Beta-cell replacement for type 1 diabetes: integrating cell products, engraftment and immune protection.

Beta-cell replacement aims to restore endogenous, glucose-responsive insulin secretion in type 1 diabetes. Donor-islet transplantation has shown that this approach can improve glycaemic control and prevent severe hypoglycaemia. Its wider use is limited by donor supply, early graft loss and chronic systemic immunosuppression. Pluripotent stem cells provide a renewable source of pancreatic progenitors and differentiated islet products. Recent clinical studies have reported C-peptide secretion and insulin independence in some recipients. However, cell supply is only one part of the problem. Produ

Pancreatic Islet Isolation and Purification from Lewis Rats Using Enzymatic Digestion and Density-Gradient Separation.

Pancreatic islets are essential for both clinical islet transplantation and preclinical studies aimed at understanding the mechanisms and progression of diabetes. However, progress in these fields is often limited by the availability of high-quality islets, as emerging therapeutic strategies require larger, cleaner, and more standardized preparations. This article presents a high-throughput, standardized protocol for surgical harvesting, enzymatic digestion, and density-gradient purification of pancreatic islets from Lewis rats. The method combines intraductal pancreas perfusion with a defined

Development of Potential Pharmacological Targets to Normalize Gene Expression in Islets of Type 2 Diabetic Patients.

BACKGROUND: Type 2 diabetes (T2D) is a disease of high prevalence that is expected to continue increasing despite the pharmacological treatments available; in most cases, it is difficult to control. Therefore, more research on experimental drugs is necessary to propose better treatments. OBJECTIVE: This study aimed to identify the molecular alterations of pancreatic islets in type 2 diabetes through multi-omics data integration and possible pharmacological targets using bioinformatics methods. METHODS: In this study, the OmicsNet tool was used to integrate the multi-omics data associated wit

Delta-Cell Area is Unchanged in Human Pregnancy: Evidence From Immunohistochemistry.

AIM: Pancreatic islet δ-cells produce somatostatin, a paracrine regulator of insulin and glucagon secretion within islets. Although adaptive changes in α- and β-cell populations during pregnancy have been described in both animals and humans, data on δ-cell plasticity are sparse and entirely lacking in human pregnancy. We aimed to determine whether pancreatic islet δ-cell mass undergoes morphological adaptation during human pregnancy. METHODS AND RESULTS: Formalin-fixed paraffin-embedded pancreatic tissue from pregnant (n = 7) and non-pregnant (n = 7) donors was analysed. Sections were immuno

Direct ultrastructural validation of endocrine cell identity in zebrafish pancreatic islets.

The zebrafish is widely used to study glucose homeostasis, diabetes, and endocrine cell plasticity, yet a definitive ultrastructural characterization of adult pancreatic endocrine cell types has remained incomplete. Here, we combined transmission electron microscopy with hormone-specific immunogold labeling to define the ultrastructural characteristics of the major endocrine cell populations in adult zebrafish pancreatic islets. Three principal endocrine cell types were distinguished based on secretory granule morphology, electron density, and intracellular organization. Glucagon-positive α-ce

Advances in Cell Replacement Therapies for Diabetes.

UNLABELLED: Islet cell replacement therapies have evolved as a viable treatment option for type 1 diabetes complicated by problematic hypoglycemia and glycemic lability. Refinements of islet manufacturing, islet transplantation procedures, peritransplant recipient management, and immunosuppressive protocols allowed most recipients to achieve favorable outcomes. Subsequent phase 3 trials of transplantation of deceased donor islets documented the effectiveness of transplanted islets in restoring near-normoglycemia, glycemic stability, and protection from severe hypoglycemia, with an acceptable s

LY6H is a marker of human pancreatic delta cells.

The identification of cell surface markers specific to pancreatic islet cell subsets is important for both the study of islet biology and for investigating the pathophysiology of diseases in which these cell types are lost or damaged. Analysis of publicly available single-cell RNAseq data showed that LY6H transcripts are highly enriched in the delta cells of the pancreas. This finding was confirmed using immunofluorescence analysis of histological sections of human pancreas, and flow cytometric analysis of human islet preparations. We found that expression of LY6H was robustly associated with

Effect of islet alone or islets after kidney transplantation on quality of life in type 1 diabetes: A systematic review.

BACKGROUND: Pancreatic islet transplantation for type 1 diabetes mellitus (T1DM) is efficacious in supressing severe hypoglycaemic episodes (SHE) and restoring glycaemic regulation, which are both pivotal in increasing health-related quality of life (HRQoL). Therefore, a systematic assessment of reports detailing HRQoL outcomes is warranted to better understand the benefits of islet transplantation. To this end, we performed a systematic review of the literature to assess the impact of islet transplantation on HRQoL in individuals with T1DM, whether as a standalone procedure (ITA) or following

Circulating metabolic signatures of rapid and slow progression to type 1 diabetes in islet autoantibody-positive children.

AIMS/HYPOTHESIS: Appearance of multiple islet cell autoantibodies in early life is indicative of future progression to overt type 1 diabetes, however, at varying rates. Here, we aimed to study whether distinct metabolic patterns could be identified in rapid progressors (RP, disease manifestation within 18 months after the initial seroconversion to autoantibody positivity) vs. slow progressors (SP, disease manifestation at 60 months or later from the appearance of the first autoantibody). METHODS: Longitudinal samples were collected from RP (n=25) and SP (n=41) groups at the ages of 3, 6, 12,

Interventional Radiology Approaches for Managing Postpancreatic Transplant Complications and Type 1 Diabetes Mellitus.

