Antepartum use of Epi-No birth trainer for preventing perineal trauma: systematic review.

INTRODUCTION AND HYPOTHESIS: In this systematic review we aimed to assess if the Epi-No birth trainer used during antepartum could prevent perineal trauma in nulliparous women. METHODS: We searched CENTRAL, MEDLINE, EMBASE, Scielo, and Conference abstracts, looking for randomized controlled studies (RCT). High heterogeneity (i(2) > 50 %) was corrected with random models. All studies were analyzed according to their quality and risk of bias. Nulliparous women or women whose previous pregnancy ended before 21 weeks' gestation were included and the main outcome measures were: episiotomy rates, p

[Incidence of gestational diabetes and relationship to obesity in Chilean pregnant women].

BACKGROUND: Gestational diabetes (GDM) is associated to a worse outcome of pregnancy. This justifies efforts for finding possible causes of GDM that would allow implementing preventive interventions. AIM: To study incidence of GDM and its relation with obesity and other traditional risk factors. MATERIAL AND METHODS: A retrospective study was performed in 234 women who had delivered a singleton during the last 12 months, attended in an outpatient clinic in Santiago, Chile. Familiar and personal history, body mass index (BMI), obstetrical-related pathology and data about the labor and the new

Maternal risk factors for postpartum readmission following vacuum-assisted vaginal delivery: A retrospective cohort study.

OBJECTIVE: To identify risk factors associated with postpartum readmission following vacuum-assisted vaginal delivery. METHODS: A retrospective cohort study at a single university-affiliated tertiary medical center from 2012 to 2023, including all singleton pregnancies delivered at ≥37 weeks' gestation. Women who underwent a vacuum-assisted vaginal delivery (VAD) trial were identified. Multiple gestations, nonviable fetuses, and failed VAD requiring emergency cesarean delivery (CD) were excluded. Participants were categorized as readmitted or not readmitted within 6 weeks postpartum for obste

Trends in postpartum hemorrhage, associated risk factors, and severe maternal outcomes in a multihospital health system, 2016-2023: a retrospective cohort study.

BACKGROUND: Reported increases in postpartum hemorrhage (PPH) raise concerns for maternal risk and resource needs, but contemporary multicenter trends and severe outcomes remain uncertain. OBJECTIVE: To quantify trends in PPH and severe PPH and identify factors associated with PPH to inform hemorrhage surveillance and preparedness. STUDY DESIGN: Retrospective cohort of all deliveries across eight hospitals (2 tertiary, 6 community) in a U.S. health system (2016-2023). PPH was defined as blood loss ≥1000 mL or red blood cell transfusion within 24 hours, and severe PPH as PPH plus ≥1 severe mo

Is episiotomy worthwile to prevent obstetric anal sphincter injury during operative vaginal delivery in nulliparous women?

OBJECTIVES: Episiotomy is a marker of Obstetric Anal Sphincter Injury (OASIS) condition, therefore, unmeasured factors could have biased the strength of the association between episiotomy and reduced OASIS during Operative Vaginal Delivery (OVD). The aim of this study was to compare the OASIS rate during OVD according to episiotomy practice. STUDY DESIGN: Retrospective cohort study of all nulliparous pregnant women attempting an OVD between 2014-2017. To avoid unmeasured bias, all maternal and delivery data were prospectively captured after the birth. The strong relationship between parity an

The outcomes of midline versus medio-lateral episiotomy.

BACKGROUND: Episiotomy is the surgical enlargement of the vaginal orifice by an incision of the perineum during the second stage of labor or just before delivery of the baby. During the 1970s, it was common to perform an episiotomy for almost all women having their first delivery, ostensibly for prevention of severe perineum tears and easier subsequent repair. However, there are no data available to indicate if an episiotomy should be midline or medio-lateral. We compared midline versus medio-lateral episiotomy for complication such as extended perineal tears, pain scores, wound infection rate

Obstetric factors associated with levator ani muscle injury after vaginal birth.

OBJECTIVE: To identify obstetric factors associated with development of levator ani injury after vaginal birth. METHODS: Magnetic resonance images were taken of the pelvic floor of 160 women 9 to 12 months after first term vaginal delivery. Half the women had de novo stress incontinence and half were continent controls. Abnormalities of the pubovisceral portion were identified on magnetic resonance as present or absent. Defect severity was further scored in each muscle from 0 (no defect) to 3 (complete muscle loss). A summed score for the 2 sides (0 to 6) was assigned and grouped as minor (0-

Deep Learning-Based Assessment of Sleepiness from Fingertip Pulse Wave Analysis During Objective Daytime Testing.

PURPOSE: Objective evaluation of daytime sleepiness is a key factor in the management of sleep disorders. However, objective testing is limited due to the labor-intensive nature of standard methods; the Multiple Sleep Latency Test (MSLT) and the Maintenance of Wakefulness Test (MWT). Deep learning analysis of a simple fingertip pulse wave (photoplethysmography, PPG) recording could be a viable option for the routine assessment of sleepiness in patients with suspected sleep disorders. METHODS: A deep learning-based sleep staging model was tested in MSLT (n = 143) and MWT (n = 127) datasets. Th

Management of cord and placental blood and its effect upon the newborn.

A comparative study was made of erythrocyte counts and weights of the newborn at term. Three groups were used: Cases in which the cord was clamped at once, those in which the cord was allowed to pulsate five minutes, and those in which the cord and placental blood was stripped into the baby. Standards and procedure were set up so that there would be a minimum of error. Evidence was elicited showing that babies in the "pulsating" and the "stripped" groups received a significant amount of blood which was beneficial. The amount varied, but when the stripping method was used, the term baby receive
