Spontaneous infection or vaccination as cause of acute disseminated encephalomyelitis.

This report describes an approach to determine which of 2 possible etiologies could be responsible for a disease, in this instance acute disseminated encephalomyelitis (ADEM). Information about latency periods was obtained from eighteen reference sources in the literature. Analysis of these data indicate that it would be 9-18 times more likely for ADEM to develop 5 days after a wild virus infection (measles, for example) than 28 days after a vaccination.

Association of acute disseminated encephalomyelitis (ADEM) and COVID-19 in a pediatric patient.

Cases of acute disseminated encephalomyelitis (ADEM) and its hyperacute form, acute hemorrhagic leukoencephalitis (AHLE), have been reported in coronavirus disease 2019 (COVID-19) patients as rare, but most severe neurological complications. However, histopathologic evaluations of ADEM/AHLE pathology in COVID patients are extremely limited, so far having only been reported in a few adult autopsy cases. Here we compare the findings with an ADEM-like pathology in a pediatric patient taken through a biopsy procedure. Understanding the neuropathology may shed informative light on the autoimmune pr

Does breastfeeding reduce risk for breast cancer? A short lesson in evidence-based practice.

Breastfeeding has been promoted by some healthcare providers and childbirth educators as having a protective effect against breast cancer. In an effort to use the best evidence available for teaching patients, this article was written to summarize the literature about the protective effects of breastfeeding. The studies examined consistently demonstrated a mild to moderate protective effect from breast cancer; however, many variables including age, medication use, smoking, and family history affected the risk. While nurses and childbirth educators are in key positions to disseminate informatio

ACHTSAM study protocol: outreach diagnostics and assessment of tolerability in severe ME/CFS-a pilot study.

INTRODUCTION: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a severe, multisystem condition that often emerges after viral infections and affects physical and cognitive function. Severely affected patients are underrepresented in research due to immobility and exertional intolerance. METHODS: This is a prospective, non-interventional observational study investigating the feasibility and tolerability of home-based diagnostics in patients with severe and very severe ME/CFS. Phase 1 includes remote identification using validated questionnaires. Phase 2 involves home visits for p

Disseminated cutaneous mast cell tumor in a South American coati (Nasua nasua).

Cutaneous mast cell tumors have been described in all species of domestic mammals. However, information about these tumors in free-ranging and captive wild mammals is limited, with no reports in procyonids. This report aims to describe a case of disseminated cutaneous mast cell tumor in a captive South American coati (Nasua nasua). The animal was submitted to euthanasia and necropsy. On external examination, multiple papules, nodules, and plaques were distributed over the body surface. On cut section, they were soft and white. Histologically, they were composed of dense sheets of neoplastic ce

Musculoskeletal Manifestations of Disseminated Fungal Infections.

» Disseminated musculoskeletal fungal infections, though rare, present significant diagnostic and therapeutic challenges, affecting both immunocompromised and previously healthy individuals. » Ubiquitous species (spp.) such as Candida, Aspergillus, and Cryptococcus contrast with dimorphic fungi, including Histoplasma capsulatum, Blastomyces spp., and Coccidioides spp., which are endemic to specific regions. » These infections typically present insidiously, with non-specific symptoms such as fever, joint pain, and swelling that mimic autoimmune, bacterial, or viral diseases, often leading to de

Managing Urinary Incontinence for Women in Primary Care: Environmental Scan (Base Year).

Urinary incontinence (UI) is a highly prevalent condition among women worldwide. Although effective nonsurgical treatments exist, including pharmacological, behavioral, and physical therapies, many women with the condition are never diagnosed because of a lack of information, stigma, and the absence of regular screening in primary care, and those who are diagnosed might not receive or adhere to treatment. In this study, the authors present an environmental scan of studies published from 2012 through 2022 that assess the dissemination and implementation of nonsurgical UI treatment-including scr

Herbal medicine (Gyejibongneyong-hwan) for treating primary dysmenorrhoea: a protocol for a systematic review of randomised controlled trials.

INTRODUCTION: Gyejibongneyong-hwan (GBH), also known as Guizhi Fuling formula, and is widely used for uterine fibroids in East Asian countries. Many clinical trials assessing the efficacy and safety of GBH formula for the treatment of dysmenorrhoea have been reported. This review will assess the clinical evidence for and against the use of GBH formula as a treatment for dysmenorrhoea. It will also discuss the proposed mechanism(s) that could link herbal medicine to improvements in dysmenorrhoea. METHODS AND ANALYSIS: Fourteen databases will be searched until September 2016. We will include ra

Diagnosis of multiple sclerosis: progress and challenges.

The diagnosis of multiple sclerosis is based on neurological symptoms and signs, alongside evidence of dissemination of CNS lesions in space and time. MRI is often sufficient to confirm the diagnosis when characteristic lesions accompany a typical clinical syndrome, but in some patients, further supportive information is obtained from cerebrospinal fluid examination and neurophysiological testing. Differentiation is important from other diseases in which demyelination is a feature (eg, neuromyelitis optica spectrum disorder and acute disseminated encephalomyelitis) and from non-demyelinating d

Paracoccidioidomycosis in Mexico: clinical and epidemiological data from 93 new cases (1972-2012).

