BACKGROUND: Patients with Meniere's disease (MD) receive treatment to reduce vertigo.
PURPOSE: To explore the fluctuation of vertigo symptoms and the changes in endolymphatic hydrops (EHs) in MD patients during long-term regular medication.
MATERIALS AND METHODS: We enrolled MD patients who had received two magnetic resonance imaging with intravenous contrast agents.
RESULTS: Of the 42 patients in the study, 18(42.9%) had progressive EHs and 3(7.1%) had remission. The change value of the signal intensity ratio (SIR; cochlear perilymph/cerebellum) on the affected side (1.2) was larger than t

OBJECTIVE: To present a case of an iatrogenic inner ear third window after vestibular schwannoma microsurgery.
PATIENTS: A 42-year-old male presented 9-months after left-sided retrosigmoid approach for an intracanalicular vestibular schwannoma with hearing-preservation attempt performed elsewhere. Immediately postoperatively, he developed the following disabling and persistent symptoms on the ipsilateral side: autophony, pulsatile tinnitus, high-pitched ringing tinnitus, and hearing his footsteps. He denied vertigo. Otoscopy was normal. Tuning fork (512-Hz) lateralized to the left and Rinne w

The aim of the present study was to evaluate the severity of vestibular drop attack (VDA) in Ménière's disease (MD) and to examine the association between VDA severity and other MD-related complaints. The study used a cross-sectional survey design using an electronic questionnaire. The mean age of participants was 56.7 years, and the mean duration of MD was 12.4 years. Four categories of VDA were identified based on level of severity. VDA occurred in 305 (50.7%) of the 602 patients. Of these, 133 patients (22%) experienced mild VDA (i.e., associated with tripping); 80 (13%) experienced moderat

Dizziness is a common symptom encountered by all physicians. Dizziness and related symptoms are frequently linked to 1 or more of a list of contributors that includes benign paroxysmal positional vertigo, Ménière disease, migraine, acute peripheral vestibulopathy, cerebral ischemia, and anxiety disorders. Awareness of these common clinical patterns increases the likelihood of making a rapid, accurate diagnosis.

The vestibular system is a small bilateral structure located in the inner ear, known as the organ of balance and spatial orientation. It senses head orientation and motion, as well as body motion in the three dimensions of our environment. It is also involved in non-motor functions such as postural control of blood pressure. These regulations are mediated via anatomical projections from vestibular nuclei to brainstem autonomic centers and are involved in the maintenance of cardiovascular function via sympathetic nerves. Age-associated dysfunction of the vestibular organ contributes to an incre

The medical records of five cats that were diagnosed with otitis media and soft palate abnormalities, three of which had concurrent otitis interna, were reviewed retrospectively. The animals presented with unilateral or bilateral otitis media or otitis interna associated with soft palate hypoplasia (four cases) or unilateral soft palate cleft (one case). Otitis media was confirmed by radiography, CT or MRI. The soft palate abnormalities present were discovered on oropharyngeal examination at induction of anaesthesia. These five cases provide additional support of a link between otitis media an

Because the blood supply to the inner ear originates from the vertebrobasilar system, vertebrobasilar ischemic stroke can present with vertigo and hearing loss due to infarction of the inner ear (i.e., labyrinthine infarction). Sometimes vertigo and hearing loss are warning symptoms of impending vertebrobasilar ischemic stroke (mainly in the anterior inferior cerebellar artery territory). In this case, the magnetic resonance imaging (MRI) scan is normal and the clinician must rely on other clinical features to make the diagnosis. Here the authors review the keys to the diagnosis of vertigo and

INTRODUCTION: Motion sickness is a recurrent problem in pilot schools. Many techniques to overcome motion sickness are used worldwide, mostly including sedative medication and psychotherapy. Motion sickness does not correlate with future pilot skills: it should not be a criterion for selection of candidates. The problem must be controlled from the onset in an objective way. In the Swiss Air Force selection program, time compression does not allow any delay between the selection flights, nor can drugs be used.
METHODS: We created an after-flight motion sickness checklist. We present a descript

