The potentiation of infection is a recognized adverse effect of steroids. This study examined the in vitro effect of glucocorticoids on sheep alveolar macrophage phagocytic function, an essential means of cellular defence in the lungs, by evaluating the phagocytosis of zymosan particles after 30 min of contact between alveolar macrophages and steroids. Hydrocortisone, dexamethasone phosphate and methylprednisolone significantly reduced the phagocytic capability of sheep alveolar macrophages, but the effects of dexamethasone isonicotinate, prednisolone and flumethasone were less marked.

The authors propose a method for combined treatment of patients with varying neurologic syndromes of cervical osteochondrosis which includes manual therapy, traction of the cervical portion of the vertebral column and some physiotherapeutic procedures (hydrocortisone phonophoresis and various baths). The use of drugs was limited. The above treatment was given to 330 patients with reflectory and radicular syndromes of cervical osteochondrosis. The results of the treatment proved significantly better as against those observed in patients receiving conventional treatment.

Glucocorticoid hormones inhibited the early stage of propagation of Inoue-Melnick virus in a dose-dependent manner in human diploid cell cultures. Prednisolone and hydrocortisone achieved complete inhibition of replication of the three Inoue-Melnick virus serotypes at concentrations of 0.2 X 10(-4) and 10(-5) M, respectively; strong inhibition was found even at 10(-5) and 10(-6) M, respectively. The steroid hormones did not show a direct effect on the virus.

Fourteen patients suffering from rosacea treated by prolonged topical applications of fluorinated steroids showed adverse effects. Aggravation and extension of telangiectasia occurred in all, and in most cessation of the treatment was followed by severe rebound inflammatory oedema and acute pustular eruption. The telangiectasia cleared or was much improved within three months of cessation of the use of fluorinated steroids. Hydrocortisone did not produce the same effect, and hence it was used, together with oral tetracycline, for treatment.

Itching of the ear canal, with oozing, edema and cracking of the skin due to absence or insufficiency of wax was treated in more than 200 cases by instructing the patient to massage the ear canal with a cotton tip applicator soaked with a hydrocortisone preparation and inserted a half to three-quarters of an inch beyond the meatus. Some 95 per cent of patients reported good to excellent results in three to four days after the beginning of treatment. In the remainder results were fair.

Hormonal homeostasis was examined in 104 children aged 1 to 14 years with thymus enlargement. There was a significant growth of somatotropic hormone in children of the first 3 years of life, characterized by thyroid dysfunction and hypofunction increase with age, by a tendency towards ACTH and hydrocortisone lowering under 6 years of age, and a significant decrease of those parameters in children over 6 years. The children with thymus enlargement also showed other disorders of hormonal homeostasis.

The blood hydrocortisone and corticosterone content and the aldosterone level in the urine were determined for 24 hours in patients with congenital heart disease before and after operative treatment. In patients with symptoms of cardiac insufficiency both glucocorticoid and mineralocorticoid activity of the adrenal cortex was marked by deviations from the diurnal physiological rhythm. This is reflected in the continuous activation of the adrenal cortex during 24 hours; activation of mineralocorticoid function in the evening and during the night is a particularly characteristic phenomenon.

Proquazone, 1-isopropyl-4-phenyl-7-methyl-2(1H)-quinazolinone is an orally effective anti-inflammatory analgesic and antipyretic compound in animal models. The compound is less ulcerogenic than indomethacin in the rat and appears to potentiate the anti-inflammatory and thymus involution effects of hydrocortisone in this species. That a nonacidic nonsteroidal compound would display a spectrum of anti-inflammatory activity similar to that previously found only with acidic compounds will be the subject of further investigations.

The clinical course and diagnosis of a patient with Cushing's disease complicated by pregnancy is described, and the anaesthetic management of trans-sphenoidal selective adenomectomy performed during the second trimester outlined. Problems included obesity, diabetes, hypertension and a suboptimal airway. Fibreoptic awake intubation and intravenous anaesthesia were used. Insulin requirements decreased substantially after surgery. Early administration of hydrocortisone after surgery avoided the risk of an addisonian crisis but delayed biochemical confirmation of a metabolic cure.

