Adrenal adenoma presenting with torsade de pointes--a case report.

A case of primary hyperaldosteronism due to an adrenal adenoma with near syncope and torsade de pointes is described. A woman patient had a history of high blood pressure and severe hypokalemia that was the cause of her ventricular arrhythmia, which was controlled by administering potassium supplementation but no antiarrhythmic medication. Adrenal adenoma was identified on axial computerized tomography. This case report suggests that there may be a chance of complete cure from torsade de pointes if the underlying cause of QT prolongation can be identified.

Prenatal diagnosis of Bartter syndrome.

Bartter syndrome, an autosomal recessive disorder of hyperaldosteronism and increased plasma renin, was suspected in an at-risk pregnancy due to the early occurrence of polyhydramnios. Further establishment of the diagnosis was accomplished by demonstrating increased levels of aldosterone in amniotic fluid and fetal cord blood. Electrolyte levels did not differ significantly from reported controls. It is thus suggested that polyhydramnios is the result of increased fetal urine output in Bartter syndrome and that amniotic fluid aldosterone is a reliable marker for the prenatal diagnosis of this…

A Case of Retroperitoneal Liposarcoma Mimicking an Adrenocortical Carcinoma.

An adrenal mass can be a diagnostic challenge as it is not easy to differentiate the adrenal glands from other adrenal pseudotumours with only radio-imaging. We report a 28-year-old patient who was diagnosed radiologically as an adrenal cortical carcinoma after he presented with abdominal pain and fullness. Biochemically, he demonstrated secondary hyperaldosteronism. Intra-operatively there was a huge mass, inferior to a normal right adrenal, which was histopathologically proven to be a dedifferentiated liposarcoma.

With a pinch of salt.

This report highlights a case of severe hyponatraemia secondary to excessive sweating and salt [corrected] poor fluid consumption and low salt diet in hot conditions. The case was complicated by the presence of marked hypokalaemia caused by secondary hyperaldosteronism confirmed, for the first time, by the presence of grossly elevated serum renin and aldosterone concentrations. With the rise in global temperature affecting even temperate climates doctors, especially in acute and general medicine, may be faced with this condition more often.

Hyporeninemic hypoaldosteronism in a patient with cirrhosis and ascites.

A patient with cirrhosis and coexistent hyporeninemic hypoaldosteronism secondary to diabetic nephropathy rapidly formed ascites despite marked reductions in plasma aldosterone concentration and urinary aldosterone excretion. To my knowledge, this association has not been previously reported. This case supports the concept that hyperaldosteronism is not a necessary component of the salt retention of advanced liver disease. Furthermore, it suggests that certain renal disorders should be considered in cases of cirrhosis and ascites with decreased plasma renin activity.

Cyproheptadine and mineralocorticoid effector mechanisms.

Cyproheptadine has recently been reported to blunt the furosemide-induced rise in PRA in normal subjects and to acutely lower plasma aldosterone levels in patients with hyperaldosteronism due to bilateral adrenal hyperplasia; both actions have tentatively been ascribed to the antiserotoninergic action of the drug. We here describe receptor studies showing cyproheptadine to occupy mineralocorticoid, but not glucocorticoid, receptors in rat and mouse kidney. On bioassay, cyproheptadine is a partial mineralocorticoid agonist/predominant antagonist.

Surreptitious habitual vomiting simulating Bartter's syndrome.

A patient with hypokalemic alkalosis, normotensive hyperreninism, hyperaldosteronism, increased levels of urinary and plasma prostaglandin E, and vascular hyporesponsivity to angiotensin II was thought to have Bartter's syndrome. Results of a kidney biopsy showed hyperplasia of the juxtaglomerular apparatus but no renomedullary cell hyperplasia. A 24-hour urine collection showed a low chloride level and no increase in the fractional chloride clearance, thus excluding Bartter's syndrome. Subsequent disclosure of surreptitious, habitual vomiting explained the hypokalemia.

Functioning unilateral adrenocortical carcinoma in a dog.

An 11-year-old, 24-kg, intact female Siberian husky dog in anestrus had a 2-month history of polyuria and polydipsia. The dog had signs of mineralocorticoid excess such as hypertension and hypokalemia refractory to potassium supplementation. Abdominal ultrasound revealed an irregular mass in the left adrenal gland. The ACTH stimulation test for aldosterone concentration did not reveal hyperaldosteronism. Unilateral adrenalectomy was performed and histopathology identified adrenal cortical carcinoma. All clinical signs of mineralocorticoid excess ceased after surgery.

Liquorice health check, Oro-dental implications, and a case report.

Liquorice has an active substance, Glycyrrhizin which inhibits the conversion of precursor cortisol to cortisone by inhibiting the enzyme 11-betahydroxysteroid dehydrogenase. When imbibed, liquorice acts like hyperaldosteronism which presents with typical symptoms including high blood pressure, low blood potassium, and muscle pain and weakness. This article appraises physiological and pharmacological effects on health of liquorice, critiques products containing liquorice, describes a typical case report of liquorice-induced hypertension, and appraises oral effects from consumption of liquorice…

A hypertensive emergency with acute visual impairment due to excessive liquorice consumption.

