy plates in fracture fixation. We summarize key biological requirements for secondary fracture healing, includ

Because the hand tolerates disuse so poorly, the beginning of early motion following fractures is important in achieving early restoration of function. This report concerns the use of a unique, expandable intramedullary device for internal fixation of metacarpal or phalangeal shaft fractures that renders enough intrinsic stability that motion may be started early without fear of loss of reduction. The device is applicable for transverse or short oblique fractures and consists of a cylindrical apparatus made of titanium that allows for collapse in the circumferential diameter. It is introduced

BACKGROUND: Breast cancer is the most common cancer and second cause of death in women worldwide. Patients with breast cancer are classified into subgroups based on the presence or absence of hormone receptors and the human epidermal growth factor 2-neu (HER-2) marker, the different molecular profiles come with an associated prognosis and variety of possible treatment options. Patients with triple negative cancer have a worse prognosis, a more aggressive behavior, higher likelihood of spreading, a higher risk of recurrence and a poorer outcome overall. Intramedullary rod fixation has proven to

UNLABELLED: Tranexamic acid (TXA) is a cheap and powerful drug that has several uses in surgery and is well established in elective orthopaedic surgery. At present, limited small studies have looked into its role in the acute hip fracture. Transfusion in the geriatric population presents risks and increased costs to healthcare systems around the world. Our retrospective study looks at the role of TXA administration at induction for both intracapsular fracture requiring hemiarthroplasty and our preferred method of fixation of extracapsular fracture by intramedullary nail (IM nail). We show a st

BACKGROUND: Reamed intramedullary nailing is the current gold standard for the treatment of diaphyseal fractures of the femur and tibia. Current concepts of orthopedic damage control surgery for patients with multiple injuries have placed an emphasis on appropriate surgical timing, limiting blood loss, and the duration of the initial operative procedure(s). Proponents of unreamed nailing have stated that reaming places polytraumatized patients "at risk," in part because it adds to the length of the surgical procedure and may exacerbate the severity of a patient's pulmonary injury. The purpose

Nonunion after a proximal fifth metatarsal fracture can cause considerable pain, with high morbidity and loss of work. Although many authors advocate early surgical management of zone 3 injuries (Jones fractures), zone 1 and 2 fractures are generally expected to heal with conservative management. Uncommonly, zone 1 and 2 fractures can develop nonunions. The aim of this study was to evaluate the efficacy of closed intramedullary screw fixation for nonunions of the fifth metatarsal base. We performed a prospective study involving all fifth metatarsal base nonunions treated in our department over

The treatment of massive tibial bone defects takes a very long time. An Ilizarov trifocal approach decreases the lengthening time. Subsequent intramedullary fixation decreases the complications of the conventional Ilizarov method. This technique was applied between June 2010 and June 2011 in 10 male patients with a mean age of 30 years. All had grade III open tibial fractures. The patients were treated at Mansoura University Hospital and a private hospital. The sequence of treatment included trifocal lengthening using the Ilizarov method, followed by removal of the fixator, temporary plaster c

INTRODUCTION: The femur is the most frequent involved site by post-attinic fractures. The appropriate treatment of pathological fractures after radiotherapy is still controversial as they are associated with a high risk of delayed consolidation and non-union. Authors review a single Center series of pathological fractures after radiation therapy in patients affected by soft tissue sarcomas analyzing incidence, risk factors, failure rate and proposing a flow chart of treatment of postattinic fractures of the femur.
METHODS: Authors selected 570 patients treated by limb salvage surgery associat

Ipsilateral femoral neck and shaft fractures are challenging injuries, and there are different fixation options but no consensus on a superior construct. Our preferred method is cannulated screw fixation of the femoral neck and antegrade reconstruction nailing of the shaft. Compressive fixation of the femoral neck fracture with cancellous lag screws followed by reconstruction nail placement provides compressive fixation and fixed angle support of the neck fracture while allowing for more optimal treatment of femoral shaft fractures at or above the isthmus. The purpose of this submission is to

Treatment of tibial shaft fractures is still discussed controversial. In the present study current literature was reviewed with the objective to demonstrate current recommendations concerning tibial shaft fractures. Tibial shaft fractures are often caused by high-energy trauma with severe concomitant soft-tissue injuries. Solid bone union without hypertrophy, fast mobilization and full range of motion without further soft-tissue damages are the aims of the therapy. Non-displaced tibial shaft fractures in patients with good compliance can be treated conservatively. Deep venous thrombosis, compa

