Kearns-Sayre syndrome is a rare mitochondrial disorder characterized by large-scale deletion or rearrangement of mitochondrial DNA, which is usually not inherited but occur spontaneously probably at the germ cell level or very early in embryonic development by Mehndiratta et al. (Neurol India 50:162-167, 2002). Neuromuscular and cardiac conduction abnormalities are most commonly involved in these patients, which may have subtle presenting signs.

Escape-capture bigeminy is a well-known arrhythmia characterized by group beating of a junctional beat followed by a conducted sinus beat. We report a variant of this arrangement where a junctional beat gives rise to a retrograde P wave, which is then conducted back to the ventricles producing a hitherto undescribed combination (escape-echo bigeminy) resembling escape-capture bigeminy. The clinical significance of escape-echo bigeminy appears similar to that of classic escape-capture bigeminy.

An 18-year-old woman with a concealed right midseptal accessory pathway and refractory supraventricular tachycardia with a cycle length of 280-400 ms and a wide echo zone of 280-520 ms is reported. The transvenous electric catheter ablation with two shocks of 200 and 300 J, each on a separate occasion, was followed by anterograde and retrograde atrioventricular block. A unique mechanism of "cross-talk" has recently been described in the Gemini 415A DDD pacemaker in which cross-inhibition of the ventricular output stimulus may occur in the early Gemini 415A models which were manufactured with dual anodal rings interconnected by a wire. The Gemini 415A units with dual anodal rings were implanted under investigational protocol in 53 patients from September 1982 to August 1984. We describe a patient, in whom electrophysiologic study revealed a "negative" P-A interval in the His bundle electrogram, a prolonged intraatrial conduction time and a prolonged spike to the P wave interval during atrial pacing. The abnormalities were due to extensive sinuatrial disease.

Malalignment of the interatrial and interventricular septal structures is rarely seen associated with atrioventricular septal defects. Such malalignment gives rise to an anomalous disposition of the conduction tissue specifically in regard to the siting of the atrioventricular node. Recognition of this abnormality prior to (or at) surgery is important if post-operative rhythm problems are to be avoided.

Personal observations of 22 patients with permanent inhibited ventricular PM for SSS during the period July 1976-February 1979 and re-examined with electrophysiological study after a follow-up made for an average period 25,5 months (14 to 49 months), after PM inhibition from the outside are presented. A 61-year-old man presented with symptomatic 2 degree Mobitz II AV block in association with bifascicular block. Ventricular pacing resulted in temporary depression of AV conduction. The extent of pacing-induced AV block varied directly with the duration and rate of ventricular pacing. Intracardiac recordings proved that the site of spontaneous and pacing-induced AV block was distal to the His bundle. Possible mechanisms by which this phenomenon may arise and several implications of practical importance are discussed.

The clinical and electrophysiological data in 52 consecutive patients with bundle branch block and followed-up for an average period of 20.8 +/- 10.4 months was reviewed. In a patient with spells of paroxysmal supraventricular tachycardia of two types, the stimulation tests demonstrated a dual intranodal conduction responsible sometimes for "usual" reciprocal rhythms (slow anterograde pathway and rapid retrograde pathway), sometimes for "reversed" reciprocal rhythms (rapid anterograde pathway and slow retrograde pathway).
