Strengthening laboratory surveillance of viral pathogens: Experiences and lessons learned building next-generation sequencing capacity in Ghana.

OBJECTIVE: To demonstrate the feasibility of applying next-generation sequencing (NGS) in medium-resource reference laboratories in Africa to enhance global disease surveillance. METHODS: A training program was developed to support implementation of NGS at Noguchi Memorial Institute for Medical Research (NMIMR), University of Ghana. The program was divided into two training stages, first at the Centers for Disease Control and Prevention (CDC) in Atlanta, GA, followed by on-site training at NMIMR for a larger cohort of scientists. RESULTS: Self-assessment scores for topics covered during the

[Molecular Targeted Therapy and Laboratory Tests].

Molecular targeted therapy is medical treatment targeting specific molecules, which are essential in the pathology of diseases. Most agents used are monoclonal antibodies ("biologics") and low-molecular-weight compounds. Molecular targeted therapy is widely utilized against malignancies, such as imatinib for chronic myelogenous leukemia blocking BCR-ABL tyrosine kinase, gefitinib for non-small cell lung cancer interrupt- ing signal transduction through EGFR, and trastuzumab for HER2-positive breast cancer. It is a companion diagnostic used as a companion to a molecular targeted drug to determi

Safe laboratory handling of Neisseria meningitidis.

Transmission of Neisseria meningitidis mainly occurs by formation of aerosols generated in the nasopharynx. Working with meningococci in vitro and performing manipulations where aerosols may be generated may result in laboratory acquired infections if appropriate safety precautions are not taken. This review details the practical aspects and experience of safe working with N. meningitidis in the laboratory. The specific risk factor for laboratory-acquired infection is exposure to aerosols containing meningococci. Prevention should therefore focus on the use of microbiology safety cabinets duri

A review of self-testing for HIV: research and policy priorities in a new era of HIV prevention.

Inadequate uptake of testing for human immunodeficiency virus (HIV) remains a primary bottleneck toward universal access to treatment and care, and is an obstacle to realizing the potential of new interventions for preventing HIV infection, including treatment for prevention and preexposure prophylaxis. HIV self-testing offers an approach to scaling up testing that could be high impact, low cost, confidential, and empowering for users. Although HIV self-testing was first considered >20 years ago, it has not been widely implemented. We conducted a review of policy and research on HIV self-testi

Legionnaires' disease: a rational approach to therapy.

Optimal therapy against Legionella infection is based on agents with a high intrinsic activity, an appropriate pharmacokinetic and pharmacodynamic profile (including the ability to penetrate phagocytic cells), a low incidence of adverse reactions and an advantageous cost-efficacy relationship. Newer macroazalides and fluoroquinolones are among the first-line therapies and in severe infections, particularly those occurring in immunocompromised patients, azithromycin and later fluoroquinolones are the agents of choice. Delay in the onset of adequate therapy is a key factor associated with a poor

Prostatitis and urinary tract infection in men: what's new; what's true?

Urinary tract and prostatic infections are common in men, and most are treated by primary providers. Acute bacterial prostatitis is caused by uropathogens, presents with a tender prostate gland, and responds promptly to antibiotic therapy. Chronic bacterial prostatitis is a subacute infection, may present with a variety of pelvic pain and voiding symptoms, and is characterized by recurrent urinary tract infections. Effective treatment may be difficult and requires prolonged antibiotic therapy. Nonbacterial prostatitis and chronic pelvic pain syndrome are more common than bacterial prostatitis,

Creating a national service for the diagnosis of meningococcal disease by polymerase chain reaction.

The widening gap observed between numbers of culture confirmed and notified cases of meningococcal infection has driven the development of non-culture based polymerase chain reactions (PCRs) to enhance the detection of meningococcal disease. The development of a national PCR-based service increased the number of laboratory confirmed cases of meningococcal disease by 35% in the first year. Advancing technology in the laboratory is reducing in-lab processing time.

Q fever pneumonia.