Hypokalaemic hypertension is the classical presentation of primary hyperaldosteronism but may also result from other mineralocorticoid activity, such as liquorice ingestion. Onset of hypertension as well as serum renin and aldosterone levels are central for the diagnosis. Liquorice ingestion has been reported to induce hypertension, hypokalaemia and metabolic alkalosis due to inhibition of the enzyme 11-β-hydroxy steroiddehydrogenase 2. Here, we report the case of a hypertensive emergency with acute visual impairment due to hypertensive retinopathy in clear conjunction with a considerable cons…

Endocrine hypertension in small animals.

Hypertension is classified as idiopathic or secondary. In animals with idiopathic hypertension, persistently elevated blood pressure is not caused by an identifiable underlying or predisposing disease. Until recently, more than 95% of cases of hypertension in humans were diagnosed as idiopathic. New studies have shown, however, a much higher prevalence of secondary causes, such as primary hyperaldosteronism. In dogs and cats, secondary hypertension is the most prevalent form and is subclassified into renal and endocrine hypertension. This review focuses on the most common causes of endocrine h…

Extent of adrenalectomy for adrenal neoplasm: cortical sparing (subtotal) versus total adrenalectomy.

The standard operation on adrenal neoplasias is a complete adrenalectomy. Accepted exceptions are bilateral inherited pheochromocytomas. In these cases, clinical and biochemical cure, as well as preservation of cortical function, can be achieved by a noncomplete adrenalectomy. In that procedure, at least one third of one gland has to be preserved. In unilateral adrenal tumors, partial resection has been used, especially in Conns adenomas, with early results comparable to those of total adrenalectomy. Because longterm results are still limited in hyperaldosteronism, final conclusions are not po…

Secondary hypertension: a practical diagnostic approach.

Secondary hypertension is a relatively uncommon, potentially reversible condition for which physicians should maintain a high index of suspicion. When a patient is found to have elevated blood pressure, a through history should be obtained and a complete physical examination should be performed. Depending on the findings, additional screening and diagnostic studies may be necessary. The most common causes of secondary hypertension are renal parenchymal disease, renovascular disease, coarctation of the aorta, pheochromocytoma, primary hyperaldosteronism, thyroid dysfunction and Cushing's syndro…

Secondary hypertension due to renin secreting tumor: a case report.

We report a 20-year-old female with severe hypertension, hypokalemia, hyperaldosteronism and elevated level of plasma renin activity (PRA). The clinical diagnosis of renin secreting tumor of left kidney was made before surgery by the lateralization of plasma renin activity via selective renal venous blood sampling and CT scan of kidney. Prompt normalization of blood pressure and decrease of PRA and aldosterone levels were found after surgical removal of the tumor. The pathological findings showed a juxtaglomerular cell tumor.

Fludrocortisone therapy for hypercalcemia.

A patient with mild hypercalcemia associated with adenocarcinoma of the lung was treated with fludrocortisone, a synthetic mineralocorticoid. Hypercalcemia resolved during therapy in conjunction with sodium retention and increased weight. Volume expansion and weight gain were highly correlated with decreases in the serum calcium level. Augmented urinary calcium excretion was documented in the first week of therapy and is the likely mechanism for the observed lowering of serum calcium levels. Long-term fludrocortisone therapy was not associated with signs of hyperaldosteronism and appears to be…

Histochemical investigation of aldosterone-secreting cells adenoma of the adrenal cortex.

Two cases of adenomas of adrenal cortex were examined. Both cases presented clinical symptoms of autonomic hyperaldosteronism. Histologically one adenoma had a benign structure, the other a malignant one. An ultrastructural and cytochemical assay of the benign form suggests that it is composed of so called "hybrid cells". Histochemically the cells of both adenomas were characterized by a 3-beta hydroxysteroid and succinic dehydrogenase reaction, the presence of MAO and nonspecific esterases and in some single cells 17-beta hydroxysteroid dehydrogenase.

[Antialdosterones and enzymes of the adrenal gland].

With subcellular fractions of animal adrenals, the authors have looked for the effect of antialdosterones on different aspects of adrenal metabolism (steroidogenesis, some enzymes of intermediary metabolism, mitochondrial respiration). Antialdosterones act on mitochondrial respiration by inducing an uncoupling effect. Nature of this uncoupling cannot yet by precised. Antialdosterones inhibit steroidogenesis with concentrations corresponding to blood concentrations observed during treatment with these compounds. This inhibition could explain the annulation of hyperaldosteronism secondary to sod…

Inhibition of frusemide-induced natriuresis by indomethacin in patients with the nephrotic syndrome.

1. In four patients with nephrotic syndrome indomethacin not only reduced proteinuria but also inhibited the natriuretic effect of high doses of frusemide. 2. The inhibition of natriuresis by indomethacin could not be antagonized by albumin infusions. 3. Only the combined use of spironolactone and frusemide induced a natriuresis during indomethacin treatment. Spironolactone alone was ineffective. 4. It is suggested that inhibition of prostaglandin synthesis by indomethacin, in the presence of a stimulated renin-angiotensin system and hyperaldosteronism, may cause this strong tendency to sodium…
