An ethical defense of safer supply.

Safer supply is a medical form of harm reduction in which a patient is prescribed a drug to which they are addicted. Despite ample high-quality evidence that it is a safe and effective last-line treatment for severe, treatment-refractory substance use disorders, safer supply is not widely used or advocated. This is likely due in part to ethical concerns, but I contend that safer supply is ethical by both general moral standards and the professional ethical standards of healthcare. Recent defenses of various harm reduction interventions argue that they are permissible regardless of the moral qu…

[Safety and effectiveness of the early-onset sepsis calculator to reduce antibiotic exposure in at-risk newborns: a cluster-randomised controlled trial].

OBJECTIVE: Assessing the safety and effectiveness of the early-onset sepsis (EOS) calculator compared with the categorical guideline in reducing empiric antibiotic treatment in newborns with suspected EOS. DESIGN: Open-label, cluster-randomised controlled trial in 10 Dutch hospitals, randomised 1:1 to the EOS calculator or the categorical guideline. METHOD: 1830 newborns (gestational age ≥34 weeks, ≥1 EOS risk factor) were included. Non-inferiority was assessed using four predefined harm criteria (respiratory support, circulatory support, culture-proven EOS, referral to tertiary hospital). S

We Must Do Better: Addressing HCV Treatment Barriers in Persons Who Inject Drugs in the United States.

The opioid epidemic in the United States, along with a lack of adequate harm reduction services, has contributed to a sharp rise in hepatitis C virus (HCV) infections. Despite considerable evidence of the effectiveness of HCV treatment in people who inject drugs (PWID), and recommendations from clinical guidelines to prioritize treatment in PWID, there are multiple barriers to broad uptake of HCV treatment. These barriers exist at the systems level, as well as at the level of medical providers and patients. Interventions to remove treatment barriers in the United States include harm reduction …

Medicinal cannabis (Bedrolite) substitution therapy in inpatients with a psychotic disorder and a comorbid cannabis use disorder: A case series.

Cannabis use disorders are frequently comorbid in patients with a psychotic disorder and are associated with worse outcomes. To date there are no proven effective strategies to achieve cannabis abstinence in this population. An alternative for abstinence might be harm reduction, i.e. replacing the use of street cannabis with high tetrahydrocannabinol and low cannabidiol levels by medicinal cannabis variants with relatively low tetrahydrocannabinol and relatively high cannabidiol levels, thereby reducing the psychosis inducing effects of cannabis and enhancing the antipsychotic effects of canna…

Navigating the risk environment: structural vulnerability, sex, and reciprocity among women who use methamphetamine.

BACKGROUND: Drug users' risk sexual practices contribute to their increased risk for contracting HIV and other sexually transmitted infectio … behaviour in ways that are sometimes deemed transactional and risky by research, public health, or harm reduction professionals. CONCLUSION: Understanding the embeddedness of sexual practices in structural context and networks of reciprocity is essential to understanding implications for policy and harm reduction.

European Union policy on smokeless tobacco: a statement in favour of evidence based regulation for public health.

RATIONALE: This statement is an updated version of one released by the same authors in February 2003. The statement was produced to follow up the Royal College of Physicians (RCP) Tobacco Advisory Group report "Protecting smokers, saving lives: the case for a tobacco and nicotine regulatory authority", which argued for an evidence based regulatory approach to smokeless tobacco and harm reduction and posed a series of questions that regulators must address in relation to smokeless tobacco. The purpose of this statement is to provide arguments of fact and principle to follow the RCP's report and…

Self-harm and the COVID-19 pandemic: A study of factors contributing to self-harm during lockdown restrictions.

INTRODUCTION: The COVID-19 pandemic and resulting public health measures may have major impacts on mental health, including on self-harm. We have investigated what factors related to the pandemic influenced hospital presentations following self-harm during lockdown in England. METHOD: Mental health clinicians assessing individuals aged 18 years and over presenting to hospitals in Oxford and Derby following self-harm during the period March 23rd to May 17, 2020 recorded whether the self-harm was related to the impact of COVID-19 and, if so, what specific factors were relevant. These factors we

Animation-based education as a gambling prevention tool: correcting erroneous cognitions and reducing the frequency of exceeding limits among slots players.

