Predicting treatment adherence in patients with drug-resistant tuberculosis: insights from socioeconomic, demographic, and clinical factors of patients in the rural Eastern Cape.

BACKGROUND: Drug-resistant tuberculosis (DR-TB) poses a serious challenge to global health. Patients must follow complex medication regimens over long periods, and any failure to comply with these treatment plans can result in treatment failure, higher mortality rates, and an increased risk of developing additional drug resistance. SETTING: The study was conducted in the rural Eastern Cape. AIM: This study aims to identify the key factors influencing treatment adherence among patients with DR-TB. Furthermore, it rigorously evaluates the predictive accuracy of machine learning models in asses

The Experiences of Women With Polycystic Ovary Syndrome of the Healthcare They Receive: A Qualitative Systematic Review.

BACKGROUND: More than 12% of women worldwide are affected by polycystic ovary syndrome (PCOS), whose symptoms are similar to those of puberty, often leading to delayed diagnosis and missing the opportunity for early intervention. This not only puts PCOS women under physical and mental stress but also reduces their trust in doctors and makes them dissatisfied with the healthcare they receive, which in turn affects their quality of life. Therefore, to improve the doctor-patient relationship and promote health, it is essential to investigate and understand the healthcare experiences that women wi

Global, Regional, and National Epidemiological Burden of Motor Neuron Disease: A Comprehensive Assessment of Prevalence, Incidence, Mortality, and Disability-Adjusted Life Years with Projections to 2036.

BACKGROUND: Motor neuron disease (MND), a rare and fatal neurodegenerative disorder, poses significant challenges to healthcare systems. Existing research has limitations, such as sparse data in some regions, insufficient exploration of sociodemographic impacts, and a lack of long-term burden projections. Understanding its global, regional, and national impact is crucial for healthcare planning. OBJECTIVE: This study comprehensively evaluates the worldwide MND burden from 1990 to 2021, forecasts trends until 2036, and identifies key determinants of disease burden changes. The findings aim to

Survey on national practices regarding iodine thyroid blocking in 2016-2017.

Severe nuclear accidents may lead to a release of radioactivity, including radioactive iodine, into the environment. The thyroid gland in the human body needs natural or stable iodine to function properly and this iodine is normally absorbed in small quantities from food. Following a release of radioactive iodine from a nuclear or radiological accident, the body will absorb and accumulate the radioactive iodine in the thyroid gland. This increases the risk of thyroid cancer, especially in children. Since the thyroid gland cannot distinguish between radioactive and stable iodine, stable iodine

COVID-19 and pandemic planning in the context of rural and remote homelessness.

Addressing the vulnerability and unique needs of homeless populations during pandemics has been a major component of the Canadian federal response to the COVID-19 crisis. Rural and remote communities, however, have received little to no funding to aid in their care of homeless people during the pandemic. Similarly, there has been little to no research on rural communities' pandemic preparedness in the context of homelessness. There are large numbers of homeless individuals in rural and remote Canada, including Indigenous peoples who are over-represented in homeless populations. Rural communiti

Exploring global efforts in mental health screening and early detection.

INTRODUCTION: Screening tools are frequently used to assess symptoms of mental disorders in different settings before they worsen. They may theoretically be used in large-scale programs to support implementation of national action plans and mental health strategies in consideration of broader or more specific lines of action for support of individuals struggling with mental health issues. Although screening tools and programs are generally effective, there are inconsistencies in their design, planning, application, measurement of outcomes, mechanisms used for follow-up, and ethical considerati

Development of a Dutch Information Standard for Advance Care Planning.

Advance care planning (ACP) information, concerning goals and preferences for future medical treatment and care, is often recorded but its digital exchange between healthcare providers is not yet self-evident. This paper describes the process of the development of a Dutch information standard for ACP, aimed to support this information exchange. For the current release of the information standard, a functional design with a dataset description based on Dutch Health Care Information Models and FHIR implementation guides for technical specifications were developed. This release underwent public c

Testosterone Therapy for Cognition in Menopause: Systematic review and meta-analysis.

Menopause is a stage of life that can affect thinking and memory, often colloquially called 'brain fog'. Brain fog, can affect daily life and relationships. Hormone treatments are often used to manage symptoms, but it is still not clear how well they work. Testosterone is sometimes prescribed for low sex drive, but its effects on thinking and memory are unclear. This study reviews the existing evidence. Five studies were identified. These studies looked at different areas of thinking and memory, including attention, planning, decision-making and learning. We found no clear evidence that testos

Ethical and legal considerations in the care of children and young people with high consequence infectious diseases (HCIDs): an approach to decision making.