We report a girl with acute lymphoblastic leukemia who developed severe conjugated hyperbilirubinemia (total bilirubin 34.32 mg/dl, direct bilirubin 26.94 mg/dl) following gram-negative (Escherichia coli ) sepsis. Despite broad-spectrum antibiotics, her hyperbilirubinemia progressed. Her mental status deteriorated and steroid therapy (hydrocortisone 8 mg/kg/d) was initiated. Her condition then recovered rapidly. Based on our findings, we suggest that steroid may help to treat progressive conjugated hyperbilirubinemia due to gram-negative sepsis.

A 45-year-old man presented with anterior pituitary failure, requiring thyroxine, hydrocortisone and androgen replacement. An MRI scan revealed a large cystic pituitary mass and thickening of the pituitary stalk. Over three years, diabetes insipidus and bitemporal hemianopia developed and the cystic mass had enlarged on MR scanning. Transphenoidal resection was performed with normalisation of the visual fields. Histology revealed lymphocytic hypophysitis, which is rare in men. The presentation with cystic enlargement is unique.

A composite physical model involving the simultaneous receding boundary release of drug from a drug suspension-silicone polymer matrix system, diffusion across the aqueous layer, and passive transport across the vaginal membrane consisting of parallel lipoidal and aqueous pore pathways is described. Simulation studies with progesterone and hydrocortisone illustrate matrix release-limiting, membrane absorption, and aqueous diffusion layer-limiting cases when the cylindrical silicone delivery device is interfaced with the vaginal membrane of the rabbit.

Addison's disease is an uncommon endocrine condition manifested by a variety of nonspecific symptoms, such as malaise, anorexia and nausea. Symptoms usually do not occur until most of the adrenal gland has been destroyed. Autoimmune disease has surpassed tuberculosis as the primary cause of Addison's disease. Nevertheless, tuberculosis still accounts for a significant proportion of cases. The rapid adrenocorticotropic hormone (ACTH) stimulation test is useful for identifying adrenal insufficiency. Maintenance therapy consists of hydrocortisone and fludrocortisone.

The effect of various pathogenic factors and drugs on sensitivity to strophanthin and the possibility of pharmacoprophylaxis of toxicosis caused by this cardiotonic agent were studied in acute and chronic experiments on 375 cats. A 51-year-old patient exhibited severe neutropenia over a period of 12 months following lobectomy for carcinoma of the lung. Marrow neutrophil mass and marrow neutrophil production were measured quantitatively and were within normal limits. Mobilization into a skin window was absent. A defect in neutrophil motility could be demonstrated in vitro and was confirmed in vivo by poor neutrophil release from the marrow following injection of hydrocortisone; it thus represented a lazy leukocyte syndrome of the adult.

OBJECTIVE: To compare low and high dose, and short and long acting corticosteroids in the treatment of carpal tunnel syndrome.
METHODS: A randomised, controlled, single blind trial with electromyographic and subjective outcome measures.
RESULTS: 25 mg hydrocortisone is as effective as higher doses or long acting triamcinolone at a six week and six month follow up.
CONCLUSION: As low dose steroid is as effective, and potentially less toxic, this should be the recommended dose for injection of carpal tunnel syndrome.

We experienced a case of serious adverse reaction following the administration of Gd-DTPA (gadolinium with diethylenetriamine pentaacetic acid). The patient showed sneezing and hoarseness when Gd-DTPA was administered for the first time. At the second injection, when hydrocortisone was used in advance to prevent allergic reaction, she lapsed into a pre-shock state and anaphylactic reaction was strongly suggested by the laboratory data. The possibility that Gd-DTPA may cause the severe side effects must be noted.
