In vivo assessment of the subacromial space during shoulder elevation following acromioclavicular joint reconstruction.

High-grade disruptions to the acromioclavicular joint (ACJ) significantly impair scapular function, which may compromise the subacromial space and increase the risk of impingement related pathology. Research has traditionally focused on the minimum distance between the humeral head and undersurface of the acromion, which may not accurately address the space above the supraspinatus footprint. This study aimed to quantify the subacromial space between the anterior acromion and the supraspinatus footprint, as well as the general humeral-acromial spacing during muscle-driven shoulder motion, in in…

Left shoulder pain from humeral Rosai-Dorfman disease.

Rotator cuff impingement was suspected when a 23-year-old male police officer presented to the orthopedic clinic with gradual-onset left shoulder pain and weakness; however, conservative management did not improve the patient's symptoms. Subsequent imaging revealed a lytic lesion in the left humeral head, with CT-guided biopsy findings consistent with Rosai-Dorfman disease, an extremely rare histiocytic disorder. Typically self-limiting and benign, Rosai-Dorfman disease is characterized by overproduction and accumulation of rare non-Langerhans cell histiocytes, a type of immune cell, in variou…

A Detailed Analysis of the Blood Supply to the Subscapularis Muscle.

This study aimed to provide a comprehensive description of the arterial supply to the subscapularis (SSC) muscle. This will provide critical information for various surgical procedures. Ten specimens of embalmed Korean cadavers were dissected and subjected to modified Sihler's method to reveal the branching pattern of the arteries surrounding the subscapularis, and its intramuscular blood supply. The SSC muscle was generally supplied by branches from the subclavian artery (suprascapular artery, supraSA; circumflex scapular artery, CxSA; and dorsal scapular artery, dSA) and the axillary artery

The Rotator Cuff and the Superior Capsule: Why We Need Both.

Tears of the rotator cuff are frequent. An estimated 250,000 to 500,000 repairs are performed annually in the United States. Rotator cuff repairs have been successful despite fatty infiltration and atrophy of the rotator cuff muscles. Although the emphasis in rotator cuff repair has historically focused on re-establishing the tendon attachment, there is growing interest in and understanding of the role of the superior capsule. The superior capsule is attached to the undersurface of the supraspinatus and infraspinatus muscle-tendon units, and it resists superior translation of the humeral head.…

Functional results and unfavorable events after treatment of proximal humerus fractures using a new locking plate system.

BACKGROUND: Proximal humerus fractures are often treated with a fixed-angle titanium plate osteosynthesis. Recently, plates made of alternative materials such as carbon fibre-reinforced polyetheretherketone (CFR-PEEK) have been introduced. This study presents the postoperative results of patients treated with a CFR-PEEK plate. METHODS: Patients with proximal humerus fractures treated with a CFR-PEEK plate (PEEKPower™ Humeral Fracture Plate (HFP)) were included. In follow-up examination, age and gender adjusted Constant-Murley Score (ACS), Subjective Shoulder Value (SSV), Quick Disabilities of

Anatomical Study of the Biceps Sheath for Safe and Accurate Unguided Anterior Shoulder Injections.

The tendon of the long head of the biceps brachii (LHBT) contributes to shoulder joint stability, but can become a source of shoulder pain because of trauma or degeneration. Injection of local anesthetics into the biceps sheath (BS) is an effective treatment for managing anterior shoulder pain and is widely performed clinically; however, the accuracy of unguided injections remains low. This study aimed to clarify the anatomical characteristics of the BS and identify the optimal injection site to improve the accuracy and safety of unguided anterior shoulder injections. Eight 5% formalin-fixed c

Difficult elbow fractures: pearls and pitfalls.

Complex elbow fractures are exceedingly challenging to treat. Treatment of severe distal humeral fractures fails because of either displacement or nonunion at the supracondylar level or stiffness resulting from prolonged immobilization. Coronal shear fractures of the capitellum and trochlea are difficult to repair and may require extensile exposure. Olecranon fracture-dislocations are complex fractures of the olecranon associated with subluxation or dislocation of the radial head and/or the coronoid process. The radioulnar relationship usually is preserved in anterior but disrupted in posterio

Prospective study of double-eccentric hemi shoulder arthroplasty in different aetiologies: midterm results.

This prospective study aimed to analyse the effect of a newly developed double-eccentric adjustable stemmed prosthesis on reconstruction of … pathology. The best results were observed for primary osteoarthritis and avascular necrosis of the humeral head. All heads were eccentrically positioned. Specific stem-related complications were not observed. Because of the eccentric positioning of all heads it is reasonable to use adjustable shaft prostheses. The clinical results are comparable to data in th …

[Experiences with primary osteosynthesis in radial head fractures].

Displaced fractures of the radial head are indications for early surgery. The resection is unsatisfactorily leading often to an instable elbow in valgus deformity and to painful troubles in the distal radio ulnar joint. Prosthesis implantation has its own problems. Reconstructive surgery is obviously the best solution. 21 internal fixations in 20 patients were performed using small screws (and 2 plates) between 1968 and 1976. The functional and radiological results are excellent in 15 out of 19 controlled patients. 3 patients have a moderate reduction of the mobility, 1 patient a non-union. -W

Secondary subacromial impingement after valgus closing-wedge osteotomy for proximal humerus varus.

A 31-year-old construction worker had been suffering from both the motion pain and the restriction of elevation in his right shoulder due to severe varus deformity of humeral neck, which occurred after proximal humeral fracture. The angle for shoulder flexion and abduction was restricted to 50 and 80 degrees, respectively. Valgus closing-wedge osteotomy followed by the internal fixation using a locking plate was carried out at 12 months after injury. Postoperatively, the head-shaft angle of the humerus improved from 65 to 138 degrees. Active flexion and abduction angles improved from 80 to 135

Ovine fetal growth curves in twin pregnancy: ultrasonographic assessment.

PURPOSE OF INVESTIGATION: The fetal-lamb model is a fundamental tool for developing clinical applications for the treatment of human fetal pathology (e.g., open fetal surgery). Accurate estimation of gestational age is important to avoid size-related problems during surgery. METHODS: To evaluate the effect of twinning on ovine fetal growth, we followed seven twin pregnancies in Comiso ewes from mating through parturition. Fetal growth indexes (muzzle-occipital and mean abdominal diameters, abdominal circumference, femoral and humeral length) were measured weekly using perimammary ultrasonogra

Severely displaced proximal humeral epiphyseal fractures: a follow-up study.

Forty-eight patients with severely displaced proximal humeral epiphyseal fractures were reviewed. Twenty-one patients were followed up for an average of 9 years after injury with a personal interview, examination, and radiographs. Average age was 14.1 years, half of whom were age 15 years or older. Initial head-shaft displacement averaged 80%, and 31 fractures were displaced by > or = 80%. All underwent attempted closed reduction. Radiographs for all except three were sufficient to judge the adequacy of reduction. Twenty-six of 45 attempted closed-reductions failed to improve displacement sign

The three-dimensional shoulder pain alignment (3D-SPA) mobilization improves pain-free shoulder range, functional reach and sleep following stroke: a pilot randomized control trial.

Background and purpose: Following a stroke, three-dimensional clavicular/scapular/humeral joint rotations may become restricted and contribute to post-stroke shoulder pain.
