PURPOSE OF REVIEW: The number of displaced people worldwide continues to rise because of conflict and disasters-related crises. Kidney disease is increasingly recognized as an important contributor to morbidity among displaced populations, yet it remains under-prioritized in humanitarian responses. This review summarizes current evidence on the burden, risk factors, barriers, and management of kidney disease in displaced settings.
RECENT FINDINGS: Recent literature highlights multiple risk factors linking displacement and kidney disease. Acute kidney injury may result from trauma, dehydration…

Cancer is the second leading cause of death in kidney transplant recipients, after cardiovascular diseases. These patients are at least twice as likely to develop cancer or die from a cancer as the general population. This increased risk is multifactorial and mainly attributed to oncogenic viruses and immunosuppression. Transplant candidates and kidney donors should be screened for cancer as part of the evaluation process. Waiting periods after remission are recommended, depending on the type of cancer and stage. Cancer screening after transplantation should be tailored to each patient. If a c…

Renal disease is a frequent complication of symptomatic multiple myeloma, that increases morbidity and reduces quality of life and overall survival. It may result from various lesions, the most frequent being light chain cast nephropathy (LCCN), related to precipitation of monoclonal free light chains (FLC) with uromodulin in distal tubules. Rapid identification of the type of kidney disease with appropriate management is key. LCCN typically reveals the underlying myeloma and manifests with severe acute kidney injury, high serum FLC level (>500 mg/l) and predominant light chain proteinuria (ur…

Chrononutrition, the study of the interaction between biological rhythms and nutrition, has emerged as a promising field for addressing metabolic health. However, its role in chronic kidney disease (CKD) remains underexplored. CKD patients often experience circadian disruptions due to renal, metabolic, treatment-related, and lifestyle factors, which may influence their nutritional status and clinical outcomes. Objective: to synthesize and analyze the existing evidence on chrononutrition in CKD patients, identify knowledge gaps, and propose directions for future research across different stages…

Chronic kidney disease (CKD) affects approximately 7% of the German population, leading to increased morbidity, mortality and subjective symptoms that significantly impact quality of life. In Germany, approx. 86000 individuals rely on renal replacement therapies with a median waiting time of nearly eight years for a kidney transplant. The initiation of dialysis often results in limitations of functional and physical capabilities, as well as reduced participation in daily activities. The development of frailty during this period is a strong negative predictor of mortality. Thus, patients with C…

The term immunosenescence refers to the collective effects of aging on the immune system, which involves both the innate and the adaptive im … n that occurs, among others, in allotransplantation due to constant exposure to donor alloantigens. Kidney transplantation is the preferred treatment for all patients with end-stage renal disease, yet it places the immune system in a unique and aberrant state. On one hand, immunosenescence could be an important determinant of tolerance to the transplant. …

The kidney proximal tubule reabsorbs and degrades filtered plasma proteins to reclaim valuable nutrients and maintain body homeostasis. Defects in this process result in proteinuria, one of the most frequently used biomarkers of kidney disease. Filtered proteins enter proximal tubules via receptor-mediated endocytosis and are processed within a highly developed apical endo-lysosomal system (ELS). Proteinuria is a strong risk factor for chronic kidney disease progression and genetic disorders of the ELS cause hereditary kidney diseases, so deepening understanding of how the proximal tubule hand…

Among individuals with kidney disease hospitalization rates are up to 38% higher than the general population, increase proportionally with each stage of CKD, and significantly contribute to over $130 billion in annual care costs in the United States (US). Many hospitalizations, including readmissions, are potentially preventable, and their avoidance could substantially lessen the burden and cost of kidney disease. Across the spectrum of types of kidney disease, shared drivers of preventable hospitalizations include clinical complexity and difficulty with self-management at the patient level; c…

The nuanced landscape of anticoagulation therapy in patients with chronic kidney disease (CKD) presents a formidable challenge, intricately balancing the dual hazards of hemorrhage and thrombosis. These patients find themselves in a precarious position, teetering on the edge of these risks due to compromised platelet functionality and systemic disturbances within their coagulation frameworks. The management of such patients necessitates a meticulous approach to dosing adjustments and vigilant monitoring to navigate the perilous waters of anticoagulant therapy. This is especially critical consi…