Pneumonia is one of the principal manifestations of Q fever, a disease caused by Coxiella burnetii. This bacterium can replicate only within cells, yet it is capable of surviving in the environment because it can withstand drying and substantial temperature variations. Livestock, especially sheep, goats and cattle, are a major reservoir of C burnetii. The organism is transmitted to humans by direct contact with animal products, especially during parturition. Aerosols have transmitted infection over considerable distances. The illness is characterized by an influenza-like syndrome with patchy p

[Vaccination against mouse pox].

Attenuated MVA-strain of vaccinia virus has been efficient in the control of enzootic mousepox and in prophylactic vaccination. The virus has been used as a live vaccine for prophylactic and emergency vaccinations as well as for sanitation of populations. More than 100 000 vaccinations were carried out safely. Even after suspension of the obligatory vaccination of humans against smallpox the MVA-vaccine can be employed without risk and danger.

Campylobacteriosis--a "new" disease in laboratory animals.

Campylobacter fetus ss jejuni is now recognized as a leading cause of diarrhea in humans. With the development of proper culture techniques, C jejuni has been isolated from humans with diarrhea as frequently as Salmonella or Shigella species. Laboratory animal models are being developed to study the pathogenesis of campylobacter-induced diarrhea; also, identification of C jejuni is occurring more frequently in a variety of laboratory animals, whether asymptomatic carriers or those with clinical diarrhea. Animals shedding C jejuni also have been incriminated as a source of campylobacter diarrhe

Leveraging PEPFAR- and Global Fund-Supported Laboratory Network for Integrated Disease Testing: Lessons From Integrated HIV, Tuberculosis, and COVID-19 Testing in Nigeria.

The Nigeria Centre for Disease Control and Prevention, as part of the strategic approach to the COVID-19 outbreak response in Nigeria, coordinated the implementation of an integrated disease testing (IDT) strategy with support from multiple partners. This involved adaptation of existing US President's Emergency Plan for AIDS Relief (PEPFAR)- and Global Fund-supported molecular laboratory networks and integration of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) testing into these. This enabled the country to provide large-scale testing and rapid return of results for SARS-CoV-2 f

Molecular mechanisms of acquired antifungal drug resistance in principal fungal pathogens and EUCAST guidance for their laboratory detection and clinical implications.

The increasing incidence and changing epidemiology of invasive fungal infections continue to present many challenges to their effective management. The repertoire of antifungal drugs available for treatment is still limited although there are new antifungals on the horizon. Successful treatment of invasive mycoses is dependent on a mix of pathogen-, host- and antifungal drug-related factors. Laboratories need to be adept at detection of fungal pathogens in clinical samples in order to effectively guide treatment by identifying isolates with acquired drug resistance. While there are internation

Evaluating the preservation and isolation of stool pathogens using the COPAN FecalSwab™ Transport System and Walk-Away Specimen Processor.

The isolation of stool pathogens is difficult due to their fastidious nature and the rapid overgrowth of fecal flora. In this study, we evaluate the preservation and isolation of enteric pathogens from stool using the automated COPAN Walk-Away Specimen Processor (WASP®) in conjunction with FecalSwab™ and selenite media. Pathogen viability and fecal commensal abundance were stable in FecalSwab™ media under both room-temperature and refrigerated incubation conditions, resulting in a significantly increased number of well-isolated pathogen colonies observed when compared to samples incubated in m

A comparison of weather variables linked to infectious disease patterns using laboratory addresses and patient residence addresses.

BACKGROUND: To understand the impact of weather on infectious diseases, information on weather parameters at patient locations is needed, but this is not always accessible due to confidentiality or data availability. Weather parameters at nearby locations are often used as a proxy, but the accuracy of this practice is not known. METHODS: Daily Campylobacter and Cryptosporidium cases across England and Wales were linked to local temperature and rainfall at the residence postcodes of the patients and at the corresponding postcodes of the laboratory where the patient's specimen was tested. The p