Paracoccidioidomycosis (PCM) is an endemic systemic infection in several countries of Latin America. The few registered cases in Mexico most likely do not reflect the real frequency. Disseminate the epidemiological and clinical data of unreported cases of PCM in Mexico from 1972 until 2012 is the aim of this work. Epidemiological and clinical information of non-published cases of PCM was requested from the principal mycological diagnosis centres in Mexico. A total of 93 cases were received. The infection was found predominantly in men (95.7%), peasants (88.5%) and individual between 31 and 60

Congenital herpes simplex virus, type 2, bilateral endophthalmitis.

A set of dizygotic twins, both born prematurely, developed herpes simplex encephalitis shortly after birth. The second twin had unilateral keratoconjunctivitis and bilateral endophthalmitis and subsequently died from disseminated herpes infection. Herpes simplex, type 2, was isolated from conjunctiva of both eyes, cerebrospinal fluid, nasopharynx and trachea. Histologic examination of the eyes revealed bilateral endophthalmitis with a necrotizing retinitis. Intranuclear inclusion bodies were demonstrated by light microscopy in the retina, choroid and iris, and virus particles were demonstrated

Role of systems science in preventing and controlling emerging infectious diseases: protocol for a scoping review.

INTRODUCTION: In recent history, many new infectious diseases have affected humans for the first time or have appeared in previously unaffected areas of the world; these diseases are known as emerging infectious diseases (EIDs). Examples of EIDs include COVID-19, Middle East respiratory syndrome and Ebola virus disease. EIDs are known for their complexity. Multiple factors play a role in their spread, including increases in human population, conflicts, urbanisation, air travel, global trade and inequalities in wealth distribution and access to healthcare. In order to gain a better understandin

Disseminated bacille Calmette-Guérin disease in HIV-infected South African infants.

OBJECTIVE: To determine the population-based incidence of disseminated bacille Calmette-Guérin (BCG) disease in HIV-infected infants (aged <or= 1 year) in a setting with a high burden of tuberculosis and HIV infection coupled with a well-functioning programme for the prevention of HIV infection in infants. METHODS: The numerator, or number of new cases of disseminated BCG disease, was derived from multicentre surveillance data collected prospectively on infants with a confirmed HIV infection during 2004-2006. The denominator, or total number of HIV-infected infants who were BCG-vaccinated, wa

Ankle Scientific Knowledge and Physiotherapy Practice: A Thematic Analysis of Clinical Behaviors of French-Speaking Physiotherapists.

CONTEXT: Chronic ankle instability (CAI) is prevalent among individuals who sustain a lateral ankle sprain (LAS) injury. The persistence of the characteristic long-standing clinical symptoms of CAI may be attributable to the lack of adoption by physiotherapists of evidence-informed clinical guidelines. OBJECTIVE: To investigate the extent to which French-speaking physiotherapists implement the International Ankle Consortium rehabilitation-oriented assessment (ROAST) framework when providing clinical care for individuals with an acute LAS injury. DESIGN: Cross-sectional study. SETTING: Onlin

Cocreation of a digital patient health hub to enhance education and person-centred integrated care post hip fracture: a mixed-methods study protocol.

INTRODUCTION: Older people with hip fractures often require long-term care and a crucial aspect is the provision of quality health information to patients and their carers to support continuity of care. If patients are well informed about their health condition and caring needs, particularly posthospital discharge into the community setting, this may support recovery and improve quality of life. As internet and mobile access reach every household, it is possible to deliver a new model of service using a digital education platform as a personal health hub where both patients and their providers

Disseminated peritoneal leiomyomatosis: A case report.

This report describes the case of a patient in her early 40s with a history of laparoscopic myomectomy who presented to the Beijing Shunyi District Maternal and Child Health Hospital (Beijing, China) in December 2024 with menstrual disturbances and a pelvic mass. The patient had disseminated peritoneal leiomyomatosis, a rare benign neoplastic condition characterized by multifocal smooth muscle nodules on the peritoneal surface. Its nonspecific clinical and radiological features often lead to misdiagnosis as malignant peritoneal carcinomatosis. Pelvic magnetic resonance imaging revealed a 9.5-c

Human Brain Organoids: A New Model to Study Cryptococcus neoformans Neurotropism.

With the rise in immunocompromised individuals and patients with immune-related comorbidities such as COVID-19, the rate of fungal infections is growing. This increase, along with the current plateau in antifungal drug development, has made understanding the pathogenesis and dissemination of these organisms more pertinent than ever. The mouse model of fungal infection, while informative on a basic scientific level, has severe limitations in terms of translation to the human disease. Here we present data supporting the implementation of the human cerebral organoid model, which is generated from

Delirium in neonates and infants: a scoping review protocol.

INTRODUCTION: Delirium in neonates and infants is difficult to screen or assess because of their different developmental features and distinct delirium characteristics compared with those of older children. Some delirium management strategies, including assessment, pharmacological and non-pharmacological interventions, and prevention strategies, have been previously suggested for paediatric delirium. However, whether these strategies are effective for delirium in neonates and infants is unclear. This scoping review aims to explore comprehensive information on delirium in neonates and infants,

Clinical, Microbiological Profile and Treatment Outcomes of Infants with BCG Adenitis: A Retrospective Study.

BACKGROUND: Bacille Calmette-Guérin (BCG) adenitis is an uncommon complication following BCG vaccination. In rare cases, infants can develop other complications. Controversy exists regarding the diagnosis and management of these cases. Not much information is available in literature regarding their microbiological and immunological characteristics. METHODS: Electronic medical records of children presenting to the Pediatric Infectious Diseases clinic in a tertiary care hospital from January 2011-December 2020 with a diagnosis of BCG adenitis were retrospectively reviewed.