There are several common types of fixations for metacarpal fractures: pins, plates, lag, and intramedullary (IM) screws. The advantages of pins are that they are ubiquitous, cost-effective, have shorter operative times, and preserve soft tissues, thereby minimizing adhesions. In this article, we describe metacarpal fracture fixation utilizing the technique of retrograde IM pinning through collateral recess access. We present the postoperative outcomes of our patients who underwent metacarpal fracture fixation utilizing this technique. Details of the fractures, patient comorbidities, demographi

Two patients, mother and daughter, had major hip joint problems related to osteopetrosis consisting of (1) subtrochanteric femoral fractures including delayed union, septic nonunion, and failed intramedullary fixation; and (2) coxa vara with symptomatic osteoarthrosis. Total hip arthroplasty (THA) was performed on three hips of these two patients, and the patients were followed for more than three years. One patient continues to ambulate without pain or the need for external support, and the potential for ambulation was established for the second patient, compromised only by her poor motivatio

Fractures around total knee arthroplasties are challenging clinical problems and include the following: stress fractures of the pelvis and femoral neck, supracondylar femur fractures, fractures of the proximal tibial metaphysis and diaphysis, and patellar fractures. Treatment focuses on restoration of the patient's prefracture functional status. The etiology of supracondylar femur fractures is multifactorial and treatment includes immobilization, retrograde intramedullary nailing, open reduction and internal fixation, and revision arthroplasty. The "Less Invasive Stabilization System" plate ha

An 85-year-old woman with a history of insulin-dependent diabetes mellitus, hypertension, and chronic venous insufficiency with underlying venous stasis who sustained a fall in her house presented to the emergency room with a displaced distal diaphyseal tibial fracture and a stable total knee arthroplasty. At her third day of admission, an intramedullary nail was inserted in a retrograde fashion through the calcaneus and talus into her tibial shaft to stabilize the fracture; there were no postoperative complications. Three years after surgery, the patient remains pain-free, the fracture had un

Intramedullary Nailing is a standard procedure for fixation of various fractures in orthopedic practice. With the procedure also important is being aware of the complications and methods of managing them. A broken or stuck hardware can result in these procedures. We present a case with broken guide wire in a case of femoral shaft fracture in a young individual and the technique we used to retrieve it retrogradely from the fracture site. This technique shall be a useful technique to all the orthopedic surgeons if such a situation arises.

We reviewed the cases of fifteen children who were treated for a Monteggia injury. Transverse and metaphyseal fractures of the ulna were stable after closed reduction, but oblique fractures of the shaft redisplaced and required intramedullary pinning. Five patients who were seen late had open reduction and internal fixation of the fracture. Four of these children had reconstruction or repair of the annular ligament, with excellent results. But in the child in whom the ligament was not repaired the radius subluxated.

PURPOSE: Distal femur fractures occur in older, osteoporotic patients and carry a measurable risk of subsequent ipsilateral hip fracture. Prophylactic femoral neck screw placement is believed by many to mitigate this risk, but clinical evidence is limited. This study evaluates the association between prophylactic femoral neck fixation after distal femur fracture and future ipsilateral hip fracture.
METHODS: A retrospective cohort study was conducted at a single Level 1 tertiary trauma center between 2018 and 2024. Adult patients (≥ 18 years) with AO 33A/B/C fractures and Su type 1-3 periprost

Impacted fractures of the base of the middle phalanx frequently lead to joint incongruity and functional impairment. Although percutaneous intramedullary elevation of depressed articular fragments has been described previously, PRIME (Percutaneous-Pin, Reduce, Internal Fixation, Mobilize Early) represents a technical refinement that emphasizes true closed intramedullary disimpaction under local anesthesia, fully subcutaneous short-cut K-wire fixation, and a protocol of immediate active postoperative proximal interphalangeal (PIP) motion. It was applied in six consecutive patients with acute im

Background: Kirschner wire (K-wire) fixation is widely used to repair metacarpal and phalangeal fractures. In this study, we simulated K-wire osteosynthesis of a 3-dimensional (3D) phalangeal fracture model and investigated the fixation strength at various K-wire diameters and insertion angles to clarify the optimal K-wire fixation method for phalangeal fractures. Methods: The 3D phalangeal fracture models were created by using computed tomographic (CT) images of the proximal phalanx of the middle finger in five young healthy volunteers and five elderly osteoporotic patients. Two elongated cyl