No widely accepted protocol or guideline exists for monitoring ototoxicity in patients who take powerful and potentially cochleotoxic and/or vestibulotoxic agents. Many physicians in other specialties who prescribe these drugs do not understand the important role of otolaryngologists and audiologists in pretreatment counseling and evaluation and the need for follow-up assessments of their patients' auditory function. Based on our combined experience of more than 50 years, we have developed a uniform yet flexible approach to monitoring cochlear and vestibular function in these patients. We disc

Auditory neuropathy is characterized by mild-to-moderate pure-tone hearing loss, poor speech discrimination out of proportion with this loss, absent or abnormal auditory brainstem responses and normal outer hair cell function as measured by otoacoustic emissions and cochlear microphonics. We followed three patients in our clinic whom we classified as auditory neuropathy patients. These patients also complained of balance disorders and we report our auditory and vestibular system analyses of these patients. The data presented herein include results of audiometric tests (serial pure-tone audiome

Labyrinthine fistula is a complication of chronic otitis media with cholesteatoma. Canal wall down tympanoplasty with partial mastoid cavity obliteration and complete removal of cholesteatoma matrix was adopted in five cases of labyrinthine fistula. The operation resulted in elimination of vertigo. Sensory hearing has been saved in two cases. Air conduction hearing improved in one case. In three cases whose air conduction deteriorated post-operatively, bone conduction worsened in two cases and remained unchanged in one case (Tympanoplasty type 0 was performed in the other one case.) During the

BACKGROUND: Dysplasias of the bony labyrinth are frequently associated with cerebrospinal fluid fistula and are usually discovered because of recurrent meningitis.
CASE REPORT: A 1 year-old infant was admitted for a pneumococcal meningitis which appeared 2 days after the occurrence of a clear otorrhea from the right ear. The same organism was isolated from the otorrhea fluid, which also contained cerebrospinal fluid as confirmed cytochemically. The meningitis rapidly resolved with antibiotic treatment. Auditory brain stem responses were abolished from the right ear. CT of the temporal bones s

Toluene is a widely used organic solvent, heavily employed in many manufacturing industries. Recently, evidence has begun to accumulate on the deleterious effect of toluene exposure has on the auditory and vestibular systems. Although little published information exists regarding these effects, the reported findings indicate a need for further investigation. The results of such investigations may dramatically affect occupational hearing conservation practices and legislation. Both human and animal studies will be summarized in discussing the effects of toluene alone or in combination with nois

A growing body of evidence supports the idea that dizziness that persists for months and even years can be caused by an unsuspected perilymphatic fistula. Perilymphatic fistulas are abnormal ruptures that allow perilymph to leak out of the inner ear into the middle ear space. Most commonly, these ruptures occur secondary to a traumatic event. The term postconcussive syndrome has been used to describe a myriad of symptoms following head trauma. Some of these symptoms, such as cognitive changes, tinnitus, neck stiffness, and dizziness, are also commonly caused by active perilymphatic fistulas. T

The coils of the cochlea are now readily demonstrated by thin-section high-resolution computed tomography (CT) in the axial plane. This assessment has become important in severely deaf patients who are candidates for cochlear implant surgery. The commonest abnormality shown in these cases is post-meningitic or tympanogenic labyrinthitis ossificans, which can obstruct the passage of the electrode. Severe otosclerosis can cause a similar problem. Congenital deformities of the labyrinth are rarely suitable for implantation, but the procedure may be indicated for the true Mondini deformity. Natural antibodies against laminin (ALA) have been detected recently in some acute and chronic infectious disorders. In our present study sera of a large number of patients with inner ear disorders of unknown etiology (n = 413) were tested for ALA. Control sera were taken from patients with diseases of known infectious etiology and patients with diseases of unknown etiology in which an infectious etiology was suggested. Patients with classical autoimmune diseases as well as healthy blood donors were also included in this study. The highest frequency of ALA was found in patients with sensorineu