High consequence infectious diseases (such as Ebola virus or avian influenza) require specialist management with strict isolation to avoid spread to health-care staff and the wider community. These infections present various ethical and legal issues for children and young people. Specific challenges include the impact of isolation on the child and family (potentially without a child's consent), limitations to care due to staff safety considerations, and reduction of resources for other children (due to potential closure of paediatric intensive care unit beds). The complex decision making requi

Machine learning-driven imaging data for early prediction of lung toxicity in breast cancer radiotherapy.

One possible adverse effect of breast irradiation is the development of pulmonary fibrosis. The aim of this study was to determine whether planning CT scans can predict which patients are more likely to develop lung lesions after treatment. A retrospective analysis of 242 patient records was performed using different machine learning models. These models showed a remarkable correlation between the occurrence of fibrosis and the hounsfield units of lungs in CT data. Three different classification methods (Tree, Kernel-based, k-Nearest Neighbors) showed predictive values above 60%. The human pre

An examination of postpartum family planning in western Kenya: "I want to use contraception but I have not been told how to do so".

Postpartum family planning (FP) in Kenya is low due to inadequate sensitization and awareness among women, particularly in rural areas. This paper identifies most widely used types of FP, intent and unmet needs among women, FP counseling and barriers to FP uptake. Focus group discussions with providers, traditional birth attendants (TBAs) and mothers, as well as in-depth interviews identify key themes including preferred postpartum FP, limits to existing FP counseling and barriers to FP uptake. Postpartum FP is common including injectable contraceptives, oral contraceptives, coils, condoms, an

Barriers to the best care of the dying in Queensland, Australia.

The process of dying for many Australians is not ideal. To improve the care of the dying in our community, the barriers preventing optimal care must be identified. Forty-two important barriers were identified by focus groups. Health care professionals (HCPs) working in palliative care (PC) throughout Queensland were asked to rate the importance of each of the barriers. Inadequate funding for PC, lack of after-hours care, insufficient medical support and the lack of HCPs in PC across several different settings were highlighted as the most important barriers. Uncertainty regarding death certific

A comfortable place to say goodbye.

No death is easy, but we hope to die peacefully, with our loved ones surrounding us, comforted by each other's presence and the knowledge that the right decision has been made. The nurse's actions in the intensive care unit setting can promote hope for a comfortable place to say goodbye for the patient and his/her loved ones and the nursing staff and physicians. When nurses make the intensive care unit a comfortable place for the dying patients and their loved ones, we also make the patients' deaths comfortable for us. The problems encountered in implementing end-of-life care are explored in t

The impact of child mortality on fertility in South Africa: Do child support grants and antiretroviral treatment matter?

This paper investigates the effect of under-five mortality, child support grant (CSG) coverage and the rollout of antiretroviral therapy (ART) on fertility in South Africa. The study employs the quality-quantity trade-off framework to analyse the direct and indirect factors affecting fertility using the two stage least squares fixed effects instrumental variable approach. The analysis uses balanced panel data covering nine provinces from 2001-2016. This period was characterised by significant increases in the child support grant coverage and ART coverage. Furthermore, this period was character

Building a Sustainable Comprehensive Multiple Myeloma Program in Western Kenya.

Despite improved treatment strategies for multiple myeloma (MM), patient outcomes in low- and middle-income countries remain poor, unlike high-income countries. Scarcity of specialized human resources and diagnostic, treatment, and survivorship infrastructure are some of the barriers that patients with MM, clinicians, and policymakers have to overcome in the former setting. To improve outcomes of patients with MM in Western Kenya, the Academic Model Providing Access to Healthcare (AMPATH) MM Program was set up in 2012. In this article, the program's activities, challenges, and future plans are

Exploring readiness for advance care planning in Japan: A qualitative interview study with older adults with frailty, family caregivers, and health and social care practitioners in the homecare setting.

BACKGROUND: Older adults with frailty are at risk of rapid health decline and poor outcomes, including end of life. While advance care planning (ACP) can support person-centred care, how to assess and support older adults' ACP readiness remains unclear. Health and social care practitioners, and family caregivers, are often main providers of care, but their role in ACP is poorly understood. OBJECTIVES: To understand the readiness of older adults with frailty to engage in ACP from the perspectives of older adults, family caregivers, and health and social care practitioners. DESIGN: An explorat

Managing acute bromazolam intoxication in a custodial setting: a prison health case study.

PURPOSE: Synthetic benzodiazepines pose significant health risks, especially within prison settings where substance use, multi-morbidity and constrained healthcare environments intersect. As the illicit drug market continues to evolve, prisons face increasing challenges in responding to changing patterns of use. This paper aims to present a case study of a bromazolam-related drug incident in a Scottish prison to identify operational learnings that can support future approaches to meeting the health needs of those within prison settings. DESIGN/METHODOLOGY/APPROACH: The authors conducted a des