In light of the financial harm that often accompanies problem gambling, and the difficulty in resolving it, there is a pressing need for prevention resources. In the present study, we examined the preventive effects of an animation-based video that educated participants on how slot machines function, the prudence of setting financial limits, and strategies to avoid problems. Non-problem gamblers (N = 242) at a slots venue were randomly assigned to watch either an animation or a control video. Compared to participants who watched the control video, those who watched the animation endorsed strat

Global, Regional, and National Burden of Self-Harm and Projections to 2030: A Systematic Analysis for the Global Burden of Disease Study 2021.

BACKGROUND AND AIMS: Self-harm is a global public health problem, yet comprehensive analyses of its trends and the impact of sociodemographic factors remain insufficient. This study aimed to systematically examine the spatial and temporal trends of self-harm burden from 1990 to 2021 and forecast trends through 2030. METHODS: We analyzed data from the Global Burden of Disease Study 2021, examining age-standardized incidence, mortality, and disability-adjusted life years (DALYs) for self-harm by region, age, gender, and sociodemographic index (SDI) from 1990 to 2021. Attributable factors were a

Systematic reviews as a 'lens of evidence': Determinants of benefits and harms of breast cancer screening.

This systematic review, stimulated by inconsistency in secondary evidence, reports the benefits and harms of breast cancer (BC) screening and their determinants according to systematic reviews. A systematic search, which identified 9,976 abstracts, led to the inclusion of 58 reviews. BC mortality reduction with screening mammography was 15-25% in trials and 28-56% in observational studies in all age groups, and the risk of stage III+ cancers was reduced for women older than 49 years. Overdiagnosis due to mammography was 1-60% in trials and 1-12% in studies with a low risk of bias, and cumulati

Self-compassion in Treatment and with Ourselves.

PURPOSEOF REVIEW: This article explores the emerging role of compassion in the treatment of individuals who have caused sexual harm. It also brings focus to the importance of self-care and self-compassion for the practitioners who do this work. RECENT FINDINGS: Early studies into interventions designed to prevent offending behavior focused more on risk reduction than client resilience and well-being. The good lives model shifted focus to rehabilitation centered on clients' strengths. Recent critiques highlight the importance of compassion in treatment and self-compassion for practitioners wor

Agonist treatment in opioid use: advances and controversy.

Opioid dependence is a chronic relapsing condition which requires comprehensive care; pharmacological agents form the mainstay of its long term treatment. The two most popular approaches are the harm reduction method using agonists and the complete abstinence method using antagonists. Currently, particularly from the harm minimization perspective and the low feasibility of an abstinence based approach, there is an increasing trend toward agonist treatment. The use of buprenorphine has gained popularity in view of its safety profile and the availability of the buprenorphine-naloxone combination…

Behavior change techniques in mobile apps targeting self-harm in young people: a systematic review.

Despite the high prevalence of self-harm among young people, as well as their extensive use of mobile apps for seeking support with their mental healthcare, it is unclear whether the design of mobile apps aimed at targeting self-harm is underpinned by behavior change techniques (BCTs). To systematically analyze and identify (a) what BCTs and (b) what theories are present in self-harm apps for young people in an attempt to understand their active components. Systematic searches in Apple store, followed by thorough screening, were conducted to identify free mobile apps targeting self-harm in you

Interventions to reduce harm associated with adolescent substance use.

A major proportion of the disease burden and deaths for young people in developed nations is attributable to misuse of alcohol and illicit drugs. Patterns of substance use established in adolescence are quite stable and predict chronic patterns of use, mortality, and morbidity later in life. We integrated findings of systematic reviews to summarise evidence for interventions aimed at prevention and reduction of harms related to adolescent substance use. Evidence of efficacy was available for developmental prevention interventions that aim to prevent onset of harmful patterns in settings such a