Type 1 diabetes mellitus (T1DM) is a chronic autoimmune disorder characterized by the destruction of insulin-secreting beta cells in the pancreas, resulting in metabolic disturbances and long-term complications. While subcutaneous insulin remains the primary approach for achieving normoglycemia, pancreatic transplantation has emerged as an effective intervention for long-standing T1DM, providing insulin independence and normalized glycosylated hemoglobin levels. However, complications associated with pancreatic transplantation are frequent, necessitating thorough evaluation using diverse imagi

Opposing roles of the entero-pancreatic hormone urocortin-3 in glucose metabolism in rats.

AIM/HYPOTHESIS: Urocortin-3 (UCN3) is a glucoregulatory peptide produced in the gut and pancreatic islets. The aim of this study was to clarify the acute effects of UCN3 on glucose regulation following an oral glucose challenge and to investigate the mechanisms involved. METHODS: We studied the effect of UCN3 on blood glucose, gastric emptying, glucose absorption and secretion of gut and pancreatic hormones in male rats. To supplement these physiological studies, we mapped the expression of UCN3 and the UCN3-sensitive receptor, type 2 corticotropin-releasing factor receptor (CRHR2), by means

Fat-soluble Vitamin Deficiency is Common in Children With Chronic Pancreatitis Undergoing Total Pancreatectomy With Islet Autotransplantation.

The prevalence of fat-soluble vitamin (FSV) deficiency in children undergoing total pancreatectomy with islet autotransplantation (TPIAT) for chronic pancreatitis (CP) is unknown. We quantified FSV deficiency in 100 children (age ≤18) undergoing TPIAT. FSV levels (vitamins A, E, D) and clinical history were abstracted from medical records. Vitamin A was low in 4% before and 7% at 1 year after TPIAT, vitamin E in 17% and 18%, and vitamin D in 22% and 24%, respectively, regardless of pancreatic enzyme or vitamin supplement dosing. Longer duration of CP was associated with pre-TPIAT vitamin D ins

Evidence for the existence and potential roles of intra-islet glucagon-like peptide-1.

Glucagon-Like Peptide-1 (GLP-1) is an important peptide hormone secreted by L-cells in the gastrointestinal tract in response to nutrients. It is produced by the differential cleavage of the proglucagon peptide. GLP-1 elicits a wide variety of physiological responses in many tissues that contribute to metabolic homeostasis. For these reasons, therapies designed to either increase endogenous GLP-1 levels or introduce exogenous peptide mimetics are now widely used in the management of diabetes. In addition to GLP-1 production from L-cells, recent reports suggest that pancreatic islet alpha cells

Measurement of Insulin Secretion Using Pancreas Perfusion in the Rodent.

Under in vivo conditions, the study of physiological and pharmacological functions of an organ is difficult due to whole-body interactions with the organ. Thus, an in vitro technique for the perfusion of isolated pancreata was developed for physiologic and response studies including the investigation of endocrine function and secretory responsiveness under a variety of diabetes-associated conditions. The pancreas is isolated from the connecting spleen, stomach, and duodenum and transferred to a pre-warmed chamber where it is perfused in isolation from all other organs. A detailed description o

Portal flow dynamics after total pancreatectomy and autologous islet cell transplantation.

BACKGROUND: Portal vein thrombosis (PVT) is a serious complication of total pancreatectomy and autologous islet cell transplant (TPAIT); therefore, portal flow dynamics are monitored by Doppler ultrasound postoperatively. The practical value of scheduled Doppler ultrasound examinations and the relationship between portal vein velocity, liver function, and complications have not been clearly studied. METHODS: A retrospective review of 16 TPAIT was performed. Correlation analysis of portal vein velocity with indices of liver function, portal pressure, and volume of islet cells infused was condu

Application of Single-Cell RNA Sequencing in Pancreatic Cancer and the Endocrine Pancreas.

The pancreas is a complex organ composed of an endocrine (pancreatic islets) and an exocrine portion. This mixed cell population has resulted in an implacable barrier to exploring the detailed mechanism and function of each cell type in previous investigative approaches. In recent years, single-cell RNA sequencing (scRNA-seq) technologies have provided in-depth analysis of cell heterogeneity in the pancreas and in pancreatic cancer. It is especially effective in cell-type-specific molecule identification and detection of interactions between cancer cells and the stromal microenvironment. To da

INFLUENCE OF TESTOSTERONE ON MORPHOLOGICAL CHANGES IN PANCREATIC ISLETS AND LIVER TISSUE OF RATS WITH ALLOXAN-INDUCED DIABETES.

The role of exogenous testosterone (methyltrienolone) in the repair of damaged pancreatic islet and liver cells during alloxan diabetes was studied. Modeling of diabetes was performed through single intraperitoneal injection of Alloxan, with the dose 2 mg per 10 g of body weight. Pancreatic and liver tissues were investigated microscopically using 3-4 µm slices embedded in paraffin and stained with hematoxylin and eosin. Software Adobe Photoshop was used to measure the areas of pancreatic islets as well as areas of necrotic foci in these islets of animals with Alloxan-induced experimental diab

An immunohistochemical study of the endocrine pancreas in raptors.

The cytoarchitecture of the endocrine pancreas of 10 raptors (golden eagles, peregrine falcons, Saker falcon, turkey vultures, red-tailed hawk and unspecified falcon) was examined by immunohistochemistry. Three islet types were identified: type A mixed islets composed mainly by glucagon (A)-secreting cells, type B mixed islets with predominantly insulin (B)-secreting cell component and type M mixed islets (type M) consisting of variable number of glucagon-, insulin- and somatostatin (D)-secreting cells. The latter were further characterized into Type I, II or III according to the cell distribu