Drug-induced kidney disease (DIKD) is a frequent cause of acute and chronic kidney disease (CKD) that leads to high morbidity, hospitalization, and increased healthcare costs. There is a need to constantly update our knowledge in this field, given the ever-burgeoning list of newer treatments that are emerging, especially in the field of cancer immunotherapy. Generalizing the complex pathways causing DIKD from different agents, the common mechanisms include direct toxicity, immune-mediated injury, and drug-induced alterations in renal blood flow. Proper management of this condition involves ris…

Acute kidney injury (AKI) is a multifactorial syndrome with a high risk of short- and long-term complications as well as increased health care costs. The traditional biomarkers of AKI, serum creatinine and urine output, have important limitations. The discovery of new functional and damage/stress biomarkers has enabled a more precise delineation of the aetiology, pathophysiology, site, mechanisms, and severity of injury. This has allowed earlier diagnosis, better prognostication, and the identification of AKI sub-phenotypes. In this review, we summarize the roles and challenges of these new bi…

Chronic kidney disease (CKD) affects approximately 14% of adults in the United States and is present in at least 10% of the population worldwide. Blood glucose and blood pressure control are imperative to adequately manage CKD as they are the only primary prevention measures for the condition. Recent changes in CKD evaluation and medication therapies that modify disease progression and aid in managing complications such as anemia of CKD have emerged, including a newly approved mineralocorticoid receptor antagonist and hypoxia-inducible factor-prolyl hydroxylase inhibitor, respectively. This fo…

In kidney transplantation, ongoing alloimmune processes-commonly triggered by HLA incompatibilities-can trigger chronic transplant rejection, affecting the microcirculation and the tubulointerstitium. Continuous inflammation may lead to progressive, irreversible graft injury, culminating in graft dysfunction and accelerated transplant failure. Numerous experimental and translational studies have delineated a complex interplay of different immune mechanisms driving rejection, with antibody-mediated rejection (AMR) being an extensively studied rejection variant. In microvascular inflammation, a …

The number of patients on the waiting list for a kidney retransplant has increased. Patients who are candidates for a second kidney transplant often have higher levels of PRA (Panel of Reactive Antibodies). The previous failed kidney transplant is one of the main factors that leads to the production of antibodies against human leukocyte antigens ‒ HLA. The consequences of sensitisation are a long waiting time for repeated kidney transplantation and a negative effect on graft survival after retransplantation. The aim of our analysis was to evaluate the immunological parameters of patients underg…

BACKGROUND: Antiphospholipid syndrome (APS) is an autoimmune disease characterized by the development of autoantibodies and the impairment of the coagulation system. Knowledge about this syndrome is increasing over time, but kidney involvement, especially APS nephropathy, still represents a challenge for physicians.
SUMMARY: A "two hit" model has been hypothesized to explain APS pathophysiology, and the role played by some factors, such as the complement system, is becoming more and more clear. From a clinical point of view, along with thrombosis in any site and/or obstetric morbidities, that…

Arterial hypertension (HTN) in children after kidney transplantation is an important risk factor not only for graft loss but also for cardiovascular morbidity and mortality. The prevalence of posttransplant HTN ranges between 60% and 90%. The etiology of posttransplant HTN is multifactorial and includes residual chronic native kidney disease, immunosuppressive therapy, and chronic allograft dysfunction among other causes. Clinic blood pressure (BP) should be measured at each outpatient visit. However, ambulatory blood pressure monitoring (ABPM) is the gold standard method for BP evaluation in …

Melatonin is a neurohormone that is mainly secreted by the pineal gland. It coordinates the work of the superior biological clock and consequently affects many processes in the human body. Disorders of the waking and sleeping period result in nervous system imbalance and generate metabolic and endocrine derangements. The purpose of this review is to provide information regarding the potential benefits of melatonin use, particularly in kidney diseases. The impact on the cardiovascular system, diabetes, and homeostasis causes melatonin to be indirectly connected to kidney function and quality of…
